Quality of life (QoL) is reduced in patients with recurrent vasovagal (VVS) or unexplained syncope (US). Little is known concerning
the natural evolution of QoL following diagnosis. Psychological factors contributing to QoL improvement, such as psychological
distress and self-efficacy remain to be explored. The objective of this study is to examine whether QoL changes 2 months and
6 months following head-up tilt (HUT) testing as well as to determine whether psychological factors are predictive of changes
in QoL. Seventy-three patients undergoing HUT for recurrent syncope were interviewed 1 month before as well as 2 months and
6 months following HUT, using semi-structured interviews and questionnaires. Pre-HUT QoL was significantly worse than at the
2-month (p = 0.000) and 6-month follow-ups (p = 0.000). Psychological distress at baseline was associated with worse QoL 2 months post-HUT (Beta = .280; p = .024), independently of socio-demographic and clinical factors. Improved self-efficacy at follow-up predicted improvements
in QoL (Beta = −.186; p = .023). Two and 6 months post-HUT, QoL is mproved in patients with recurrent syncope. Better QoL is predicted by low psychological
distress and a heightened sense of self-efficacy following HUT. Strategies for enhancing self-efficacy and reducing psychological
distress could be an efficient way to promote QoL in patients suffering from recurrent syncope. 相似文献

AbstractThis study investigates the interplay between family risk and the quality of classroom interactions and their impact on self-regulation skills in two groups of children—children from socioeconomically disadvantaged backgrounds (at-risk) and non-risk children. Participants included 485 children (N = 93 classrooms)—233 (N = 47 classrooms) of which were from socioeconomically disadvantaged areas. Temperament was measured using teacher report. Trained observers rated classroom quality via live observations of a typical preschool day. Behavioral regulation was assessed using direct measures and emotional regulation was assessed using a teacher report. Analyses using multiple group structural equation modeling showed that at-risk children had lower levels of cool behavior regulation than non-risk children. In addition, classroom quality was positively linked to cool behavioral self-regulation. Moderating effects were additionally found between family risk and classroom quality on both hot behavioral regulation and emotional regulation. Classroom quality served as a protective factor for children exposed to more risk factors. Results showed complex associations between individual characteristics, classroom quality, and family risk, indicating a need to further explore the interplay among these variables. 相似文献

The current study uses a mixed-methods approach to describe the nature and context of the initiations undergone by 54 participants among a sample of 73 incarcerated gang members. The study describes and compares types of initiation and how they vary based on individual characteristics of the individuals being initiated. Three general types of initiation are found: (1) the ego violent event, (2) the crime commission, and (3) the expressive violence toward others. An ego violent initiation was more frequent among younger prospective members and those who were coerced into joining. Individuals who were looking for respect were more likely to be required to perpetrate an act of violence toward someone in order to get in. No individual characteristics were associated with crime commission type. The policy implications of such findings are discussed in conclusion. 相似文献

The aim of this study was to examine the overall changes in healthcare services utilization after providing an empirically supported cognitive-behavioral treatment for panic disorder with agoraphobia. Data on healthcare utilization were collected for a total of 84 adults meeting DSM-IV criteria. Participants were completers of a cognitive-behavioral treatment for panic disorder with agoraphobia. Data on utilization of healthcare services and medication were obtained from semi-structured interviews from baseline to 1-year after treatment. Results of the Friedman non-parametric analysis reveal a significant decrease in overall and mental health-related costs following treatment. This study shows a significant reduction in healthcare costs following cognitive behavior therapy for panic disorder with agoraphobia. More studies are needed to examine the potential long-term cost-offset effect of empirically supported treatments for panic disorder. 相似文献

The study objective was to compare breast cancer screening practices, lifestyle behaviors and psychological distress among 640 women initiating BRCA1/2 testing to those among 9,498 similarly-aged women from the general population. Health behaviors and psychological distress were reported in a self-administered questionnaire at pre-test genetic counseling. Regression analyses indicate that high-risk women were more frequently performing breast cancer screening and, in the case of those previously diagnosed with cancer, were more likely to be non-smokers and physically active than women from general population. However, women initiating BRCA1/2 testing were significantly more distressed than controls. Globally, high-risk women seemed to be well informed about recommendations for women who are at high risk and to have access to screening adapted to their risk level. Given their significant psychological distress, providing minimal psychosocial support to all women undergoing BRCA1/2 testing at pre-test genetic counseling is relevant. 相似文献

This study explored gender differences in the relationship between parental influences and body dissatisfaction and disordered eating among offspring. Participants were 221 male and 380 (63%) female French high school students of mean age 16 (SD?=?.95) years old, from the Toulouse area, who completed a survey exploring parental influences, body dissatisfaction, drive for thinness and bulimic symptoms. Girls (compared to boys) perceived more maternal comments regarding both their own weight and shape, and the importance of physical appearance. Parental variables explained a larger amount of the variance in girls’ body dissatisfaction and disordered eating (compared to boys’). Findings support both a cumulative model and a gender-linked model of transmission of attitudes and behaviors related to shape and eating. 相似文献

Executive functioning (EF), needed for goal‐oriented behaviour, thoughts, and emotions, is important for various life domains. This study examined the relationship between family demographics and EF subcomponents. A kindergarten sample was tested on subcomponents of working memory, inhibition, and cognitive flexibility. Parents provided information on demographic variables. For 78 children both EF and demographic data were available. First, demographic profiles were identified within the sample. Two profiles were found: A low‐risk profile of mainly two‐biological‐parent, high‐income families with a highly educated mother who did not smoke during pregnancy and a high‐risk profile of low‐income families with a young, low‐educated mother who more often smoked during pregnancy. Second, children with different demographic profiles were compared on EF subcomponents. Results indicate differential relations between family demographics and EF subcomponents: Whereas for most EF subcomponents no association with family demographics was found, high‐risk children performed better on response shifting and tended to perform worse on verbal memory than low‐risk children. Parenting stress decreased performance only for high‐risk children. Although this study found limited impact of family demographics for EF, further longitudinal research can provide nuanced insights about which factors influence specific EF subcomponents during which developmental periods and guide targeted prevention of EF difficulties. 相似文献
