Although EQ-5D has been used in the context of acute cough/lower respiratory tract infections (acute cough/LRTI), it has not
been formally validated in this disease area. The primary aim of this study is to assess the construct validity of EQ-5D as
a measure for economic evaluation in the context of acute cough/lower respiratory tract infections. Data were collected alongside
an observational study of the management of patients with acute cough/LRTI in thirteen European countries. However, the analysis
was limited to 349 patients from the UK. Hypotheses were specified a priori and tested in order to establish the construct
validity of EQ-5D. Statistical methods used to test the a priori hypotheses included Wilcoxon signed rank sum test, Kruskal-Wallis
test, Chi-square test and regression analysis. Most of the relationships that were tested were in line with the a priori hypotheses.
The main results indicate that there was a negative relationship between symptom severity scores and EQ-5D, smoking was associated
with worse EQ-5D scores and EQ-5D was able to discriminate between patients with headache, interference with normal activities
and disturbed sleep. This study also showed that the EQ-5D is moderately responsive in patients with acute cough/LRTI, and
is a suitable measure for use in economic evaluation studies of acute cough/LRTI 相似文献

Three experiments with musicians and nonmusicians (N=338) explored variations of Deutsch’s musical scale illusion. Conditions under which the illusion occurs were elucidated and data obtained which supported Bregman’s suggestion that auditory streaming results from a competition among alternative perceptual organizations. In nExperiment 1, a series of studies showed that it is more difficult to induce the scale illusion than might be expected if it is accepted that an illusion will be present for most observers despite minor changes in stimuli and experimental conditions, The stimulus sequence seems better described as an ambiguous figure. Having discovered conditions under which the scale illusion could be reliably induced, Experiments 2 and 3 manipulated additional properties of the stimulus (timbre, loudness, and tune) to provide cues to streaming other than pitch and location. The data showed that streaming of this sequence can be altered by these properties, supporting the notion of a general parsing mechanism which follows general gestalt principles and allows streaming by many stimulus dimensions. Finally, suggestions are made as to how this mechanism might operate. 相似文献

Using 12-year longitudinal data from deceased participants of the Berlin Aging Study (N = 414; age 70-103 years, at first occasion; M = 87 years, SD = 8.13), the authors examined whether and how old and very old individuals exhibit terminal decline in reported life satisfaction at the end of life. Relative to age-related decline, mortality-related decline (i.e., distance-to-death) accounted for more variance in interindividual differences in life satisfaction change and revealed steeper average rates of decline, by a factor of 2. By applying change-point growth models, the authors identified a point, about 4 years before death, at which decline showed a two-fold increase in steepness relative to the preterminal phase. For the oldest old (85+ years), a threefold increase was observed. Established mortality predictors, including sex, comorbidities, dementia, and cognition, accounted for only small portions of interindividual differences in mortality-related change in life satisfaction. The authors conclude that late-life changes in subjective well-being are related to mechanisms predicting death and suggest routes for further inquiry. 相似文献

Community‐Based Learning and Critical Community Psychology Practice: Conducive and Corrosive Aspects

Community‐based learning (CBL) has been more recently introduced into some psychology programmes in the UK than in the USA, where it has existed for a number of decades in the form of ‘service learning’. CBL holds promise as a means of promoting and developing critical community psychology practice, but there are risks involved in its acritical adoption in the psychology curriculum. If associated power dynamics are not considered, CBL has the capacity to serve neoliberal interests and perpetuate, rather than challenge, oppressive social relations. This article examines ways in which CBL can be both conducive and corrosive to critical community psychology practice. Drawing on interdisciplinary literature, it explores ways in which students participating in CBL can be vulnerable to exploitation—both as victims and perpetrators—through collusion and cultural voyeurism. Consideration is given to ways of resisting institutional and other pressures to comply uncritically with the demands of the ‘employability agenda’. These include the importance of facilitated reflective processes in associated modules, to consider aspects of the interactions of people and systems. The article concludes that whilst CBL is inherently risky and involves discomfort for students, this enables development of a more informed consciousness where truly participatory work evolves towards greater social justice. Cancer is now the biggest cause of mortality worldwide. Although the debilitating physical symptoms of cancer have long been known, the psychological and social impacts of cancer have become the subject of examination only relatively recently. The psychological outcomes that have been examined are primarily negative emotional variables, e.g., anxiety, but emerging research has focused on positive emotional variables, e.g., post‐traumatic growth, or cognitive outcomes. In this article, we provide a synthesis of reviews that have addressed the psychosocial impact of cancer. The framework for this synthesis is provided by a conceptualisation in which the presence of cancer impacts on psychosocial outcomes either directly or via mediating variables, including physical symptoms and treatment, and that this effect may be moderated by several variables, some characteristic of the person with cancer (demographic or personality‐related variables) and some characteristic of their environment (social support and medical variables). We also briefly examine the impact of cancer on the broader family unit following cancer diagnosis, treatment, survivorship and bereavement. We conclude that the heterogeneity of the cancer experience highlights the need for theoretically driven research and consistency in measurement approaches to determine mechanisms by which cancer exerts influence on psychosocial outcomes. This would allow development and delivery of targeted psychological interventions and a clearer delineation of the roles of the various parties, including clinical psychologists, family members and policymakers. 相似文献

Change blindness refers to the difficulty most people find in detecting a difference between two pictures when these are presented successively, with a brief interruption between. Attention at the site of the change is required for detection. A number of studies have investigated change blindness in adults and children with autism spectrum disorders (ASD). Some have produced evidence that people with ASD find changes to social stimuli harder to detect and changes to non‐social stimuli easier to detect, relative to comparison participants. However, other studies have produced entirely contradictory findings. There is a need for consistency in methodology to aid understanding of change blindness and attentional processes in ASD. Here, we replicate a change blindness study previously carried out with typically developing (TD) children and adults and with adults with ASD. Results reveal attenuated change blindness for non‐social stimuli in children with ASD relative to TD norms. Our results are interpreted, alongside others' findings, as potentially indicative of a complex relationship between different influences on attention over time. 相似文献
