Individually-tailored, Internet-based treatment for anxiety disorders: A randomized controlled trial

Previous studies on Internet-based treatment with minimal to moderate therapist guidance have shown promising results for a number of specific diagnoses. The aim of this study was to test a new approach to Internet treatment that involves tailoring the treatment according to the patient’s unique characteristics and comorbidities.A total of 54 participants, regardless of specific anxiety diagnosis, were included after an in-person, semi-structured diagnostic interview and randomized to a 10 week treatment program or to a control group. Treatment consisted of a number of individually-prescribed modules in conjunction with online therapist guidance. Significant results were found for all dependent measures both immediately following treatment and at 1 and 2 year intervals. Mean between-group effect size including measures of anxiety, depression and quality of life was Cohen’s d = 0.69 at post-treatment, while the mean within-group effect size was d = 1.15 at post-treatment and d = 1.13 and d = 1.04 at 1 and 2 year follow-up respectively.The tentative conclusion drawn from these results is that tailoring the Internet-based therapy can be a feasible approach in the treatment of anxiety in a homogeneous population. 相似文献

Previous literature on the impact of culture on distinctiveness seeking shows diverging predictions and inconclusive results.
We propose that this may be partly due to the fact that distinctiveness can be sought on various levels of awareness. We theorize
that cultural values will influence explicit, intentional forms of distinctiveness seeking, but will not necessarily moderate
the underlying strength of the identity motive for distinctiveness. Participants in the UK and Sweden completed a measure
of cultural values and explicit and implicit measures of distinctiveness seeking. National differences were found in two explicit
measures of distinctiveness seeking, and these were partially mediated by individual differences in value priorities. However,
no difference was found in our implicit measure of the distinctiveness motive. These findings highlight the importance of
attending to the multi-faceted and culturally flexible nature of identity motives. 相似文献

Decision-makers tend to change the psychological attractiveness of decision alternatives in favor of their own preferred alternative after the decision is made. In two experiments, the present research examined whether such decision consolidation occurs also among individual group members in a large group decision-making situation. High-school students were presented with a decision scenario on an important issue in their school. The final decision was made by in-group authority, out-group authority or by majority after a ballot voting. Results showed that individual members of large groups changed the attractiveness of their preferred alternative from a pre- to a post-decision phase, that these consolidation effects increased when decisions were made by in-group members, and when participants identified strongly with their school. Implications of the findings for understanding of group behavior and subgroup relations are discussed. 相似文献

Current energy debates are often framed in terms of the extent to which energy sources may mitigate global climate change
(GCC) and facilitate energy independence (EI). However, little is understood about whether and how attitudes towards GCC and
EI influence attitudes toward energy sources. Our study included 277 undergraduate students enrolled in introductory psychology
classes at a large, Southeastern university. As hypothesized, results from ordinal and binary logistic regression analyses
revealed that the likelihood participants would support the use of solar, wind, coal or nuclear energy decreased as a function
of their belief that an energy source contributed to GCC. The belief that an energy source contributed to EI increased participants’
likelihood of support. Results suggest that beliefs about GCC and EI are important to determining support for energy sources
and will likely contribute to effective marketing strategies and efforts to implement more environmentally-sustainable behavior

The outcomes of health interventions are co‐constructed through negotiation between the competing knowledges of different stakeholders. In such a context, local implementing partners – who mediate between international donors/programme managers and local beneficiary communities – have a critical ‘knowledge brokering’ role to play. With appropriate support, they can facilitate integration of internationally established knowledge with local beliefs and practices in ways that support health‐enhancing behaviour change. This paper focuses on the local field officers of an HIV prevention programme with Cambodian military couples. The paper asks: (1) what are the outcomes of knowledge encounters between international organizations and local field officers, as expressed in field officers' interpretation of programme goals, strategies and messages? (2) To what extent does the intervention context support the field officers' knowledge brokering efforts? Data collected includes semi‐structured interviews, observations of programme meetings and activities, and textual materials. The findings demonstrate that field officers privilege international knowledge, particularly in interactions with international manager–partners. However, in the field, they both hybridize programme messages and struggle to resolve dilemmas provoked by conflicting international and local knowledges. Material and symbolic asymmetries within the intervention context are shown to undermine their knowledge brokering efforts, as field officers attempt to claim identities and futures as health development professionals. The roles of muscarinic and nicotinic cholinergic receptors in perirhinal cortex in object recognition memory were compared. Rats' discrimination of a novel object preference test (NOP) test was measured after either systemic or local infusion into the perirhinal cortex of the nicotinic receptor antagonist methyllycaconitine (MLA), which targets alpha-7 (α7) amongst other nicotinic receptors or the muscarinic receptor antagonists scopolamine, AFDX-384, and pirenzepine. Methyllycaconitine administered systemically or intraperirhinally before acquisition impaired recognition memory tested after a 24-h, but not a 20-min delay. In contrast, all three muscarinic antagonists produced a similar, unusual pattern of impairment with amnesia after a 20-min delay, but remembrance after a 24-h delay. Thus, the amnesic effects of nicotinic and muscarinic antagonism were doubly dissociated across the 20-min and 24-h delays. The same pattern of shorter-term but not longer-term memory impairment was found for scopolamine whether the object preference test was carried out in a square arena or a Y-maze and whether rats of the Dark Agouti or Lister-hooded strains were used. Coinfusion of MLA and either scopolamine or AFDX-384 produced an impairment profile matching that for MLA. Hence, the antagonists did not act additively when coadministered. These findings establish an important role in recognition memory for both nicotinic and muscarinic cholinergic receptors in perirhinal cortex, and provide a challenge to simple ideas about the role of cholinergic processes in recognition memory: The effects of muscarinic and nicotinic antagonism are neither independent nor additive. 相似文献

