This article focuses on the patients’ view of the causes and consequences of premature therapy termination in inpatient psychosomatic setting. In this study, drop-out patients and non-drop-out patients were questioned in semi-structured phone interviews (n=62). The causes of early termination are seen in this study to be connected with an internal, somatic concept of disease, discontent with treatment, and social interaction problems in the hospital. In 80% of the examined cases more than two causes were reported. Consequently, early termination of treatment can be considered as an outcome of multifactorial influences. Furthermore, a group of “inner drop-outs” has been identified within the non-drop-out patients. Despite high discontent, these patients remained in treatment because they often feared possible negative consequences of early treatment termination. Both drop-outs and inner drop-outs scored lower in outcome analysis than the remaining patients. This study points to problem areas that can help identify potential drop-out patients. 相似文献

The current study focuses on auditory task switching, more precisely on switching attention between different temporal patterns of the same auditory stimulus. Tone sequences consisting of nine different pitch tones were presented aurally. Three repetitive short 3-tone patterns (local focus) were combined to a long pattern (global focus), and each could be either rising or falling, resulting in congruent or incongruent combinations. Participants were informed by a cue if they had to attend to the short or to the long pattern, and they indicated if the target pattern was rising or falling by pressing one of two keys. In two experiments, we investigated cued switches between the two attentional foci. Switch costs in reaction times and errors were observed when switching from the long to the short pattern but not when switching from the short to the long pattern. These asymmetric switch costs were reduced when participants had more time to prepare for the switch in a condition with a prolonged cue-stimulus interval. In addition, participants made more errors when global and local patterns did not correspond to each other (i.e., in incongruent trials) when attending to either of the patterns, but this congruency effect was not modulated by preparation time. The data suggest that the mechanisms of task goal prioritizing, as indicated by the asymmetric attention switch costs, are dissociable from those underlying stimulus selection, as indicated by the congruency effects. 相似文献

The present study examined the effect of instructions on sequential task preparation using a cuing paradigm with three tasks. All task transitions were predictable, whereas task identity was unpredictable in switches but predictable in repetitions. In Experiment 1, predictability (predictable vs. random) was manipulated while preparation time (i.e., the cue-stimulus interval, or CSI) remained constantly short. In Experiment 2, CSI was manipulated for predictable task transitions. Both experiments showed clear instruction effects, but these were restricted to predictable task repetitions, for which predictability determined the identity of the upcoming task. Predictability effects were small in task switches and were not modulated by instruction, suggesting that preparation is mainly task-specific rather than switch-specific. Together, these results suggest that intentional processes contribute to predictability benefits in task repetitions, probably by enhancing the monitoring of sequential transitions in working memory in order to maintain activation in task repetitions. 相似文献

Recognition memory can be supported by both the assessment of the familiarity of an item and by the recollection of the context in which an item was encountered. The neural substrates of these memory processes are controversial. To address these issues we applied repetitive transcranial magnetic stimulation (rTMS) over the right and left dorsolateral prefrontal cortex (DLPFC) of healthy subjects performing a remember/know task. rTMS disrupted familiarity judgments when applied before encoding of stimuli over both right and left DLPFC. rTMS disrupted recollection when applied before encoding of stimuli over the right DLPFC. The close relationship between attention and consciousness has led many scholars to conflate these processes. This article summarizes psychophysical evidence, arguing that top-down attention and consciousness are distinct phenomena that need not occur together and that can be manipulated using distinct paradigms. Subjects can become conscious of an isolated object or the gist of a scene despite the near absence of top-down attention; conversely, subjects can attend to perceptually invisible objects. Furthermore, top-down attention and consciousness can have opposing effects. Such dissociations are easier to understand when the different functions of these two processes are considered. Recent developments in global yoga show a tendency towards social activism in the charity market. As part of this, Yoga Aid World Challenge 2012 (founded in 2007) is a good example of how neoliberal organisational culture and generosity may become entangled. Competition stands out as an unusual strategy in the predominantly gentle type of modern postural yoga. During this 24-hour event, yoga is practised across 25 countries worldwide, following the course of the sun. Corresponding social networks and digital media strongly promote, months before the event, the joy of practising yoga and equate the meaning of life with giving. This is interpreted with findings from behavioural economics on altruism and from new institutional economics on the organisers’ communication and event marketing. Initial theorizing depicted self-complexity as the number of nonoverlapping self-aspects, such as traits, roles, and behaviors, and proposed that greater self-complexity is linked to better coping in response to stress and negative events. A review of the literature, however, finds inconsistent results. The inconsistency apparently arises from variation in the measurement of self-complexity. The different measures stem from disagreement over the definition of self-complexity, and the various definitions apparently result from theoretical disagreement about how to conceptualize the structure of self-knowledge. The present paper reviews the self-complexity literature and suggests directions for future research. The present paper suggests a positive, moderating relationship between self-complexity and coping, and additional research that includes careful measurement and definition of self-complexity may provide stronger support for this relationship. 相似文献
