The paper gives an account of a man, now 32 years old, who has exhibited the typical characteristics of a "Münchhausen Syndrome", increasing in intensity, since later childhood. The increasingly neurological-neurosurgical range of his predominantly simulated and manipulated complaints covers in the main seizures, attempted suicide, craniocerebral injuries, hemiplegia, and paraplegia. The patient has had four operations for a "Cauda Syndrome". The complex conditioning the complaint, and appropriate medical reaction, are discussed with critical reference to previous literature on the subject. 相似文献

The Depressive Experiences Questionnaire (DEQ; Blatt, D'Afflitti, & Quinlan, 1976b) was designed to assess the introjective and anaclitic personality dimensions hypothesized by Blatt (1974) to underlie different forms of depression. Welkowitz, Lish, and Bond (1985) revised the DEQ (RDEQ) in order to simplify its scoring and facilitate cross-gender comparisons. The study described herein explored the relation between the original and revised forms of the DEQ and assessed the reliability and validity of the Welkowitz et al. (1985) version of the inventory. Two samples were employed: (a) 163 psychiatric outpatients; and (b) 144 adolescent and young-adult offspring of patients with major affective disorders, chronic physical diseases, and normal controls. The results indicated that the three scales comprising the RDEQ were highly correlated with the corresponding scales from the original DEQ, however, the original and revised forms of the inventory exhibited different patterns of intercorrelations between scales. The RDEQ was internally consistent and stable over a 6-month period. In addition, it exhibited the predicted patterns of relationships with interview and self-report measures of depression and depressive personality traits and cognitive styles. Finally, the RDEQ was significantly associated with the course of depression in a 6-month follow-up study. The findings were generally similar across samples and genders. Overall, these results support the reliability and validity of the RDEQ, but indicate that there are important structural differences between the original and revised forms of the inventory. 相似文献

This study examined the test-retest reliability of maternal reports of lifetime psychopathology using DSM-III-R criteria in nonreferred offspring. Sixty-three mothers reported on 79 children, ages 6 to 18 years. Retest intervals were between one and 12 months. The results indicated acceptable reliability for all supraordinate categories (any disorder, any anxiety disorder, any disruptive disorder, externalizing disorders, internalizing disorders) as well as for most individual diagnoses (separation anxiety disorder, simple phobias, oppositional defiant disorder, attention deficit disorder with and without hyperactivity, simple phobias, and adjustment disorder). Maternal reports for any diagnosis were significantly better for intervals under 6 months than for longer retest intervals. Reliability of maternal reports was not significantly affected by child's age or gender. This study supports the expectation that a history of psychiatric disturbance in children can be obtained reliably from mothers. 相似文献

In order to investigate the temporal disruption and social dislocation resulting from the birth of a child, 37 couples were studied at 4 time points: pre-birth (second trimester), and at 1, 4, and 12 months postpartum. A diary-based instrument (the Social Rhythm Metric-SRM) was used to evaluate the habitual times at which 17 events during the day took place, the proportion of events done alone, as a couple only, and with nonfamily members. Also evaluated were perceptions of the quality of the marriage relationship, feelings of depressive symptoms, and feelings of anger. The baby's birth had a major effect on the timing with which daily events were done, and on differences between mothers' and fathers' habitual times, especially in the morning. There were also dramatic differences (50–80% reductions) in the proportion of events done alone, as a couple only, and with nonfamily members. However, these effects were much more marked in mothers than in fathers and were attenuated when there were prior children. Changes in marital quality and mood were also observed. The birth was associated particularly with a decline in marital quality and in the levels of anger and depressive symptoms felt by parents at 1 month postpartum, although these effects interacted with gender and prior parental experience. A variety of early childhood education programs have been established in the last decade that differ markedly in their philosophical and theoretical foundations. This article illustrates how distinct foundations are reflected in all program variables in a unified program. Two unified early childhood education programs are contrasted: a skill development program based upon behaviorist theory, and a cognitive growth program based upon cognitive developmental theory. Three groups of four pigeons, trained to press a treadle on a free-operant avoidance schedule, were given auditory discrimination training. Alternating 2-min components of avoidance and no shock were paired with either a tone or white noise. The pigeons were subsequently given two types of generalization tests, with and without avoidable shocks scheduled. Two of the groups, trained interdimensionally, produced excitatory and inhibitory generalization gradients along the tone frequency dimension. A predicted post-discrimination gradient was computed from the algebraic summation of these gradients of excitation and inhibition. The predicted gradient was compared with the actual post-discrimination gradient obtained from the third group of pigeons that had been given intradimensional discrimination training on the tone frequency dimension. The predicted postdiscrimination gradient agreed in shape with the empirical postdiscrimination gradient. The results in general support Spence's (1937) gradient interaction theory. 相似文献

In a previous paper, it was argued that alertness, selectivity (set), and processing capacity (consciousness) could be identified and studied as separate components of attention. The current paper develops this theme by showing that alertness does not affect the buildup of information within the memory system but only the rate at which a later system responds to that information. Thus, in standard reaction-time tasks, increased alertness produces a reduction in reaction ’time but no decrease in errors. In contrast, providing a model of the signal the S is to process improved both speed and accuracy. The,. presence of a model of what the S is to process varies the vertex neural response to that specific signal as compared to a mismatching signal in the first 200–300 msec after its presentation. Three accounts of this effect are: speeded processing of a matching stimulus, habituation of the electrical response to a matching stimulus, and prolonged or enhanced processing of a mismatch. Evidence favors the first of these explanations, but the other two cannot be dismissed as possible contributors to this effect. 相似文献