Although many cognitive functions require information about the orientations of objects, little is known about representation or processing of object orientation. Mirror-image confusion provides a potential clue. This phenomenon is typically characterized as a tendency to confuse images related by left-right reflection (reflection across an extrinsic vertical axis). However, in most previous studies the stimuli were inadequate for identifying a specific mirror-image (or other) relationship as the cause of the observed confusions. Using stimuli constructed to resolve this problem, Gregory and McCloskey (2010) found that adults' errors were primarily reflections across an object axis, and not left-right reflections. The present study demonstrates that young children's orientation errors include both object-axis reflections and left-right reflections. We argue that children and adults represent object orientation in the same coordinate-system format (McCloskey, 2009), with orientation errors resulting from difficulty encoding or retaining one (adults) or two (children) specific components of the posited representations. 相似文献

Distinguishing between relational and physical aggression has become a key feature of many developmental studies in North America and Western Europe, but very little information is available on relational and physical aggression in more diverse cultural contexts. This study examined the factor structure of, associations between, and gender differences in relational and physical aggression in China, Colombia, Italy, Jordan, Kenya, the Philippines, Sweden, Thailand, and the United States. Children ages 7–10 years (N = 1,410) reported on their relationally and physically aggressive behavior. Relational and physical aggression shared a common factor structure across countries. In all nine countries, relational and physical aggression were significantly correlated (average r = .49). Countries differed in the mean levels of both relational and physical aggression that children reported using and with respect to whether children reported using more physical than relational aggression or more relational than physical aggression. Boys reported being more physically aggressive than girls across all nine countries; no consistent gender differences emerged in relational aggression. Despite mean‐level differences in relational and physical aggression across countries, the findings provided support for cross‐country similarities in associations between relational and physical aggression as well as links between gender and aggression. Changes in the processing of emotional information are key features of affective disorders. Neuropsychological tests based
on emotional faces or words are used to detect emotional/affective biases in humans, but these tests are not applicable to
animal species. In the present study, we investigated whether a novel affective tone discrimination task (ATDT), developed
to study emotion-related behaviour in rats, could also be used to quantify changes in affective states in humans. To date,
the methods used in human neuropsychology have not been applicable to animal experiments. Participants completed a training
session in which they learnt to discriminate specific tone frequencies and to correctly respond in order to gain emotionally
valenced outcomes, to obtain rewards (money), or to avoid punishment (an aversive sound clip). During a subsequent test session,
additional ambiguous probe tones were presented at frequencies intermediate between the reward and avoidance paired tones.
At the end of the task, participants completed self-report questionnaires. All participants made more avoidance responses
to the most ambiguous tone cues, suggesting a bias towards avoidance of punishment. Individual differences in the degrees
of bias observed were correlated with anxiety measures, suggesting the task’s sensitivity to differences in state anxiety
within a healthy population. Further studies in clinical populations will be necessary to assess the task’s sensitivity to
pathological anxiety states. These data suggest that this affective tone discrimination task provides a novel method to study
cognitive affective biases in different species, including humans, and offers a novel assessment to study anxiety. 相似文献

Reward anticipation during learning is known to support memory formation, but its role in retrieval processes is so far unclear. Retrieval orientations, as a reflection of controlled retrieval processing, are one aspect of retrieval that might be modulated by reward. These processes can be measured using the event-related potentials (ERPs) elicited by retrieval cues from tasks with different retrieval requirements, such as via changes in the class of targeted memory information. To determine whether retrieval orientations of this kind are modulated by reward during learning, we investigated the effects of high and low reward expectancy on the ERP correlates of retrieval orientation in two separate experiments. The reward manipulation at study in Experiment 1 was associated with later memory performance, whereas in Experiment 2, reward was directly linked to accuracy in the study task. In both studies, the participants encoded mixed lists of pictures and words preceded by high- or low-reward cues. After 24?h, they performed a recognition memory exclusion task, with words as the test items. In addition to a previously reported material-specific effect of retrieval orientation, a frontally distributed, reward-associated retrieval orientation effect was found in both experiments. These findings suggest that reward motivation during learning leads to the adoption of a reward-associated retrieval orientation to support the retrieval of highly motivational information. Thus, ERP retrieval orientation effects not only reflect retrieval processes related to the sought-for materials, but also relate to the reward conditions with which items were combined during encoding. 相似文献
