Celtic culture,Calvinism, social and mental health on the Island of Lewis

The five points of federal Calvinism, as outlined by the Synod of Dort, have had a strong influence on the Protestant church. Their negative effects on the culture and society of the Western Isles of Scotland are often overlooked, or simply accepted as an expresion of that culture. In this paper, an attempt is made to show how Celtic culture on the Isle of Lewis has been held back by this theology, and the effect of this repression on society and the individual. Particular areas of concern are the neglect of the Gaelic language, and the incidence of violence, alcoholism, and depression. Iain Macritchie, S.T.M., comes from the village of Ballantrushal on the Island of Lewis. He is a Church of Scotland minister with a particular interest in training ministers for work in the Highlands and Islands of Scotland, to which he has returned after a year on the Scottish Fellowship at Union Theological Seminary.     

The role of neuroimaging in the discovery of processing stages a review

In this contribution we show how neuroimaging methods can augment behavioural methods to discover processing stages. Event Related Brain Potentials (ERPs), Brain Electrical Source Analysis (BESA) and regional changes in cerebral blood flow (rCBF) do not necessarily require behavioural responses. With the aid of rCBF we are able to discover several cortical and subcortical brain systems (processors) active in selective attention and memory search tasks. BESA describes cortical activity with high temporal resolution in terms of a limited number of neural generators within these brain systems. The combination of behavioural methods and neuroimaging provides a picture of the functional architecture of the brain. The review is organized around three processors: the Visual, Cognitive and Manual Motor Processors.     

CONTRIBUTIONS OF TIPLER'S OMEGA POINT THEORY

Abstract. An attempt to discover what can be learned from the recent work of Frank Tipler on the Omega Point theory requires an analysis of his framework of understanding from scientific, philosophical, and theological perspectives. A critique of his crucial ideas, and of the salient points raised by some of his critics, can then be undertaken within the compass of his strengths. A critique of the critiques of Tipler's work allows one to evaluate the extent and limitations of his contributions.     

Decision-making incapacity among nursing home residents: results from the 1987 NMES survey. National Medical Expenditure Survey

Recent legislative and regulatory developments have focused attention on older adults' capacity for involvement in health care decision-making. The Omnibus Budget Reconciliation Act of 1987 (OBRA 87) focused attention on the rights of nursing home residents to be involved in health care decision-making to the fullest extent possible. This article uses data from the 1987 National Medical Expenditure Survey (NMES) to examine rates of incapacity for health care decision-making among nursing home residents. Elements of the Oklahoma statute were used to operationalize decision-making incapacity: disability or disorder, difficulty in decision-making or communicating decisions, and functional disability. Fifty-three percent of nursing home residents had a combination of either physical or mental impairment and an impairment in either self-care or money management. The discussion focuses on the policy and practice implications of significant rates of incapacity among nursing home residents.     

A PRELIMINARY COMPARISON OF REINFORCER ASSESSMENT METHODS FOR CHILDREN WITH ATTENTION DEFICIT HYPERACTIVITY DISORDER

We evaluated the relative treatment utility of a verbal forced-choice questionnaire, child nomination, and direct observation for identifying the most potent reinforcers for children with attention deficit hyperactivity disorder. Results demonstrated that all three methods were more likely to disagree than to agree, that a forced-choice format may enhance verbal reinforcer assessment, and that further development and evaluation of verbal reinforcer-assessment methods are needed.     

Comparing interspersed requests and social comments as antecedents for increasing student compliance

Two students were alternately presented with interspersed high-compliance requests and social comments as antecedents to low-compliance requests. An initial comparison demonstrated similar positive effects on compliance for interspersed requests and social comments. A second analysis indicated that the effectiveness of social comments for increasing compliance was related to the time interval between social comments and low-compliance requests.     

Predicting and scaling hens' preferences for topographically different responses

Six hens were exposed to several concurrent (second-order) variable-interval schedules in which the response requirements on the alternatives were varied. The response requirements were one key peck versus five key pecks, one key peck versus one door push, and five key pecks versus one door push. Response- and time-allocation ratios undermatched the obtained reinforcement ratios but were well described by the generalized matching law. Time and response bias estimates from two pairs of response requirements were used to predict bias in the third pairing. The predicted values were close to those obtained; this result supports the notion that both numerically and topographically different responses act as constant sources of bias within the generalized matching law. The differences between the response and time biases could be accounted for by the different times needed to complete each response requirement. The results also suggest that the door push is a useful operant for research with domestic hens.     

Physicians' values and physician-value neutrality

For years articles have decried the lack of empathy in physicians' relationships with patients. In addition to being empathetic, physicians are called upon to assume the posture of value neutrality, i.e., not imposing one's values when dealing with patients. Empathy is clearly an expression of deeply held values; even the language used to define it is value-laden. Physicians are consistently called upon to exhibit traits which are expressions of their underlying values. However, if proponents of value-neutrality are to be taken literally one must not impose any of one's values on one's patients. But then one wonders how empathy could ever be expressed; further, it is hard to imagine what a truly value neutral physician would be like. It is time we recognize that any relationship between two persons requires the expression of values from both parties. These values help shape that relationship and define its further history. Physicians are not excluded from this process just because they consider themselves professionals. Divulging personal values to patients is both more honest and more in keeping with the concepts of justice, beneficence, non maleficence, and autonomy than to feign value-neutrality. A presentation of some type statement of values is timely and would give patients an idea of where a physician stands on a myriad of issues. Rather than being value-neutral, value non-neutrality seems a more right and reasonable posture for physicians to assume.
