This study explored the experiences of addictions counselors who have undergone client death and the immediate and long-lasting impacts of client death on addictions counselors through the consensual qualitative research (CQR) method. We conducted semistructured interviews with 10 participants exploring their lived, in-depth, experiences in working with clients with addictive disorders with focus given to the death of clients and how they responded to the experience. Results from a data analysis process using the CQR method indicate eight domains: (a) professional ethics, (b) coping skills, (c) client care, (d) preparation for profession, (e) experience of addiction, (f) agency impact, (g) exploring the death experience, (h) recommendations, and 28 categories embedded in each of these domains. Implications for counselor education, training, and practice are discussed. 相似文献

Two spelling systems have been described. The phonological system transcodes speech sounds to letters and is thought to be useful for spelling regular words and pronounceable nonwords. Although the second system, the lexical-semantic system, is thought to use visual word images and meaning to spell irregular words, it is not known if this system is dependent on semantic knowledge. We used a homophone spelling test to examine the lexical-semantic system in five patients. The patients were asked to spell individual homophones (doe or dough) using the context of a sentence. Semantically incorrect and correct homophones were spelled equally well, whether they were regular or irregular. These results demonstrate that an irregular word may be spelled without knowledge of the word's meaning. Therefore, the lexical system can be dissociated from semantic influence. 相似文献

This article reports two experiments on the effects of status and the status distribution on performance. Grade-school children were randomly assigned a label denoting high status or one denoting low status and then randomly assigned to small groups where they were the only ones of their status or where only one child had a status different from theirs. After completing a group task, the children individually took an anagram test, which served as the performance measure. Consistent with research on the effects of social status, in both experiments children with high status performed significantly better on the anagram test than did children with low status. More important, there were significant status by status distribution interactions. Performance among low-status children was even lower if their status was relatively uncommon than if it was relatively common, whereas performance by high-status children was either unaffected (Experiment 1) or enhanced (Experiment 2) if their status was relatively uncommon. Impaired auditory comprehension and fluent but semantically empty speech in conjunction with preserved repetition characterize the syndrome of transcortical sensory aphasia (TSA). Repetition, however, may be mediated by at least two distinct processes--a lexical process that may involve the recognition and subsequent activation of discrete stored word representations and a nonlexical process that involves phonologic decoding and immediate phonologic encoding from immediate memory. We investigated the spontaneous speech, reading, and tendency to recognize and spontaneously correct syntactic errors in four patients with TSA: this analysis suggests there are two subtypes of TSA. We contend that in one subtype both the lexical and direct repetition (or speech production) mechanisms are preserved, but in the second subtype the lexical mechanism is disrupted and repetition is mediated by the nonlexical mechanism. 相似文献

Three distinct reading mechanisms have been proposed: the phonologic conversion system in which letters are converted to their sound equivalents (phonemes) before meaning is established, and two lexical or whole-word mechanisms in which a word (or root morpheme) is identified as a single unit rather than by phonologic conversion. The two lexical mechanisms differ in that one is inextricably linked to semantics but the other is not. The phonologic conversion system may depend more on the perisylvian phonologic system than do the lexical mechanisms, which may be mediated by left parietal or occipital areas or by the right hemisphere. Described here is a patient who had an infarction that partially isolated the perisylvian speech areas and disconnected the partially preserved lexical systems from the phonologic mechanism. He read visually presented orthographically irregular words but could not pronounce orally spelled irregular words. Although he could not read nonwords, he could pronounce orally spelled nonwords and orthographically regular words. These observations suggest that the lexical and phonologic conversion systems are functionally and anatomically distinct. 相似文献

By appropriate use of the starship’s weapon and defense systems, the player can kill enemy ships and avoid destruction. The player’s moves, as well as the time and state of the game environment when each move occurs, are recorded. The procedure tracks performance changes over time very well, and allows the dissection of performance into sequences of commands that reflect the subject’s play strategy. Adynamic aphasia is a form of transcortical motor aphasia characterized by sparse but otherwise normal spontaneous speech that may improve when concepts are introduced by external stimuli. Akinesia, impaired concept formation, inertia of concept generation, a defective semantic network, damage or impaired access to the verbal output lexicon, and defective semantic strategy formation have been proposed to account for this disorder. We studied a patient with adynamic aphasia and frontal lobe systems dysfunction due to bilateral striatocapsular infarctions. The patient was not akinetic but did demonstrate inertia of concept generation that could be overcome with prompting. However, prompting did not improve the number of concepts generated. He demonstrated a generally intact verbal lexicon and semantic network and normal lexical priming. However, his ability to sort closely related items into different classes without prior cuing regarding the nature of the classes was defective. Although his verbal memory was normal, he appeared to use a serial rather than a semantic strategy to recall items. Finally, despite normal lexical priming, he was impaired on a letter fluency task. These results most clearly demonstrate a defect in semantic strategy formation but indicate an additional and possibly related deficit in concept formation and a partial deficit in lexical strategy formation. All of these deficits appear to reflect impairment in the hierarchical organization of knowledge specific to the task at hand. Water-deprived rats were studied under a compound schedule that prescribed that responses terminating interresponse times (IRTs) greater than a fixed value t1 (IRT > t1 component schedule) initiated a delay of reinforcement interval t2, at the end of which water was presented if the subject did not respond ( > t2 component schedule). If the subject responded before the t2 interval elapsed, the IRT > t1 component schedule was re-initiated and water was not presented. The IRT > t1 and > t2 component schedules were not differentially correlated with distinctive stimuli. Rate of responding during the IRT > t1 component decreased as a function of the value of t2. The magnitude of the decreases in response rate appeared to be proportional to the subject's rate under the IRT > t schedule with no delay of reinforcement (t2 = 0 sec). The effects were independent of the parameter value of the IRT > t1 component schedule and of the rate of reinforcement. The results suggested that “efficiency” of performance under IRT > t schedules can be increased by appropriately arranging brief delays of reinforcement. 相似文献
