An exploratory study of parents with mentally retarded children, plus previous research by others, suggests three hypotheses on problems of stigma that were systematically tested in mailed questionnaires returned by 319 mothers of seriously retarded children. Greater patient involvement in health decision-making requires exchange of information between the patient and the healthcare professionals. Decisions regarding healthcare at the end of life include consideration of cardiopulmonary resuscitation (CPR). The stated objectives of this study were to determine how often language around concepts of resuscitation is used in the community by examination of the English language corpora (ELC); to explore the understanding of the same language by a group of older hospital patients; and to determine the patients’ knowledge of the process and success of CPR, as well as the sources of their information. Medical inpatients aged 75 years and older were surveyed to this end in the setting of a tertiary university teaching hospital. Interrogation of the Australian, British and American English Corpora was accomplished by a linguist, and a questionnaire and semi-structured interview were administered to ascertain patient knowledge. We demonstrated that although medical inpatients have some familiarity with terms relating to resuscitation, there is a lack of understanding of the context, process and outcomes of CPR. The predominant sources of information were television and print media. Examination of the ELC revealed a paucity of the use of terms related to resuscitation. This finding indicates that physicians have a duty of care to determine patients’ understanding around resuscitation language, and terms used, in discussions of their preferences before assuming their engagement in shared decision-making. More open public discussion around death and resuscitation would increase the general knowledge of the population and would provide a better foundation for the discussions in times of need.  相似文献

Two structural-priming experiments investigated how bilinguals represent syntactic structures. According to the shared-syntax account (Hartsuiker, Pickering, & Veltkamp, 2004), bilinguals have a single syntactic representation for structures that exist in both languages, whereas separate-syntax accounts claim that the representations for these structures are language specific. Our experiments tested native speakers of Swedish who were highly proficient in English. The results showed that structural priming within language and between languages was equally strong. This indicates that representations of syntactic structures from different languages are shared and, therefore, supports the shared-syntax account.  相似文献

Anesthetic-induced changes in the neural activity of the brain have been recently utilized as a research model to investigate the neural mechanisms of phenomenal consciousness. However, the anesthesiologic definition of consciousness as “responsiveness to the environment” seems to sidestep the possibility that an unresponsive individual may have subjective experiences. The aim of the present study was to analyze subjective reports in sessions where sedation and the loss of responsiveness were induced by dexmedetomidine, propofol, sevoflurane or xenon in a nonsurgical experimental setting. After regaining responsiveness, participants recalled subjective experiences in almost 60% of sessions. During dexmedetomidine sessions, subjective experiences were associated with shallower “depth of sedation” as measured by an electroencephalography-derived anesthesia depth monitor. Results confirm that subjective experiences may occur during clinically defined unresponsiveness, and that studies aiming to investigate phenomenal consciousness under sedative and anesthetic effects should control the subjective state of unresponsive participants with post-recovery interviews.  相似文献

Counselors assume an important role in the mandated multidisciplinary evaluation of children with emotional disturbance (ED), but the field lacks a standard battery of assessment procedures for working with children who have ED. This study investigated the validity of J. L. Holland's (1997) theory of vocational personalities and work environments. The sample consisted of 241 randomly selected members of the National Association of School Psychologists, each of whom completed a demographic data form, the Self‐Directed Search‐Revised (SDS‐R; J. L. Holland, 1994) and a modified short form of the Minnesota Satisfaction Questionnaire (D. J. Weiss, R. V. Dawis, G. W. England, & L. H. Lofquist, 1967). Only limited support was found for major constructs associated with Holland's theory, although the study found several significant relationships between 3‐letter SDS‐R codes and desired role function as would be predicted by Holland's theory.  相似文献

Medical schools place considerable emphasis on admissions interviews in the selection of students. Interviews, with unwritten performance criteria and subjective evaluation, contain the potential for unconscious discrimination against certain groups of applicants. This study investigates the contribution of interview scores to the total evaluation of male and female applicants for admission to one medical school in the United States. Findings reveal that interveiw scores are counted more heavily for females than males in arriving at a final ranking for admission. Further, females were rated lower in general than males on interview evaluations. While the magnitude of the associations is small, the combined effect points to a potential for discrimination, however unintended, against women applicants in the present case study.The authors appreciate the valuable criticisms of and comments on an earlier draft from Drs. Human/Companion Animal Therapy is a single process in that it introduces a companion animal into the life of a person to enhance his emotional well being. There are, however, various ways in which the companion animal may be used. These are: a) as a psychotherapeutic adjunct, b) as the sole therapist, c) as a catalytic agent for change, d) as a means of contact with nature, one's unconscious and the universe. This study examined the effects of publicly-posted performance feedback on the in-hospital behavior of patients residing on a self-management unit at a large, state operated, psychiatric treatment facility. When contrasted to a baseline phase in which staff received weekly information regarding patients' performance, scores in six of the eight assessment areas improved when this information was provided directly to patients. The authors' conclude that performance feedback may be a useful adjunct to inpatient treatment programs because it clarifies programmatic expectations, helps focus attention on developing strategies or interventions to meet those expectations, and encourages active treatment participation from patients and staff.  相似文献
