Antidepressant treatment of children: clinical relapse is unrelated to tricyclic plasma concentrations

Plasma concentrations of imipramine and amitriptyline and their desmethylated metabolites were measured in 20 children being treated for major depressive illness 2 wk. and 5 to 10 wk. after achieving drug dosages of 2.25 mg/kg body weight. At 2 wk. all children had exhibited clinical improvement, but by 10 wk. 4 of the 10 children treated with imipramine and 5 of the 10 children treated with amitriptyline had experienced clinical relapse of depressive symptoms. Tricyclic antidepressant plasma concentrations and ratios were comparable in the subgroups of children who maintained their clinical improvement and those who relapsed. There was no evidence of a systematic decline in plasma tricyclic antidepressant concentrations in those children who relapsed.

Hyperlexia and a variant of hypergraphia

A patient with hyperlexia and hypergraphia is described. The unusual co-occurrence of these two abilities is discussed. Hyperlexia may provide a useful marker to guide educational programming.     

Microcomputer-assisted relaxation

The present pilot study was designed to test the effectiveness of a microcomputer program developed by the authors to induce relaxation and focused attention, which are common to most clinical stress-reduction and hypnotic procedures. A nonclinical sample of 20 adults used the program on an APPLE IIc computer in two 30-min. sessions in a within-subjects design. Repeated measures analyses of variance showed significant decreases in the Spielberger State Anxiety Scale and on a visual analog anxiety scale. Limitations and implications of the study are discussed.     

Compliance with homework to employ self-instructions in controlling ice water pain: an analogue to counseling

40 women were taught to use self-statements to cope with pain caused by ice water. The trainer used four different strategies for reassigning a homework task of using self-statements to cope with pain produced by immersing one hand in ice water. The strategies were equally effective in eliciting compliance with the homework assignment.     

Infant hypotonia, obesity, hypogenitalism and oligophrenia--new viewpoints on the etiology and symptoms of Prader-Willi syndrome

We present four cases of Prader-Willi syndrome. Two of them have an abnormality of a chromosome 15, the other both show different chromosomal abnormalities. Translocations or deletions were found recently in the bands 15q11/12 in about 60% of the cases of Prader-Willi syndrome. The consequences for diagnosis, symptomatology and genetic counselling of the syndrome are discussed.     

The effects of child abuse as seen in adults: George Orwell

The author presents the case of a patient who showed the massive defensive effects seen in people who were abused in childhood. These effects are similar to those described in George Orwell's 1984 and in his autobiographical writings: denial and "doublethink"; masochistic submission to the tormentor; turning of anger against the self and loving "Big Brother"; identifying with the abuser and tormenting others; a burgeoning of anal mechanisms and obsessive phenomena that results in a massive isolation of affects; excessive emotional control alongside outbursts of rage. The interference with memory and emotions compromises identity and humanity. The unforseeable evolution of innate gifts in a child sometimes permits a partial transcendence of these crippling defenses, as Orwell partially transcended what appears to have been the emotional deprivation of his childhood and what he felt to have been the abuse of his schoolboy years.     

Electrophysiological correlates of semantic features

Analyses of averaged auditory evoked responses to a list of 10 positive and 10 negative words led to the identification of a restricted region in the brain response which reliably discriminated between words differing in the semantic features positive and negative. These data support the view that word meanings are composed of more basic components or features and are not stored simply as dictionary items in the brain.
