Children’s engagement and disengagement, adherence and non-adherence, compliance and non-compliance in healthcare have important implications for services. In family therapy mere attendance to the appointments is no guarantee of engaging in the treatment process and as children are not the main initiators of attendance engaging them through the process can be a complex activity for professionals. Through a conversation analysis of naturally occurring family therapy sessions we explore the main discursive strategies that children employ in this context to passively and actively disengage from the therapeutic process and investigate how the therapists manage and attend to this. We note that children competently remove themselves from therapy through passive resistance, active disengagement, and by expressing their autonomy. Analysis reveals that siblings of the constructed ‘problem’ child are given greater liberty in involvement. We conclude by demonstrating how therapists manage the delicate endeavour of including all family members in the process and how engagement and re-engagement are essential for meeting goals and discuss broader implications for healthcare and other settings where children may disengage. 相似文献
Previous studies have indicated a potential association between trauma, such as physical violence, emotional violence and sexual violence, and high-risk sexual behavior. Female sex workers (FSWs) were interviewed to elicit history of traumatic events and sexual behaviors. Among 187 participants, 79 (42.2 %) inconsistently used condoms with clients in the past 30 days. Experiencing community violence (OR: 0.4; 95 % CI: 0.2, 0.8) was found to be significantly protective for inconsistent condom use. In this study, experiencing community violence was found to be protective for inconsistent condom use among FSWs, but none of the other trauma subgroups had significant associations with inconsistent condom use. Reasons for unprotected sex among FSW may be more related to economic or other contextual factors. 相似文献
This study evaluated the influence of language of implementation on functional analysis outcomes for a child with a severe
intellectual disability from a Spanish-speaking home. Challenging behavior was assessed during 5-min sessions under 4 conditions;
attention, play-verbal, play-nonverbal, and demand and across 2 phases; implementation in English versus Spanish. The highest
levels of challenging behavior occurred during the attention and demand conditions of the English phases. These results suggest
that the language of implementation may influence the overall levels of challenging behavior within functional analysis conditions. 相似文献
The purpose of this review is to provide a systematic analysis of studies involving the use of computer-based interventions
(CBI) to teach communication skills to children with autism spectrum disorders (ASD). This review evaluates intervention outcomes,
appraises the certainty of evidence, and describes software and system requirements for each included study. This review has
three main aims: (a) to evaluate the evidence-base regarding CBI, (b) to inform and guide practitioners interested in using
CBI, and (c) to stimulate and guide future research aimed at improving the efficiency and effectiveness of CBI in communication
for individuals with ASD. Results suggest that CBI should not yet be considered a researched-based approach to teaching communication
skills to individuals with ASD. However, CBI does seem a promising practice that warrants future research. 相似文献
Recently, cross-sectional research has demonstrated that depressive rumination is significantly associated with the tendency to engage in cognitive and behavioral avoidance. This evidence suggests that rumination may be the result of attempts to avoid personally threatening thoughts, in a manner suggested by multiple contemporary theories of worry. This investigation examined the temporal relationship among daily levels of cognitive avoidance, behavioral avoidance, rumination, worry, and negative affect. Seventy-eight adolescents completed baseline questionnaires and then electronically completed daily measures of rumination, worry, behavioral avoidance, and cognitive avoidance, as well as sad and anxious affect for 7 days. Lagged-effect multilevel models indicated that increases in daily sadness were predicted by greater daily rumination and cognitive avoidance. Increases in daily anxiety were predicted by greater daily rumination, worry, and both cognitive and behavioral avoidance. Further, both daily rumination and worry were positively predicted by daily cognitive, but not behavioral, avoidance. Mediation analyses suggested that rumination mediated the effect of cognitive avoidance on both sadness and anxiety. Also, worry mediated the effect of cognitive avoidance on anxiety. Implications for models of avoidance, rumination, and worry are discussed. 相似文献
Taboo words represent a potent subset of natural language. It has been hypothesized that “tabooness” reflects an emergent property of negative valence and high physiological arousal of word referents. Many taboo words (e.g., dick, shit) are indeed consistent with this claim. Nevertheless, American English is also rife with negatively valenced, highly arousing words the usage of which is not socially condemned (e.g., cancer, abortion, welfare). We evaluated prediction of tabooness of single words and novel taboo compound words from a combination of phonological, lexical, and semantic variables (e.g., semantic category, word length). For single words, physiological arousal and emotional valence strongly predicted tabooness with additional moderating contributions from form (phonology) and meaning (semantic category). In Experiment 2, raters judged plausibility for combinations of common nouns with taboo words to form novel taboo compounds (e.g., shitgibbon). A mixture of formal (e.g., ratio of stop consonants, length) and semantic variables (e.g., ± receptacle, ± profession) predicted the quality of novel taboo compounding. Together, these studies provide complementary evidence for interactions between word form and meaning and an algorithmic prediction of tabooness in American English. We discuss applications for models of taboo word representation.
