An ethically justified framework for clinical investigation to benefit pregnant and fetal patients

Research to improve the health of pregnant and fetal patients presents ethical challenges to clinical investigators, institutional review boards, funding agencies, and data safety and monitoring boards. The Common Rule sets out requirements that such research must satisfy but no ethical framework to guide their application. We provide such an ethical framework, based on the ethical concept of the fetus as a patient. We offer criteria for innovation and for Phase I and II and then for Phase III clinical trials to improve the health of pregnant patients and of fetal patients and also criteria to responsibly manage the transition from investigation to clinical practice. Basing ethical criteria for research involving pregnant women on the ethical concept of the fetus as a patient insulates the proposed ethical framework and therefore research on pregnant women from the divisive and politicized concepts and discourses of personhood, fetal rights, and unborn child.     

Lifetime and 12‐Month Nonsuicidal Self‐Injury and Academic Performance in College Freshmen

We examined whether nonsuicidal self‐injury (NSSI) is associated with academic performance in college freshmen, using census‐based web surveys (N = 7,527; response = 65.4%). NSSI was assessed with items from the Self‐Injurious Thoughts and Behaviors Interview and subsequently linked with the administratively recorded academic year percentage (AYP). Freshmen with lifetime and 12‐month NSSI showed a reduction in AYP of 3.4% and 5.9%, respectively. The college environment was found to moderate the effect of 12‐month NSSI, with more strongly reduced AYPs in departments with higher‐than‐average mean departmental AYPs. The findings suggest that overall stress and test anxiety are underlying processes between NSSI membership and academic performance.

Bioethics in the twenty-first century: why we should pay attention to eighteenth-century medical ethics.

Those of us who work in the field of bioethics tend to think that, because the word "bioethics" is new, so too the field is new in all respects, but we are not the first to do bioethics. John Gregory (1724-1773) did bioethics just as we do it, at least two centuries before we thought to do it. He deployed philosophical methods as sophisticated as our own. Indeed, Gregory took up the very best moral philosophy available to thinkers of the Scottish Enlightenment, namely, David Hume's moral philosophy and its core concept of sympathy. Gregory also responded in a conceptually powerful and clinically applicable way to the problems of his time, just as we do. I want here to outline Gregory's accomplishment and to identify some aspects of its importance for bioethics in the twenty-first century.     

Speaking out of Turn: Martin Heidegger and die Kehre

' Speaking out of Turn : Martin Heidegger and die Kehre ' examines the difference between Heidegger's own understanding of 'the turning' and that understanding which originated with Karl Lowith and was later presented to English-speaking readers by William Richardson in Martin Heidegger: Through Phenomenology to Thought . The study focuses on Heidegger's own introduction to Richardson's book, and argues that, far from confirming Richardson's view that there is a 'Heidegger I' and 'Heidegger II' connected by the 'reversal' or turning, Heidegger sought to indicate with (sometimes indirect) reference to his own works that the 'turning' is a movement in thought that it was part of the original project of Being and Time to carry through, but which he only succeeded in describing much later. The study attempts to illustrate this by a close examination of the works to which Heidegger alludes in his Foreword to Richardson's book. Many of these were not available when Richardson published (1963), and so it has only more recently been possible to amplify Heidegger's earlier published works with reference to his lecture courses. The study concludes that the horizon of time and the analytic of Dasein never really disappear from his later thinking, as many have claimed, and proposes that the relationship between the earlier and later Heidegger be re-examined. This re-examination takes the form of accepting that far from the 'turning' representing a fracture, where Heidegger abandons the existential-temporal analytic of Dasein in favour of an attempt to think only being ( das Sein ) as such, the 'turning' represents the point of unity in Heidegger's work. This point of unity shows how Dasein and being 'belong together' in 'the event' ( das Ereignis ).
