This essay examines the modern psychiatric case study as a scientific method and as a genre of writing about the self. As psychological discourse became a privileged language of selfhood at the fin de siècle, the penultimate signifier of the self came to be found in the realm of sexuality, in the particular contours of an individual's sexual desire. The investigative tool used to uncover these secrets of identity was the case history. This article concerns an influential project of sexual research based on "auto-observations"-autobiographical patient narratives-conducted by Dr. Georges Saint-Paul, who published under the inverted pseudonym "Laupts." The article focuses on the central autobiography of his collection, the "roman d'un inverti," and related sexological literature to suggest how this emphasis on patient stories in psychiatric writing engendered new narrative possibilities for doctors and patients alike. Putting patient stories at the center of psychiatric investigation created a new relationship between patient and doctor, observation and diagnosis, subject and discourse. The tools of psychological observation simultaneously placed a subject's confession more firmly at the center of the investigation and made it more open to interpretation by nonspecialists. Rather than simply confirming the authority of the trained observer, the story of Laupts's enquête suggests that the method actually authorized inquiry by others.  相似文献

This article summarizes evidence and issues associated with psychological assessment. Data from more than 125 meta-analyses on test validity and 800 samples examining multimethod assessment suggest 4 general conclusions: (a) Psychological test validity is strong and compelling, (b) psychological test validity is comparable to medical test validity, (c) distinct assessment methods provide unique sources of information, and (d) clinicians who rely exclusively on interviews are prone to incomplete understandings. Following principles for optimal nomothetic research, the authors suggest that a multimethod assessment battery provides a structured means for skilled clinicians to maximize the validity of individualized assessments. Future investigations should move beyond an examination of test scales to focus more on the role of psychologists who use tests as helpful tools to furnish patients and referral sources with professional consultation.  相似文献

Experiments examined the effect of relationships between a response and an outcome on human judgments of causal effectiveness. In Experiment 1, the time between outcomes obtained on a variable ratio (VR) schedule became the intervals for a yoked variable interval (VI) schedule. Response rates were higher on the VR than on the VI schedule. In Experiment 2, the number of responses required per outcome on a VR schedule were matched to that on a master VI 20-s schedule. Both ratings of causal effectiveness and response rates were higher in the VR schedule. In Experiment 3, tandem VI fixed-ratio (FR) schedules produced higher rates and judgments than equivalent conjunctive VI FR schedule. In Experiment 4, a VI schedule with a reinforcement requirement for a short interresponse time (IRT) produced higher rates and judgments than a simple VI schedule. These results corroborate the view that schedules are a determinant of both response rates and causal judgments. Few current theories of causal judgment predict this pattern of results.  相似文献

This article analyzes narrative illustrations in genetic counseling textbooks as a way of understanding professional habitus--the dispositions that motivate professional behavior. In particular, this analysis shows that there are significant differences in how the textbooks' expository and narrative portions represent Down syndrome, genetic counseling practice, and patient behaviors. While the narrative portions of the text position the genetic counseling profession as working in service to the values of genetic medicine, the expository portions represent genetic counselors as neutral parties. Ultimately, this article argues that this ambiguity is harmful to the production of a professional habitus that is consistent with espoused professional values concerning respect for persons with disabilities and the promotion of psychosocial counseling.  相似文献

This article argues for the importance of the intelligibility of the sexed body to incarnational theology. Building on Mark Jordan's reading of Augustine, I focus on the paradox of the incarnation as both the bodily sign (signa) of God and God Godself as the thing that the sign signifies (res). Through an analysis of the debates around Leo Steinberg's work on the meaning of Christ's genitals in Renaissance art, I explore the ways in which depicting the incarnation is a paradoxical exercise of depicting God's fully human body. I argue that attention to the paradox of the incarnation as both sign and thing can disrupt ideologies of sexual difference that force bodies to be intelligible as unambiguously sexed, while the question of sexual difference can work within incarnational theology to disturb the equivalence of full humanity and unambiguous maleness.  相似文献

We present interim findings of a cross‐site case study of an initiative to expand Housing First (HF) in Canada through training and technical assistance (TTA). HF is an evidence‐based practice designed to end chronic homelessness for consumers of mental health services. We draw upon concepts from implementation science and systems change theory to examine how early implementation occurs within a system. Case studies examining HF early implementation were conducted in six Canadian communities receiving HF TTA. The primary data are field notes gathered over 1.5 years and evaluations from site‐specific training events (k = 5, n = 302) and regional network training events (k = 4, n = 276). We report findings related to: (a) the facilitators of and barriers to early implementation, (b) the influence of TTA on early implementation, and (c) the “levers” used to facilitate broader systems change. Systems change theory enabled us to understand how various “levers” created opportunities for change within the communities, including establishing system boundaries, understanding how systems components can function as causes of or solutions to a problem, and assessing and changing systems interactions. We conclude by arguing that systems theory adds value to existing implementation science frameworks and can be helpful in future research on the implementation of evidence‐based practices such as HF which is a complex community intervention. Implications for community psychology are discussed.  相似文献
