The purpose of this study was to assess consumer preferences for predictive genetic testing for Alzheimer disease in the United States. A rating conjoint analysis was conducted using an anonymous online survey distributed by Qualtrics® to a general population panel in April 2011 in the United States. The study design included three attributes: Accuracy (40 %, 80 %, and 100 %), Treatment Availability (Cure is available/Drug for symptom relief but no cure), and Anonymity (Anonymous/Not anonymous). A total of 12 scenarios were used to elicit people’s preference, assessed by an 11-point scale. The respondents also indicated their highest willingness-to-pay (WTP) for each scenario through open-ended questions. A total of 295 responses were collected over 4 days. The most important attribute for the aggregate model was Accuracy, contributing 64.73 % to the preference rating. Treatment Availability and Anonymity contributed 20.72 % and 14.59 %, respectively, to the preference rating. The median WTP for the highest-rating scenario (Accuracy 100 %, a cure is available, test result is anonymous) was $100 (mean?=?$276). The median WTP for the lowest-rating scenario (40 % accuracy, no cure but drugs for symptom relief, not anonymous) was zero (mean?=?$34). The results of this study highlight attributes people find important when making the hypothetical decision to obtain an AD genetic test. These results should be of interests to policy makers, genetic test developers and health care providers. 相似文献
It has become increasingly common for young children to be taken care of by multiple caregivers in China after the socio-economic reforms. Complex migration patterns and high female labour force participation have led to children receiving care from various individuals in different contexts. However, little is known about how childcare arrangements are associated with child health well-being. This study examines various early childhood caregivers and their influences on children’s physical health in China. Using data from the China Health and Nutrition Survey (CHNS 1991–2011) with 3,470 children aged 2 to 6, we first identified different types of childcare arrangements in and outside of the household based on who provides the care, where they provide the care, and the intensity of the care. Then we examined the relationship between various childcare arrangements and health outcomes for children. Overall, household members undertook early childhood care tasks in China, with an increase in grandparents as primary caregivers between 1991 and 2011. The proportion of children receiving formal childcare fluctuated around 20% during this period. The findings suggest that: 1) primary caregiver in the household other than parents is not associated with undesirable physical health outcomes; 2) formal childcare outside the household is associated with higher height and lower BMI scores; 3) primary caregivers in the household, particular grandparents, moderate the association between childcare arrangements outside the household and children’s health outcomes. It yields an implication that early childhood care policies incorporating multiple caregivers would benefit children’s well-being in China.
Nonadherence in the management of chronic illness is a pervasive clinical challenge. Although researchers have identified multiple correlates of adherence, the field remains relatively atheoretical. The authors propose a cognitive-affective model of medication adherence based on social support theory and research. Structural equation modeling of longitudinal survey data from 136 mainly African American and Puerto Rican men and women with HIV/AIDS provided preliminary support for a modified model. Specifically, baseline data indicated social support was associated with less negative affect and greater spirituality, which, in turn, were associated with self-efficacy to adhere. Self-efficacy to adhere at baseline predicted self-reported adherence at 3 months, which predicted chart-extracted viral load at 6 months. The findings have relevance for theory building, intervention development, and clinical practice. 相似文献

We propose a theoretical framework for and, via critical incidents, inductively investigate (a) the situations that trigger moral emotions toward others at work and (b) worker responses to these situations. Critical incidents were collected from a heterogeneous sample of 423 workers. Participants described an incident that caused each of four moral emotions (gratitude, anger, admiration, and contempt) and their response. Incidents and responses were coded, and frequencies, differences across the moral emotions, and associations between incidents and responses were analyzed. Several morally relevant situations at work were found to trigger the moral emotions. Participants responded in ways that had implications for their performance and their own well-being. The incidents largely align with moral foundations theory (Haidt and Graham in Social Justice Research, 20, 98–116, 2007). Responses also show evidence for social exchange, social learning, and rational expectancy-based processes and behavior. The incidents specify several practices for managers and organizations to encourage and avoid. This was the first qualitative study of moral emotions at work and one of the few workplace studies to examine these emotions empirically. We discuss measuring and detecting influential observations and outliers in the context of exponential family random graph (ERG) models for social networks. We focus on the level of the nodes of the network and consider those nodes whose removal would result in changes to the model as extreme or “central” with respect to the structural features that “matter”. We construe removal in terms of two case-deletion strategies: the tie-variables of an actor are assumed to be unobserved, or the node is removed resulting in the induced subgraph. We define the difference in inferred model resulting from case deletion from the perspective of information theory and difference in estimates, in both the natural and mean-value parameterisation, representing varying degrees of approximation. We arrive at several measures of influence and propose the use of two that do not require refitting of the model and lend themselves to routine application in the ERGM fitting procedure. MCMC p values are obtained for testing how extreme each node is with respect to the network structure. The influence measures are applied to two well-known data sets to illustrate the information they provide. From a network perspective, the proposed statistics offer an indication of which actors are most distinctive in the network structure, in terms of not abiding by the structural norms present across other actors. 相似文献
Moral relativism familiar in the Western philosophical tradition, according to David Lyons, is either agent relativism (moral judgments are relative to the standards of the agent or the agent group) or appraiser relativism (moral judgments are relative to the standards of the appraiser(s) or appraiser group(s)). As Lyons has convincingly argued, they are both problematic. However, in the ancient Chinese Daoist classic, the Zhuangzi, we can find a different type of moral relativism, which I call patient relativism (moral judgments are relative to the patients’ standards). In the essay, I aim to argue in what sense Zhuangzi is a patient relativist and how patient relativism can avoid the problem of agent relativism and appraiser relativism. 相似文献
