The persistent head rocking of a woman diagnosed with profound mental retardation was examined during three separate assessments. First, a functional analysis demonstrated that head rocking was maintained independent of social consequences. Next, an antecedent assessment revealed that exposure to alternative sources of stimulation reduced head rocking, but only when the stimulation was being provided. A third assessment, which consisted of an enriched environment, replicated the findings of the first two assessments. Across all three assessments, head rocking persisted at extremely high rates when alternative stimulation was unavailable and returned to high levels immediately following the removal of alternative sources of stimulation. An environmental enrichment (EE) intervention, which was based on the results of the functional analysis and antecedent assessment, resulted in a 70% reduction in the occurrence of the head rocking. In addition, a social validity assessment revealed that the participant's caregivers rated the EE intervention as both effective and socially acceptable. The particular challenges that satiation‐resistant aberrant behavior presents for behavior analysts are addressed. Specific suggestions for research on intervention strategies for persistent aberrant behavior are also provided. Copyright © 2000 John Wiley & Sons, Ltd.  相似文献

This article details the development of the Infant Mental Health Clinical and Training Program at the University of Colorado School of Medicine from 1976 to the present. The article discusses the clinical sites where the program takes place, which include the Neonatal Intensive Care Unit, outpatient child psychiatry clinic, and day treatment programs. Trainees from several mental health disciplines participate in the program, including predoctoral psychology externs, interns from the Clinical Psychology Internship, residents from the Child Psychiatry Training Program, and postdoctoral research fellows in developmental psychopathology. Recently, the Infant Mental Health Program has also been able to offer its own training positions. The philosophical models of clinical service and training are outlined. This paper reports on two studies, each concerned with sex differences in the estimates of Gardner's ‘seven basic types of intelligence’. In the first study, 180 British adults were asked to estimate their own intelligence on the seven intelligence factors. Only one (mathematical/logical) showed a significant sex difference, with males believing they had higher scores than females. Factor analysis of these seven scales yielded three interpretable higher‐order factors. There was a similar sex difference on only one factor (mathematical/spatial intelligence), which showed males rating themselves higher than females. In the second study, 80 student participants completed the same seven estimates of intelligence, plus a standard sex‐role inventory, in order to separate sex and sex role in the self‐estimation of intelligence. A series of sex×sex‐role ANOVAs showed some effects, particularly for mathematical, musical, and spatial intelligence, but nearly always for sex and not sex role. Results suggest that previous studies which found consistent sex differences in self‐estimates of overall intelligence (‘g’) may have over‐exaggerated the issue as the difference is clearly confined to a limited number of factors of intelligence. There is resurgent interest in the psychiatric literature in endophenotypes, variables thought to more strongly reflect the effects of candidate genes than do manifest disorders. In a sample of 176 children with attention deficit hyperactivity disorder (ADHD) and 52 of their siblings, we examined the validity of several putative endophenotypes for ADHD that represent commonly used clinical measures of multiple cognitive/neuropsychological domains of executive functions (EFs). We review their distributional normality, their relations to ADHD symptoms in probands and unaffected siblings relative to nonADHD controls, and their correlation in siblings. We also tested the EF measures’ associations with the ADRA2A gene and whether they mediated or moderated the associations between ADHD and ADRA2A. Several EF measures showed association with ADRA2A, as well as moderation, but not mediation, of its association with ADHD. Implications of the results for evaluating the validity and utility of putative endophenotype measures and for finding candidate gene effects on ADHD are discussed.  相似文献

