Randomised controlled studies in research environments have demonstrated dialectical behaviour therapy (DBT) to be more efficacious than treatment as usual in reducing suicidal behaviour in patients with borderline personality disorder (BPD). Limited evidence exists for the effectiveness of DBT in the treatment of BPD within routine clinical settings. This study examines the clinical and cost effectiveness of providing DBT over treatment as usual in a routine Australian public mental health service. Forty-three adult patients with BPD were provided with outpatient DBT for six months with patient outcomes compared to those obtained from patients in a wait list group receiving treatment as usual (TAU) from the same service. After six months of treatment the DBT group showed significantly greater reductions in suicidal/non-suicidal self-injury, emergency department visits, psychiatric admissions and bed days. Self-report measures were administered to a reduced sample of patients. With this group, DBT patients demonstrated significantly improved depression, anxiety and general symptom severity scores compared to TAU at six months. Average treatment costs were significantly lower for those patients in DBT than those receiving TAU. Therapists who received intensive DBT training were shown to produce significantly greater improvements in patients’ suicidal and non-suicidal self-injury than therapists who received only 4 day basic training. Further clinical improvements were achieved in patients offered an additional six months of DBT. This study demonstrates that providing DBT to patients within routine public mental health settings can be both clinically effective and cost effective.  相似文献

Children's performance on face identification tests improves dramatically between age 4 and adolescence, yet the source of this improvement is controversial. We used face identity aftereffects to examine whether changes in the organization of face-space during childhood could be a source of this improvement. Specifically we tested whether 7- to 9-year-old children, like adults, show patterns of aftereffects predicted by coding facial identity relative to a norm or the patterns predicted by exemplar-based coding. Consistent with use of norm-based coding children's aftereffects were larger (a) for opposite than non-opposite adapt-test pairs equated for perceptual similarity, and (b) for adaptors far from the average than for adaptors closer to the average. In addition, face identity aftereffects were present by age 5, suggesting adult-like face-space properties by 5, though we did not conduct specific tests to distinguish norm-based from exemplar-based coding in this age group. We conclude that children's poor face identification skills cannot be attributed to a failure to use norm-based coding.  相似文献

The self-teaching hypothesis proposes that orthographic learning takes place via phonological decoding in meaningful texts, that is, in context. Context is proposed to be important in learning to read, especially when decoding is only partial. However, little research has directly explored this hypothesis. The current study looked at the effect of context on orthographic learning and examined whether there were different effects for novel words given regular and irregular pronunciations. Two experiments were conducted using regular and irregular novel words, respectively. Second-grade children were asked to learn eight novel words either in stories or in a list of words. The results revealed no significant effect of context for the regular items. However, in an orthographic decision task, there was a facilitatory effect of context on irregular novel word learning. The findings support the view that contextual information is important to orthographic learning, but only when the words to be learned contain irregular spelling-sound correspondences.  相似文献

Research on narrative identity has traditionally focused on how narrative characteristics are related to personality and well-being in adults. The present pair of studies with college students (Study 1, n= 62; Study 2, n= 68 couples) examined the dynamic conversational processes that might be part of constructing that identity. We examined the characteristics of personal meanings, operationalized as self-event connections, and the retention of those connections about important past events discussed between new romantic partners. Across the 2 studies, self-event connections that were positive and about stable aspects of the self were more likely to occur. Connections that were retained over 1 month were those that were shared by both teller and listener in an independent postconversation assessment. Discussion focuses on the processes that might contribute to the construction of narrative identity and the importance of positivity, stability, and shared connections in developing and maintaining narrative identity.  相似文献

Although cognitive-behavioral treatments for panic disorder have demonstrated efficacy, a considerable number of patients terminate treatment prematurely or remain symtpomatic. Cognitive and biobehavioral coping skills are taught to improve exposure therapy outcomes but evidence for an additive effect is largely lacking. Current methodologies used to study the augmenting effects of coping skills test the degree to which the delivery of coping skills enhances outcomes. However, they do not assess the degree to which acquisition of coping skills and their application during exposure therapy augment outcomes. We examine the extant evidence on the role of traditional coping skills in augmenting exposure for panic disorder, discuss the limitations of existing research, and offer recommendations for methodological advances.  相似文献

This study investigates discriminatory peer aggression among primary school aged children as a function of minority status (based on nationality, ethnicity, religion) of the target and the relative proportions of minority and majority children in the school. Participants were 925 8‐ to 12‐year‐olds attending schools in Britain. Children of minority status were no more likely than children of majority background to experience peer aggression in general. However, minority children were more likely to experience being the victims of discriminatory aggression. Two contrasting predictions were tested: that discriminatory aggression would be more likely when the minority group was relatively small in number or, alternatively, that as the proportions of children of minority backgrounds increased across schools, discriminatory aggression would be greater. The latter hypothesis was supported. Findings also revealed that in schools with a lower minority presence, discriminatory aggression experienced by majority children was significantly lower than that reported by minority children. When the school minority rate exceeded 81%, discriminatory aggression was more commonly experienced among majority children than among minority children. Syntax is widely considered the feature that most decisively sets human language apart from other natural communication systems. Animal vocalisations are generally considered to be holistic with few examples of utterances meaning something other than the sum of their parts. Previously, we have shown that male putty-nosed monkeys produce call series consisting of two call types in response to different events. They can also be combined into short sequences that convey a different message from those conveyed by either call type alone. Here, we investigate whether ‘pyow-hack’ sequences are compositional in that the individual calls contribute to their overall meaning. However, the monkeys behaved as if they perceived the sequence as an idiomatic expression rather than decoding the sequence. Nonetheless, while this communication system lacks the generative power of syntax it enables callers to increase the number of messages that can be conveyed by a small and innate call repertoire.  相似文献

People inevitably face moments of uncertainty as they await feedback regarding self-relevant life outcomes, but they react to this uncertainty with varying amounts of anxiety. Self-construal abstractness (SCA) may be one key predictor of anxiety in the face of uncertain outcomes. SCA refers to a broad self-concept based on generalizations rather than a detailed, low-level self-concept that is based on specific behaviors or events. The current studies examined SCA and anxiety over self-relevant uncertainty. Studies 1 and 2 measured naturally occurring levels of SCA and found that reflecting on an abstract self-construal buffered people from anxiety when upcoming evaluative feedback was highly self-relevant (Study 1) and immediate (Study 2). Study 3 revealed that SCA is equally effective as a buffer against anxiety when manipulated with a subtle prime. The potential for SCA to serve as the target for anxiety-reduction interventions in uncertain situations is discussed.  相似文献

White police officers and undergraduate students mistakenly shoot unarmed Black suspects more than White suspects on computerized shoot/don't shoot tasks. This bias is typically attributed to cultural stereotypes of Black men. Yet, previous research has not examined whether such biases emerge even in the absence of cultural stereotypes. The current research investigates whether individual differences in chronic beliefs about interpersonal threat interact with target group membership to elicit shooter biases, even when group membership is unrelated to race or cultural stereotypes about danger. Across two studies, participants with strong beliefs about interpersonal threats were more likely to mistakenly shoot outgroup members than ingroup members; this was observed for unfamiliar, arbitrarily formed groups using a minimal group paradigm (Study 1) and racial groups not culturally stereotyped as dangerous (Asians; Study 2). Implications for the roles of both group membership and cultural stereotypes in shaping decisions to shoot are discussed.  相似文献
