Managed care and organized systems of care are restructuring the delivery of care in the United States. As care is reorganized, physician practice styles, autonomy, and compensation are undergoing profound changes. To successfully integrate physicians into the new managed systems of care, their organizational relationship to and their compensation within these systems must be carefully considered. This paper first explores physician motivation as it is related to compensation. The paper then describes a variety of emerging organizational designs aimed at aligning the interests of physicians and hospitals. The author considers fully integrated, physician-hospital organizations with target income compensation arrangements to be most suitable to the collective success of professionals and organized systems of care. The paper concludes with a discussion of the many dilemmas and challenges posed by the intertwining of managed care, organized networks of care, and professional compensation. 相似文献

Fee as a variable that could have an impact on college students' perceptions of counseling was explored because of the authentic concerns about college counseling centers needing to charge direct fees for their services. Participants (N = 350) represented a cross-section of potential consumers of psychological services, not just individuals currently receiving services. On an overt level, fees seemed to be less influential in counseling considerations, but were taken into account on a more covert level. This study supports the psychological literature, which suggests that fee is not the most important factor in clients' decisions to seek counseling. Implications for counselors, administrators, researchers, and program planners are discussed. 相似文献
This paper considers the question: what becomes of the notion of a logic as a way of codifying valid arguments when the customary assumption is dropped that the premisses and conclusions of these arguments are statements from some single language? An elegant treatment of the notion of a logic, when this assumption is in force, is that provided by Dana Scott's theory of consequence relations; this treatment is appropriately generalized in the present paper to the case where we do not make this assumption of linguistic homogeneity. Several applications of the resulting concept of a heterogeneous logic are suggested, but the main emphasis is on the formal development. One topic touched on is a certain contrast between the boolean and the intensional sentence-connectives in this more general setting. 相似文献
It is argued that while counsellors have adopted the principle tenets of phenomenology—consciousness and meaning—they have built their work on an interpretation of intuition which is contrary to phenomenological rationale and presents it as a form of knowing which is esoteric and mystical. Phenomenologically, intuition differs from no other way in which sense is made of the world. Intuitions should then, in principle, be accessible to conscious understanding so as to allow counsellors to reflect (cognitively?) on their practice. If the process of intuition were really unknowable, then reflective practice in counselling would not be possible. 相似文献
The Khalidi Library in Jerusalem contains a number of valuable works pertaining to Muslim‐Christian relations during the Ottoman period, and in this paper an effort is made to elucidate one such text, a unique anonymous essay entitled Al‐risala al‐sabiciyya fi ibtal al‐diyana al‐yahūdiyya. As the title suggests, the essay presents itself as a work of a Jewish convert to Islam; pressed to justify what he has done, he gives seven reasons for his conversion, and then lists two sets of seven similarities and differences between Islam and Judaism that have also served to encourage his conversion.
In this paper it is argued that the essay is actually the work of a Christian convert, probably one who wrote in Jerusalem in the late nineteenth century. Seeking to justify his conversion, but hesitant to reject openly the central doctrines of Christianity, he pretends to be a Jewish convert and argues for the superior position of Islam among the three monotheistic faiths. This tactic, while uncommon, is known from other parts of the Near East, and a convert in Jerusalem would have been in an excellent position to become aware of such writings.
While most of his arguments are familiar from the arsenal deployed in disputations of the past, this author's essay is valuable for its illustration of the hesitation and doubt that must have accompanied most acts of conversion, and for the way in which it draws attention to the fact that Christian—Muslim relations—especially in Jerusalem—have always proceeded within the broader context of relations. among the three great monotheistic religions in general.相似文献
Few British organizations utilize handwriting analysis for personality assessment, but in some European countries handwriting analysis is extremely popular. Research has examined the reliability and validity of both methods of assessment, but few studies have directly compared the two. In this study, the personality of 120 subjects was assessed by the Cattell 16PF and by handwriting analysis. Each subject was presented with five handwriting analysis textual reports and five personality textual reports (one of each being their own) and asked to rank order each set in terms of perceived accuracy. The same ranking process was undertaken by each respondent's social partner. The results demonstrated that handwriting reports were ranked at a chance level by Self and by Other and that personality reports were ranked at a well above chance level by Self and by Other. Self-rankings were more accurate than Other-rankings. 相似文献