The aims of the study were (1) to determine whether adolescents find it acceptable to have physicians explore their spiritual beliefs as part of their medical care, (2) to characterize the role of spirituality and religious beliefs in adolescents with and without HIV, and (3) to examine associations between spirituality/religion and quality of life. Adolescents receiving their medical care at an urban Adolescent Health Clinic completed a study-specific questionnaire about spiritual inquiry by their physician, the Brief Multidimensional Measurement of Religiousness/Spirituality, and the Pediatric Quality of Life Inventory 4.0. Chi-squared analysis, Fischer’s exact test, and t tests were used to assess associations. A total of 45 participants enrolled: 19 HIV+ (53% vertical transmission) and 26 HIV?; mean age 17.2 years; 80% African American. Four out of 45 (9%) had ever been asked by their doctor about their spiritual/religious beliefs, and only 8 (18%) had ever shared these beliefs with their healthcare provider. Most teens wanted their provider to ask them about their spiritual beliefs during some visits, especially when dealing with death/dying or chronic illness (67%). Those with HIV were more likely to endorse wanting their doctors to pray with them (42% vs. 15%), feeling “God’s presence” (Mean = 3.95 vs. 2.83), being “part of a larger force” (Mean = 2.58 vs. 1.69), and feeling “God had abandoned them” (Mean = 1.63 vs. 1.15). There are certain circumstances in which healthcare providers should include a spiritual history with teenage patients. Few differences emerged in the teens studied with and without HIV.  相似文献

ABSTRACT

Little is known about how recovery from self-harm is understood by individuals with personal experience of self-harm. YouTube is an important online venue for posting and discussion of user-generated self-harm videos. Drawing on this material, this study explores how self-harm recovery is presented on YouTube. The 30 most highly-viewed videos about recovery from self-harm were thematically analysed. Three themes were identified: 1) a desire for change, 2) control over change, 3) implementing change. This study identifies multiple understandings of self-harm, which in turn shape individuals’ understandings of self-harm recovery. This has implications for self-harm advice or support.  相似文献

Children and adolescents with a history of childhood maltreatment are at risk for a host of psychiatric conditions, although the underlying neurocognitive functioning of these individuals remains largely understudied. This study examined the neurocognitive functioning of childhood maltreatment victims in an adolescent psychiatric inpatient setting. The sample consisted of adolescent inpatients (ages 13–19) that completed intellectual testing as part of a neuropsychological/psychological assessment during hospitalization (n = 39). The sample was grouped based on childhood maltreatment history with one group categorized by maltreatment history (n = 15) and the other group characterized by no maltreatment history (n = 24). Analyses revealed statistically significant differences (p < .01) between maltreatment groups on the majority of assessed domains. When controlling for intelligence, only performance differences on the RCFT remained. RCFT differences remained after controlling for the influence of visual-motor and visual-perceptual/visual-spatial functioning, highlighting the influence of organizational and planning difficulties in those individuals with maltreatment history. Group differences in the frequency of impaired performance for neuropsychological tasks were largest (p < .001) for FSIQ and RCFT. Compromised neurocognitive functioning may negatively contribute to the clinical presentation of this population, highlighting the importance of the child neuropsychologist in the effective treatment of children and adolescents with a history of childhood maltreatment.  相似文献

The present study aimed to verify the association between maternal depression and emotional and behavioral problems in school children in Pelotas, Southern Brazil, considering that maternal depression increases children’s vulnerability for developing psychiatric disorders. This is a cross-sectional study with a school-based sample conducted between August 2015 and November 2016 and it is part of a major project entitled ‘Healthy Childhood in Context: A Multidisciplinary Investigation’. Schoolchildren aged between 7 and 8 years and one of their respective parents or a primary caregiver were included in the study. Maternal depression was assessed using the Mini International Neuropsychiatric Interview (MINI). The presence of emotional and behavioral problems in children was verified by the Strengths and Difficulties Questionnaire (SDQ) – parents version. Children of depressed mothers exhibited higher mean scores in all SDQ domains and in the total score when compared to children of non-depressed mothers. Lower socio-economic status was also associated with higher SDQ scores. Our results showed the effects of both maternal depression and poverty on children emotional and behavioral problems, which evidence the need for child mental health preventive care, and free quality assistance for both mothers and their children.  相似文献
