This paper describes a specific psychoanalytic psychotherapy for patients with severe personality disorders, its technical approach and specific research projects establishing empirical evidence supporting its efficacy. This treatment derives from the findings of the Menninger Foundation Psychotherapy Research project, and applies a model of contemporary psychoanalytic object relations theory as its theoretical foundation. The paper differentiates this treatment from alternative psychoanalytic approaches, including other types of psychoanalytic psychotherapy as well as standard psychoanalysis, and from three alternative non-analytical treatments prevalent in the treatment of borderline patients, namely, dialectic behavior therapy, supportive psychotherapy based on psychoanalytic theory, and schema focused therapy. It concludes with indications and contraindications to this particular therapeutic approach derived from the clinical experience that evolved in the course of the sequence of research projects leading to the empirical establishment of its efficacy.  相似文献

This study examines the effectiveness of a modified psychodynamic treatment called Transference Focused Psychotherapy (TFP) designed specifically for patients, with borderline personality disorder (BPD). Twenty-three female patients diagnosed with DSM-IV BPD began twice-weekly TFP. Patients were assessed at baseline and at the end of 12 months of treatment with diagnostic instruments, measures of suicidality, self-injurious behavior, and measures of medical and psychiatric service utilization. Compared to the year prior to treatment, the number of patients who made suicide attempts significantly decreased, as did the medical risk and severity of medical condition following self-injurious behavior. Compared to the year prior, study patients during the treatment year had significantly fewer hospitalizations as well as number and days of psychiatric hospitalization. The dropout rate was 19.1%. This uncontrolled study is highly suggestive that this structured and manualized psychodynamic treatment modified for borderline patients shows promise for the ambulatory treatment of these patients and warrants further study.  相似文献

Analysis of calls made to a northern Canadian Inuit crisis line in the territory of Nunavut between 1991 and 2001 revealed that the majority of users were adult females who called to discuss problems primarily related to relationships and loneliness/boredom. Younger callers tended to make prank calls. The volunteer staff used mostly empathetic listening and suggestions. Referral recommendations made were primarily to social services. Although some callers experienced a language barrier, others found the service to be helpful. Results suggest that the crisis line was underused by young Inuit males who represent a group that are most in need of crisis intervention.  相似文献

Remembering and forgetting reflect fundamentally interdependent processes in human memory (Bjork, 2011). This interdependency is particularly apparent in research on retrieval-induced forgetting, which has shown that retrieving a subset of information can cause the forgetting of other information (Anderson et al. Journal of Experimental Psychology: Learning, Memory, & Cognition 20:1063-1087, 1994). According to one prominent theoretical account, retrieval-induced forgetting is caused by an inhibitory process that acts to resolve competition during retrieval. Specifically, when cues activate competing, contextually inappropriate responses, those responses are claimed to be inhibited in order to facilitate the retrieval of target responses (Anderson Journal of Memory and Language 49: 415-445, 2003; Levy & Anderson Trends in Cognitive Sciences 6: 299-305, 2002; Storm, 2011b). Interest in retrieval-induced forgetting has grown steadily over the past two decades. In fact, a search of the abstracts at the 5th International Conference on Memory (ICOM, York University, 2011) revealed 40 presentations specifically mentioning "retrieval-induced forgetting," and nearly twice that number referring to the concept of inhibition. Clearly, researchers are interested in the empirical phenomenon of retrieval-induced forgetting, and inhibition is gaining increasing attention as a mechanism involved in memory. The goal of the present progress report is to critically review the inhibitory account of retrieval-induced forgetting and to provide direction so that future research can have a more meaningful impact on our understanding of human memory.  相似文献

It is by now well established that the fact that an action or aconsequence was inevitable does not excuse the agent from responsibilityfor it, so long as the counterfactual intervention which ensures thatthe act will take place is not actualized. However, in this paper I demonstrate that there is one exception to this principle: when theagent is aware of the counterfactual intervener and the role she wouldplay in some alternative scenario, she might be excused, despite the fact that in the actual scenario she acts, as we say, of her own freewill. This study reports evidence that schizophrenia patients are significantly impaired in both spatial and object (shape) working memory. A 3-s delay between exposure and recall of targets was used and Bayesian item-response theory was applied to compensate for the tasks' differential difficulty while simultaneously taking account of missing data from participant attrition. Weaker evidence was found that in schizophrenia both domains are equally impaired on average, that spatial and object working memory appear to be more highly correlated with each other in the schizophrenia population than in the normal population, and that schizophrenia patients show greater variability in spatial than object working memory performance.  相似文献

The instructional power of experimental models was recognized by the significant effort directed toward the development of EXPER SIM at several institutions. Owing to a number of factors, interest in these tools appeared to wane. The price/performance characteristics of some new microcomputers suggested that a rekindling of interest in the use of modeling as an inquiry/problem solving tool could possibly be achieved if the large timesharing versions could be successfully migrated to one or more popular micros. We have partially achieved these objectives, having both transported EXPER SIM to the LSI-11 computer and developed, tested, documented, and evaluated many new models in sensory, personality, and social psychology and psychobiology. In addition, we have begun devising techniques to migrate a version to the Apple II and III microcomputers.  相似文献

Aspects of 40 young children's (22 boys, 18 girls; mean age = 51.00 months) recall of related and unrelated item pairs were assessed. Children were primarily white and middle SES. Consistent with previous recall research, boys recalled significantly more pairs, and individual items from pairs, of related gender-typed items than pairs containing unrelated items. Girls recalled significantly more pairs, and individual items from pairs, of related animal items than any other types of related or unrelated pairings. Congruent with the component model of gender role knowledge development, girls showed significantly greater recall of pairs, and individual items from pairs, of same-sex feminine item pairs than other-sex masculine ones. Consistent with previous memory research examining young children's pair recall, boys demonstrated significantly better recall of pairs, and individual items from pairs, containing related same-sex masculine items than pairs with two unrelated items. These data suggest young children conceptualize gender roles as social categories to organize processing of social information.Completion of this project was partially facilitated by an Academic Challenge Award from the University of Toledo.
