This article examined the predictive role of perceived control in long-term changes in disability among 1541 independently living older persons. Disability referred to self-reported difficulties with (instrumental) activities of daily living. Perceived control referred to the extent to which one assumes oneself as having control over one's life chances, unlike the fatalistic assumption that one's life is ruled by external factors. Data on disability and perceived control were collected in 1993 and recollected in 2001. Covariates included age, gender, living arrangement, level of education, and the number of chronic medical conditions. Regression equations were estimated with disability in 2001 as outcome and the selected variables as predictors. The level of perceived control decreased and the level of disability increased significantly over an 8-year period. The predictive role of perceived control for subsequent change in disability was statistically significant but not very strong. The association between perceived control in 1993 and disability in 2001 was stronger for older persons (>65 years) compared to younger persons (≤65 years).  相似文献

Abstract

Ruderman discusses the challenge that contemporary professional women still face in a sexist society and from their own inner critic. She shows how a relational approach to the treatment of women can empower women to overcome both external and internalized sexism. It is suggested that as patriarchy has been challenged, there has been a backlash against feminism. Urban professional men rebel against feminism by aspiring to become playboys who will never stop “playing the field.” The playboy mentality creates a challenge for contemporary professional women who may not wish their boyfriends or husbands to be spending so much time seeking sexual gratification through Internet pornography, lap dances at strip clubs, massages with happy endings, or clandestine affairs.

Abstract

This discussion focuses on two potential dangers in our reaction to the problems associated with years of prejudice and constriction in female gender roles, holding women back from exploring and reaching their personal and career goals. One potential danger is that of creating an equally rigid, unitary system of gender roles that leaves equally little flexibility in women's choices by imitating traditional male gender roles. The other potential danger is that of attributing the main cause of psychological problems to social role constriction when greater pathology is at work.

Abstract

This article examines the relevance of relationally oriented psychoanalytic phenomenology as a frame of reference for understanding psychotic states in schizophrenia. The etiology and structural characteristics of such states are examined from both a neurophysiological, and a relational and interpersonal point of view. Many findings indicate organic abnormalities in schizophrenia, acquired through an exposure to physical risk factors and genetic predisposition, but relational traumas also seem to have an impact on the form and content of psychotic states. This may happen through interruptions in the “use-dependent” maturation of the brain in a relational milieu. In addition, at the level of meaning-making, we find that the emotional and relational contexts—both past and present—give the substance or “raw material” for hallucinations and delusions. In a psychotic state, the person also tries to handle basic affects and relational needs, and to make meaning in states of overwhelming chaos and anxiety. Donald W. Winnicott's theory of “a holding environment”, Stephen Mitchell's theory of the relational matrix, and Stolorow, Atwood, and Orange's concepts of a “shattered world” and annihilation anxiety are used as a framework in the analysis of the phenomenology of psychosis. Two case illustrations, one from child psychotherapy and one from therapy with an adult patient, are used to illuminate the theoretical points of view.  相似文献

Veterinarians have an increased risk for suicide compared with the general population, yet there is little consensus regarding why this might be. We hypothesized that veterinarians become relatively fearless about death due to their repeated exposure to euthanasia. Accordingly, we predicted that there would be a positive relationship between experience with euthanasia and fearlessness about death, due to emotional habituation to the process of euthanasia. In a sample of 130 veterinary students, results conformed to expectation and indicated that the relationship with fearlessness about death was specific to euthanasia and did not generalize to experience with surgery or necropsy.  相似文献

The effects of inpatient treatment, including manual-guided cognitive-behavioral treatment (CBT), were examined for 36 adolescents (12–18 years) with chronic school absenteeism and mental health problems. Using a within-subject control group design, no changes were noted during the untreated waiting period, whereas there was a strong, statistically significant reduction in school absence rates and mental health problems from the start to the end of inpatient treatment. At discharge, 88.9% of adolescents attended school regularly or were employed (86.1 and 63.9% at 2- and 9-months follow-up, respectively). Self- and parent-reported mental health problems were significantly reduced during treatment and remained stable during follow-up.  相似文献

The extent to which parents and other caregivers implement behavioral interventions with integrity has an important impact on treatment effectiveness. This article considers popular interventions that train caregivers and improve treatment integrity in applied behavior analysis. After considering these interventions a contextual approach is described, whereby the function of caregiver non-adherence is considered. In particular, interventions aimed at manipulating setting factors are reviewed. The examples of respite and social support groups are provided as function-based interventions to improve caregiver adherence to behavioral recommendations. Implications for more contextual, coordinated, and comprehensive function-based behavioral services are provided.  相似文献

Daily Behavior Report Cards (DBRC), which typically require teachers to evaluate students' daily behavior and parents to provide contingent consequences, are an effective and acceptable method for improving children's classroom behavior. The current study evaluated whether parent involvement is an essential treatment component or whether teacher feedback alone would be sufficient to produce children's behavior changes. The effectiveness of DBRC with teacher feedback to students and parent delivered consequences (PC) was compared to teacher feedback to students only (no parent delivered consequences, NPC) for increasing appropriate classroom behavior and academic productivity in African American, low income, elementary school children with Attention-Deficit/Hyperactivity Disorder (ADHD). Both treatments led to increased on-task behavior, although PC was superior to NPC. Academic productivity outcomes are less clear. Treatment implications and future directions for research on teacher feedback interventions are discussed.  相似文献
