Market‐oriented changes within the NHS in the last 15 years have increasingly created difficulties with funding of referrals to tertiary psychotherapy health providers. The often‐cumbersome, time‐consuming and expensive bureaucratic procedures now required by the Primary Care Trusts to fund patients referred to tertiary care services leads to lengthy delays in the pathway from assessment to treatment. This paper describes a systematic survey of funding and administrative delays endured by the 23 patients referred on a cost‐per‐case basis to the Outreach Personality Disorder service at the Cassel Hospital over a 1‐year period. The results of the survey found that the median administrative delay patients endured between referral to treatment funding decision was 141 days. Two cases are presented to illustrate the difficulties encountered by patients who have to go through several obstacles before they can obtain the treatment they need. The implications of NHS reforms for the future of socialized healthcare in general and for psychotherapy tertiary care service provision in particular are highlighted in the discussion.  相似文献

Both theoretical models and functional imaging studies implicate the involvement of emotions within the delay discounting process. However, defining this role has been difficult to establish with neuroimaging techniques given the automaticity of emotional responses. To address this, the current study examined electrophysiological correlates involved in the detection and evaluation of immediate and delayed monetary outcomes. Our results showed that modulation of both early and later ERP components previously associated with affective stimuli processing are sensitive to the signalling of delayed rewards. Together with behavioural reaction times that favoured immediacy, we demonstrated, for the first time, that time delays modify the incentive value of monetary rewards via mechanisms of emotional bias and selective visual attention. Furthermore, our data are consistent with the hypothesis that delayed and thus intangible rewards are perceived less saliently, and rely on emotion as a common currency within decision making. This study provides a new approach to delay discounting and highlights a potential novel route through which delay discounting may be investigated.  相似文献

This article draws a parallel between personal construct theory and intuitionistic logic i, in order to account for Kelly's claim to have departed from classical logic. Assuming that different theoretical paradigms correspond to different logical languages, it is argued that the constructivist paradigm is linked to intuitionism. Similarities between some key syntactic and semantic features of i logic and the underlying logic of Kelly's theory are made explicit. The strengths and limitations of such an approach are discussed in light of issues emerging from clinical observation and from the philosophy of science.  相似文献

In this paper I look at dynamic mental representations, motion detection under conditions of certainty or uncertainty, perceptual adaptation, and priming of motion direction. The goal is to bridge the boundaries created in part by the use of different terminology within different literatures. The most fruitful parallel may be between the phenomenon of dynamic mental representation and representational momentum on the one hand, and perceptual adaptation as revealed by motion priming on the other. In two studies, we empirically investigated associations among self-leadership (with subfacets) and the full range leadership model (transformational, transactional, laissez-faire leadership) in N = 447 professionals with leadership experience (Study 1) and N = 35 leaders with N = 151 followers (Study 2). In both studies, leaders' self-leadership was positively associated with active styles of leading (transformational and transactional leadership) and negatively with passive styles of leading (laissez-faire leadership), as indicated by self- and follower-reports. These relationships remained even after controlling for sex, age, and leadership experience. Overall, data support the notion that effectively leading oneself is associated with effectively leading others.  相似文献

Research into mental illness and its relationship with violence has been constrained by inconsistencies in the definition and measurement of violent behavior. We conducted a systematic literature search of Scopus, EMBASE, PsycINFO, and Ovid Medline with search terms relating to the measurement, rating and quantification of violent behavior in mentally disordered populations. We identified nine tools designed to assess violence and critically evaluated them. Broadly, measurement tools tended to focus on multiple, but different, facets of violence, which included: severity of act, severity of outcome, frequency and intent, with each suggested as a valid outcome measure for violent acts. The use of multiple sources of information to inform assessment appears to provide detail; however, that detail is then often diluted as a result of dichotomization of sample groups. This presents methodological challenges for the field. Future studies should give consideration to the trade-off between preserving the richness of data and the difficulties associated with recruiting large patient samples. Studies should move from simply defining violence towards quantification across different dimensions of violence and using multiple sources of information.  相似文献

Starting out with the idea that everybody is in principle capable of enterprising behaviour, the concept of enterprise is applied to non-business people as well as business people. A random sample of people is asked to furnish an example of enterprising behaviour that they have shown themselves. Further, they are asked what is enterprising about this behaviour. The results show that in different realms of enterprising behaviour different dimensions of enterprise are mentioned. For example, people giving an example from their leisure time regard merely being active as enterprising, employees mention risky elements in their work, business owners emphasize autonomy, and students regard it as enterprising that they work for their professional or personal development. So people can be enterprising in different areas of life and in different manners. Obtaining a domain of enterprising behaviours and dimensions is a first step in constructing a scale of ''enterprising behaviour of ordinary people''.  相似文献

The main purpose of this study was to examine the effects of self-generated emotions on sprinting times within the frameworks of Lazarus's (1991b, 2000b) cognitive-motivational-relational theory and Frederickson's (2001) broaden-and-build theory. Using self-generated emotions as an emotion induction method, 44 participants were asked to recall personal emotional episodes before sprinting and all participants took part in 3 emotion induction conditions: happiness, anxiety, and an emotion-neutral state. The results of 2 experiments indicated that the performance in the happiness condition was significantly greater compared to the anxiety condition and the emotion-neutral condition.  相似文献

Replicability of findings is at the heart of any empirical science. The aim of this article is to move the current replicability debate in psychology towards concrete recommendations for improvement. We focus on research practices but also offer guidelines for reviewers, editors, journal management, teachers, granting institutions, and university promotion committees, highlighting some of the emerging and existing practical solutions that can facilitate implementation of these recommendations. The challenges for improving replicability in psychological science are systemic. Improvement can occur only if changes are made at many levels of practice, evaluation, and reward.
