Three stages of medical dialogue

The negative consequences of physicians' failure to establish and maintain personal relationships with patients are at the heart of the “humanistic crisis” in medicine. To resolve this crisis, a new model of doctor-patient interaction is proposed, based on the ideas of Martin Buber's philosophy of dialogue. This model shows how the physican may successfully combine the personal (I-Thou) and impersonal (I-It) aspects of medicine in three stages. These “Three Stages of Medical Dialogue” include:

1. An Initial Personal Meeting stage, which initiates the doctor-patient relationship and involves mutual confirmation;
2. An Examination stage, which requires a shift from a personal to an impersonal style of interaction;
3. An Integration Through Dialogue or “Healing Through Meeting’ Stage, which involves the integration of the impersonal medical data into the ongoing dialogue between doctor and patient, as a basis for shared decision-making.

The use of the model, as well as common failures of doctor-patient dialogue are discussed.     

Radical interpretation and the permutation principle

Davidson has claimed that to conclude that reference is inscrutable, one must assume that If some theory of truth ... is satisfactory in the light of all relevant evidence ... then any theory that is generated from the first theory by a permutation will also be satisfactory in the light of all relevant evidence. However, given that theories of truth are not directly read off the world, but rather serve as parts of larger theories of behavior, this assumption is far from self-evident. A proper understanding of the role truth theories play in theories of interpretation makes the inscrutability of reference much less wide-spread than Davidson suggests, and, as a result, the radical interpretation methodology is much less likely to saddle its defenders with counterintuitive cases of indeterminacy than is commonly supposed.     

Population psychology in perspective(1)

Many aspects of population, from fertility regulation to migration, may be viewed as examples of human behavior occurring within a particular socio-economic and environmental context, and thus appropriate for psychological research. Especially considered in this paper are (1) the current status of population growth and the importance of population processes as research topics for psychologists; (2) basic assumptions concerning the complementarity of behavioral and biomedical sciences in population research; (3) behavioral science contributions, specifically from psychology; and (4) recommendations for cooperative multidisciplinary efforts.

Magnitude scales,category scales,and the general psychophysical differential equation

The general psychophysical differential equation (GPDE) which relates scale values and Weber functions for two subjective variables was tested statistically with data from a number of category rating and magnitude estimation experiments. In all tests. it was investigated whether the GPDE is compatible with the assumption that the Weber function of the category scale is constant. This assumption implies that the category scale may be regarded as a discrimination scale. The tests were carried out by estimating the Weber function of the category scale by means of the GPDE and testing these estimates for constancy, For group data, there were fairly small systematic deviations from constancy across different experiments in the Weber functions that were estimated by the GPDE, and the hypothesis of a constant estimated Weber function could not be rejected statistically (p>.05) in most experiments. For individual data the estimated Weber functions deviated from constancy in idiosyncratic ways, and these deviations were statistically significant (p<.05)for most subjects.     

Roles of anoxia and noise-induced hearing loss in the postictal refractory period for audiogenic seizures in mice.

The present series of experiments demonstrated a postictal refractory period for audiogenic seizures in DBA/2J mice, which was not related to hearing loss but apparently was related to anoxia. Unlike many previous studies, Experiment 1 controlled for the effects of noise exposure upon hearing sensitivity and demonstrated reduced susceptibility to subsequent audiogenic seizures for at least 1 hr after initial clonic-tonic convulsions. The postictal refractory period was shown to result from the occurrence of seizures per se, not from noise exposure alone. Experiment 2 demonstrated deficiencies of sensorimotor functions that accompanied reduced postictal seizure susceptibility. The two phenomena had similar time courses of recovery, which suggested a common mechanism, probably anoxia, associated with the initial convulsions. In support of this view, Experiment 3 showed that recovery from both phenomena was expedited by allowing subjects to breathe increased O2. The role of anoxia in fatal convulsions was suggested by the finding that subjects experiencing clonic-tonic convulsions in a high-O2 environment survived without exception. In contrast, seizures of air-breathing controls were almost always fatal. Taken together, the data indicate that the postictal reduced susceptibility to audiogenic seizures was closely related to metabolic depletion (in particular, anoxia). The pattern of recovery of susceptibility further suggests that the effects of anoxia impair the spread of seizure activity through the central nervous system, although the initiation of seizures is also affected for a short time.
