Currently there is much debate regarding the ability of mathematical models incorporating epidemiological information or mutation-based risk algorithms to accurately predict a woman's risk of developing breast cancer. Without access to accurate family history information these models have limited use. This study compares different methods of gathering family history information and the impact on subsequent risk assessment. These methods were compared to the gold standard interview with a trained genetics' professional. The amount and accuracy of information provided by primary care doctors' letters was found to be poor and better information was obtained by sending a postal questionnaire directly to the patient. Because of the high number of low-risk women referred to clinic a questionnaire (the FCAT) was designed to provide reassurance and piloted as part of this study. This paper highlights the importance of using appropriate methods to gather the family history information. It presents evidence for the importance of a skilled assessor and the need to allow time for women to discuss the importance of this information with their family.  相似文献

Cerebral white matter abnormalities relate to cognitive functioning in elders. We examine whether this association is (a) independent of mental ability in youth and (b) related to general and/or specific mental abilities. We retested 83 participants of the Scottish Mental Survey of 1932 on a battery of mental tests. Their brains were scanned by magnetic resonance imaging. Three independent ratings (Fazekas) were made of periventricular, and subcortical and deep white matter abnormalities. Structural equation models showed that, irrespective of brain location, white matter abnormalities contributed about 14% of cognitive function variance in old age. Some of this effect might be due to hypertension. This contribution is independent of mental function in early life and is associated with general cognitive ability.  相似文献

High-density EEG was recorded in 12 compensated dyslexics, 6 classified as dysphonetic and 6 as dyseidetic, and in 12 matched controls while they carried out a lexical-decision task. Relative to normal controls, dysphonetics showed higher beta power in anterior relative to posterior regions, while dyseidetics showed higher beta power in posterior relative to anterior regions. Further, controls (but not dyslexics) showed a positive correlation between performance on the task and the ratios of both left-to-right and anterior-to-posterior beta asymmetry. According to the dual-route theory of reading, there are two strategies that can be used in lexical decision: A visual strategy involving visual word identification and direct access to a visual lexicon, and a phonological strategy involving grapheme-to-phoneme conversion and access to a speech output lexicon. Our results therefore suggest compensation through weakness rather than strength, with phonological dyslexics focusing on a grapheme-to-phoneme strategy and dyseidetics focusing on visual word identification.  相似文献

The authors examined whether adolescent major depressive disorder (MDD) was associated with difficulties in young adult functioning and whether differences would remain significant after accounting for nonmood disorder, MDD recurrence, functioning in adolescence, or current mood state. A total of 941 participants were assessed twice during adolescence and at age 24. In unadjusted analyses, adolescent MDD was associated with most young adult functioning measures. Associations were not due to interactions with adolescent comorbidity, but differences in global functioning and mental health treatment appeared as a result of MDD recurrence. Accounting for levels of functioning in adolescence or for current depression at age 24 eliminated the remaining associations. The implications of these findings for efforts to prevent MDD in adolescence are discussed.  相似文献

The valence-enhancement hypothesis argues that because of their active coping strategies, optimists are especially likely to elaborate on valenced information that is of high personal relevance. The hypothesis predicts that as a result, optimists will be more persuaded by personally relevant positive messages and less persuaded by personally relevant negative messages than pessimists. It also predicts that when the message is not personally relevant, optimism and persuasion will not be related in this manner. The results of 3 studies support these predictions and supply evidence against several alternative hypotheses. The possibility that the observed effects are not due to optimism but to the confounding influence of 7 additional variables is also addressed and ruled out. Implications are discussed.  相似文献

Millions of people leave their religion every year. Such defection often results in religious persecution, ostracism, and heightened intergroup conflict. Yet little is known about the underlying perceptions of religious defectors and what intergroup processes predict hostility toward them. In two pre-registered studies (N = 512), we investigated how religious group members' thoughts and feelings about defectors may lead to ostracism and a lack of dialogue. In both studies, group members rated defectors as unlikeable and irrational. Further, the strength of group members' religious group identification (but not religious belief conviction) predicted dislike and unwillingness to relate wisely with defectors (Study 2). Implications for intergroup research and improving the experience of religious defectors are discussed.  相似文献

Family assessment and therapy has emphasized the negative, pathogenic features of the family relationships of persons who have suffered episodes of schizophrenia. The role of the family as the major provider of supportive care for the disabled patient has received much less attention. A case is made for greater emphasis on the coping abilities of families and family therapy methods that specifically increase these functions through enhancing the problem-solving skills of the family unit.  相似文献

This paper reports five experiments that examined memory for repeated and unrepeated pairs of words. In over 40 experimental comparisons, cued recall of the repeated pairs was better than it would have been if the words had been repeated as independent cognitive events. Therefore, memory traces do interact with other traces of the same nominal items. Our account of superadditive recall is that some encodings fail on the final test because they lack a needed piece of information. Specifically, some need additional item-specific information to enable access by the cue, and some need relational information for recall of the target. The second trial is an implicit test of memory, whose results give the system a heuristic basis for standing pat or doing more encoding. If a retrieved encoding needs less than a new one to become a success, it has a good chance of becoming a success, and recall is superadditive. However, if the ease of retrieving the encoding is for the wrong reasons, such as massed repetition, the item remains a failure, and recall is subadditive.  相似文献

Two hundred subjects made decisions based on the data of two binary variables, ethnic origin and employment status. They were asked to specify the information necessary and sufficient for making the decision before doing so. Variables studied were: (1) Type of question. Five questions were used, four of which focused on one of the four instances involved (e.g., Which information is necessary to find out if there is a connection between Asian immigrants and unemployment?). Question five was unbiased. (2) Type of variable. Half of the subjects found two symmetric variables, and half found one symmetric and one asymmetric variable. (3) Level of correlation. Five different correlations were used, three favoring Asians, one favoring Europeans, and one representing a zero correlation. Subjects made logical decisions that were not influenced by any of the experimental conditions. The selection of information before and after making choices was influenced by all variables except level of correlation. The biased questions produced deviations from normative reasoning; the two symmetrical variables and the process of making decisions encouraged normative reasoning.  相似文献
