Bioethicists have articulated an ideal of shared decision making between physician and patient, but in doing so the role of clinical uncertainty has not been adequately confronted. In the face of uncertainty about the patient's prognosis and the best course of treatment, many physicians revert to a model of nondisclosure and nondiscussion, thus closing off opportunities for shared decision making. Empirical studies suggest that physicians find it more difficult to adhere to norms of disclosure in situations where there is substantial uncertainty. They may be concerned that acknowledging their own uncertainty will undermine patient trust and create additional confusion and anxiety for the patient. We argue, in contrast, that effective disclosure will protect patient trust in the long run and that patients can manage information about uncertainty. In situations where there is substantial uncertainty, extra vigilance is required to ensure that patients are given the tools and information they need to participate in cooperative decision making about their care. 相似文献

Much attention has begun to focus on the quality of care for persons near the end of life. Palliative care, especially through hospice, has generated much discussion as possibly the most holistic care available. Consideration of how chronically ill older adults receive such care as a result of public policy can benefit from adopting a multidimensional perspective. This paper adopts Bronfenbrenner's ecological model to understand current end-of-life care for nursing home residents, followed by consideration of how each of these dimensions or levels of influence can be used to foster both research agendas and policy reforms to improve end-of-life care of nursing home residents. Specifically, the benefits of considering the influence of such policy initiatives as the Medicare hospice benefit and the Patient Self-Determination Act (PSDA) on end-of-life care in nursing homes and the means through which policy can be informed by clinical research is emphasized. 相似文献

The relationship between severe early institutional deprivation and scholastic attainment at age 11 in 127 children (68 girls
and 59 boys) adopted from institutions in Romania was compared to the attainment of 49 children (17 girls and 32 boys) adopted
within the UK from a non-institutional background. Overall, children adopted from Romania had significantly lower attainment
scores than those adopted within the UK; the children within the Romanian sample who had spent 6 months or more in an institution
had significantly lower attainment scores than those who had spent less than 6 months in an institution, but there was no
additional risk of low attainment associated with longer institutional care after 6 months. The lower scholastic attainment
in the children adopted from Romanian institutions, as compared with domestic adoptees, was mediated by IQ, and to a lesser
degree, inattention/overactivity. When these factors were taken into account, only small between-group differences in attainment

Two studies examined a novel prediction of the causal Bayes net approach to judgments under uncertainty, namely that causal knowledge affects the interpretation of statistical evidence obtained over multiple observations. Participants estimated the conditional probability of an uncertain event (breast cancer) given information about the base rate, hit rate (probability of a positive mammogram given cancer) and false positive rate (probability of a positive mammogram in the absence of cancer). Conditional probability estimates were made after observing one or two positive mammograms. Participants exhibited a causal stability effect: there was a smaller increase in estimates of the probability of cancer over multiple positive mammograms when a causal explanation of false positives was provided. This was the case when the judgments were made by different participants (Experiment 1) or by the same participants (Experiment 2). These results show that identical patterns of observed events can lead to different estimates of event probability depending on beliefs about the generative causes of the observations. 相似文献

Over the past decade, public funding for Couple and Relationship Education programs has expanded. As program administrators have been able to extend their reach to low‐income individuals and couples using this support, it has become apparent that greater numbers of relationally distressed couples are attending classes than previously anticipated. Because psychoeducational programs for couples have traditionally served less distressed couples, this dynamic highlights the need to examine the policy and practice implications of more distressed couples accessing these services. This paper reviews some of the most immediate issues, including screening for domestic violence and couple needs, pedagogical considerations, and the potential integration of therapy and education services. The integration of a novel spoken word with existing lexical items can proceed within 24 hours of learning its phonological form. However, previous studies have reported that lexical integration of new spoken words can be delayed if semantic information is provided during learning. One possibility is that this delay in lexical integration reflects reduced phonological processing during learning as a consequence of the need to learn the semantic associations. In the current study, adult participants learnt novel words via a phoneme monitoring task, in which half of the words were associated with a picture referent, and half were phonological forms only. Critically, participants were instructed to learn the forms of the novel words, with no explicit goal to learn the word–picture mappings. Results revealed significant lexical competition effects emerging one week after consolidation, which were equivalent for the picture-present and form-only conditions. Tests of declarative memory and shadowing showed equivalent performance for picture-present and form-only words, despite participants showing good knowledge of the picture associations immediately after learning. These data support the contention that provided phonological information is recruited sufficiently well during learning, the provision of semantic information does not slow the time-course of lexical integration. 相似文献

Acceptance-based behavioral therapies (ABTs) for obesity may be superior to standard behavioral therapies but have not been adequately tested with American Indians (AIs). Neurocognitive function is also unexamined in relation to behavioral weight loss among AIs despite findings that neurocognition predicts outcomes in general samples, may help explain some of the benefits of ABTs, and may be relevant to marginalized groups. The primary objective of this pilot was to examine the feasibility/acceptability of ABT in an AI sample. Exploratory analyses examined the relationship between neurocognition and weight loss. Forty-eight AI adults with overweight/obesity (ages 43.3 ± 10.3 years, 85% female; baseline body mass index = 36.8 ± 4.4 kg/m2) enrolled in a 6-month open ABT weight loss trial. Feasibility indices, including screening/enrollment, session attendance, retention rates for posttreatment assessments, and program acceptability were examined. Percent weight loss (%WL) was assessed as well as fluid and crystalized neurocognition (National Institutes of Health Toolbox Cognition Battery [NIHTB-CB]). We enrolled 79% of the eligible sample and retained 75% (N = 36) at posttreatment assessments. Program completers lost an average of 5.2 ± 4.9% of initial body weight (dz = 1.14), whereas intent-to-treat analyses show a mean loss of 4.1 ± 4.7%. Participants reported high satisfaction, effectiveness, and cultural appropriateness. Exploratory analyses of neurocognitive domains suggested that crystalized cognition was higher among completers, and higher baseline cognitive flexibility predicted greater %WL (β = .34, p = .05). ABT resulted in clinically significant weight loss in an AI sample. A controlled trial of ABT in a larger, more diverse sample is warranted to determine whether (a) the findings are robust, generalizable, and/or superior to other treatments and (b) neurocognitive factors moderate outcomes. 相似文献
