The Service Delivery Model Task Force (SDMTF) was appointed in 2009 by the leadership of the National Society of Genetic Counselors (NSGC) with a charge to research and assess the capacity of all existing service delivery models to improve access to genetic counseling services in the context of increasing demand for genetic testing and counseling. In approaching this charge, the SDMTF found that there were varying interpretations of what was meant by "service delivery models" and the group held extensive discussions about current practices to arrive at consensus of proposed definitions for current genetic service delivery models, modes of referral and components of service delivery. The major goal of these proposed definitions is to allow for conversations to begin to address the charge to the committee. We propose that current models of service delivery can be defined by: 1) the methods in which genetic counseling services are delivered (In-person, Telephone, Group and Telegenetics), 2) the way they are accessed by patients (Traditional referral, Tandem, Triage, Rescue and Self-referral) and 3) the variable components that depend upon multiple factors unique to each service setting. This report by the SDMTF provides a starting point whereby standardized terminology can be used in future studies that assess the effectiveness of these described models to overcome barriers to access to genetic counseling services. 相似文献

In Aplysia, noxious stimuli induce sensitization of defensive responses. However, it remains largely unknown whether such stimuli also alter nondefensive behaviors. In this study, we examined the effects of noxious stimuli on feeding. Strong electric shocks, capable of inducing sensitization, also led to the suppression of feeding. The use of multiple training protocols revealed that the time course of the suppression of feeding was analogous to that of sensitization. In addition, the suppression of feeding was present only at the time points in which sensitization was expressed. These results suggest that, in Aplysia, noxious stimuli may produce concurrent changes in neural circuits controlling both defensive and nondefensive behaviors. 相似文献

This longitudinal study analyzed the interactions between the quality of life and the coping strategies of 100 patients treated for breast cancer and their caregivers. Data were collected after diagnosis, at the end of treatment, and 6?months after treatment with the Quality of Life Questionnaire-C30 (QLQ-C30), Duke Health Profile and Ways of Coping Checklist for both patients and caregivers. The theoretical model was tested using a typology of patients and mixed model analyses. The quality of life of patients changed over time and no cluster effect was found. The influence of the sociodemographic characteristics, coping strategies (patients and caregivers) and the quality of life of caregivers on patient's quality of life were different according to the quality of life dimensions considered. To understand the adaptation of patients to their disease, it is therefore essential to look at whether the caregiver is capable of playing a supporting role. 相似文献

Two experiments tested the effects of preview sentences and headings on the quality of college students' outlines of informational texts. Experiment 1 found that performance was much better in the preview sentences condition than in a no-signals condition for both printed text and text-to-speech (TTS) audio rendering of the printed text. In contrast, performance in the headings condition was good for the printed text but poor for the auditory presentation because the TTS software failed to communicate nonverbal information carried by the visual headings. Experiment 2 compared outlining performance for five headings conditions during TTS presentation. Using a theoretical framework, "signaling available, relevant, accessible" (SARA) information, to provide an analysis of the information content of headings in the printed text, the manipulation of the headings systematically restored information that was omitted by the TTS application in Experiment 1. The result was that outlining performance improved to levels similar to the visual headings condition of Experiment 1. It is argued that SARA is a useful framework for guiding future development of TTS software for a wide variety of text signaling devices, not just headings. Negative feedback can signal poor performance, but it also provides information that can help learners reach the goal of task
mastery. The primary aim of this study was to test the hypothesis that the amount of information provided by negative feedback
during a paired-associate learning task influences feedback-related processing in the caudate nucleus. To do this, we manipulated
the number of response options: With two options, positive and negative feedback provide equal amounts of information, whereas
with four options, positive feedback provides more information than does negative feedback. We found that positive and negative
feedback activated the caudate similarly when there were two response options. With four options, the caudate’s response to
negative feedback was reduced. A secondary goal was to investigate the link between brain-based measures of feedback-related
processing and behavioral indices of learning. Analysis of the posttest measures showed that trials with positive feedback
were associated with higher posttest confidence ratings. Additionally, when positive feedback was delivered, caudate activity
was greater for trials with high than with low posttest confidence. This experiment demonstrated the context sensitivity of
feedback processing and provided evidence that feedback processing in the striatum can contribute to the strengthening of

The relations between young children's mutual (reciprocated) and overall positive emotion (PE) with same- and other-gender peers and their social adjustment were explored. Children's PE and peers' PE were observed across the preschool year during peer interactions (N = 166; 46% girls; M age = 52 months). Results revealed that girls and boys had similar frequencies of overall PE and mutual PE when interacting with same-gender peers, but girls were marginally higher compared with boys in overall and mutual PE when interacting with other-gender peers. Girls and boys did not have greater rates of either type of PE after controlling for gender segregation during same- or other-gender interactions. Using structural equation modeling, children's mutual PE, regardless of their gender, positively predicted indicators of positive adjustment (e.g., prosocial behavior, cooperation) and negatively predicted indicators of negative adjustment (e.g., hyperactivity, disruption, exclusion by peers). Children's overall PE did not predict either type of adjustment. Findings support the importance of mutual PE for children's development. 相似文献

The current study investigated the effect of frequency of loss and awareness on the ability to make advantageous decisions in preschoolers using a child variant of the Iowa Gambling Task (Bechara, Damasio, Damasio, & Anderson, 1994). Preschoolers (N = 50) were randomly assigned to either a low loss frequency (0.1) or a high loss frequency (0.5) condition. The results suggested that increase in frequency of loss and higher awareness improved performance in the 4-year-olds. Finally, further analysis indicated that the effect of increased frequency on decision making was partially mediated by increased awareness of the game. The findings have implications for the integration of explicit and implicit processes in making adaptive choices. 相似文献

This study examined the relation between religiousness and depressive symptoms in African American, Asian American, European American, Hispanic American, and Native American adolescents (N = 13,317) in the United States with self-esteem and school attachment as potential mediators in this link. The data were taken from a nationally represented sample of adolescents in Grades 7 through 12, from the National Longitudinal Study of Adolescent Health (Add Health). Structural equation analyses with measurement and structural invariance across the five ethnic groups and gender supported the model that religiousness, as a composite of internal and external religiousness items, negatively predicted depressive symptoms 1 year later controlling for baseline depressive symptoms. ABSTRACTWe live in a 3D world, and yet the majority of vision research is restricted to 2D phenomena, with depth research typically treated as a separate field. Here we ask whether 2D spatial information and depth information interact to form neural representations of 3D space, and if so, what are the perceptual implications? Using fMRI and behavioural methods, we reveal that human visual cortex gradually transitions from 2D to 3D spatial representations, with depth information emerging later along the visual hierarchy, and demonstrate that 2D location holds a fundamentally special place in early visual processing. 相似文献
