Cortical plasticity associated with stuttering therapy   总被引:1,自引:0,他引:1  
Neuroimaging studies have indicated that persistent developmental stuttering (PDS) may be associated both with an abnormality in white matter of left-hemispheric speech areas and a right-hemispheric hyperactivity. The latter may compensate for the deficient structural connectivity in the left hemisphere. To investigate the effects of stuttering therapy on brain activity nine male adults with PDS underwent functional magnetic resonance imaging (fMRI) before and within 12 weeks after fluency shaping therapy. Brain response differences during overt sentence reading before and after therapy were assessed by utilizing random effects analyses. After therapy, a more widespread activation was observed in frontal speech and language regions and temporal areas of both hemispheres, particularly and more pronounced on the left side. Interestingly, distinct posttreatment left-sided activation increases were located directly adjacent to a recently detected area of white matter anomaly [M. Sommer, M.A. Koch, W. Paulus, C. Weiller, C. Buchel (2002). Disconnection of speech-relevant brain areas in persistent developmental stuttering. The Lancet, 360, 380-383] suggesting that fluency shaping techniques reorganize neuronal communication between left-sided speech motor planning, motor execution, and temporal areas. Hence, a therapeutic mechanism can be assumed to remodel brain circuitry close to the source of the dysfunction instead of reinforcing compensation via homologous contralateral brain networks. EDUCATIONAL OBJECTIVES: The reader will learn about and be able to: (1) describe brain activation changes detected shortly after fluency-shaping therapy; (2) identify left-hemispheric regions where a (re)functionalization after fluency-shaping therapy seems to occur adjacent to a recently described abnormal white matter region in PDS subjects; and (3) discuss how an effective cerebral compensation mechanism for stuttering could work.  相似文献

Zusammenfassung Die Transaktionsanalyse (Berne 1958) sollte vom ursprünglichen Konzept her dazu dienen, personale Strukturen und psychodynamische Prozesse auch dem Laien verstehbar und nutzbar zu machen. Obwohl dieser Anspruch bei manchen Autoren zu Oberfl?chlichkeit und „happy-go-lucky” Sicht verw?sserte, zeigt die vor allem unter europ?ischen Transaktionsanalytikern übliche Rückbesinnung auf die ursprünglichen Inhalte den Umfang klinischer Relevanz der Konzepte für Verst?ndnis und Behandlung der verschiedensten St?rungen. Dies anhand einer sog. frühen St?rung, dem Borderlinesyndrom, aufzuzeigen, ist Anliegen des vorliegenden Beitrags. Das Strukturmodell der Ich-Zust?nde dient dabei als Beschreibungsansatz für die Borderlinestruktur, Bezugsrahmen und Skript als Muster für die Entstehung und Aufrechterhaltung der Pathologie, Transaktionen, Spiele, Symbiosen und Passivit?t als Merkmale stabiler Instabilit?t der Beziehungen und Abwertungen und Grandiosit?ten als Beispiele für Abwehrmuster. Es folgen praxisorientierte überlegungen und Hinweise zum therapeutischen Vertrag als Grundlage des Arbeitsbündnisses, zu Methoden der Ent?ngstigung im Therapieproze?, zu Begegnung und Beziehung als Substrat für korrigierende Erfahrungen, heilendes Klima und Entdramatisierung des realen Geschehens. Die Ausführungen enden mit transaktionsanalytischen überlegungen zu übertragung, Gegenübertragung und dem Umgang mit Regressionsprozessen.   相似文献

Parents of 85 blind children aged from 10 months to the 6th year of life were asked regarding the frequency, duration and typical situations of the occurrence of various stereotypic behavors in their children. The Bielefeld Parents' Questionnaire for Blind and Sighted Infants and Preschoolers was used as the instrument of measurement. All of the children displayed at least one stereotypic behavior; most displayed several stereotypic behaviors according to the parents' reports. Eye poking and body rocking dominated within the prevalence hierarchy. Four typical situations could be identified in which stereotypic behaviors were shown: monotony, arousal, demand, and during feeding or eating. The results suggested that repetitive hand and finger movements, stereotypic manipulation of objects, and making a face(s) mainly occur within arousal situations whereas eye poking, whimpering, and sucking thumbs or fingers especially are linked to monotony.This research was carried out within a research program on early intervention and family care for blind infants and preschoolers. In this paper non-normal worlds semantics is presented as a basic, general, and unifying approach to epistemic logic. The semantical framework of non-normal worlds is compared to the model theories of several logics for knowledge and belief that were recently developed in Artificial Intelligence (AI). It is shown that every model for implicit and explicit belief (Levesque), for awareness, general awareness, and local reasoning (Fagin and Halpern), and for awareness and principles (van der Hoek and Meyer) induces a non-normal worlds model validating precisely the same formulas (of the language in question).  相似文献

A suggestion is made for representing iterated deontic modalities in stit theory, the “seeing-to-it-that” theory of agency. The formalization is such that normative sentences are represented as agentive sentences and therefore have history dependent truth conditions. In contrast to investigations in alethic modal logic, in the construction of systems of deontic logic little attention has been paid to the iteration... of the deontic modalities. L. Goble (1966, p. 197) N. Belnap and P. Bartha (1995) present a formalization of iterated deontic modalities in stit theory, the “seeing-to-it-that” theory of agency, due to Belnap, Perloff, and Xu (1996). In the present paper a simplification of Belnap and Bartha's approach is suggested. In order to support easy comparison with (1995), I shall take up Belnap and Bartha's discussion of R. Barcan Marcus' (1966) example Parking on highways ought to be forbidden. The simplified account underlines the power of stit theory and represents obligations, prohibitions and permissions as agentive sentences, as required by iteration. Therefore, in particular, the truth of obligations, prohibitions and permissions is it history dependent. To investigate why men offer sperm donation via the internet, a questionnaire was disseminated via 39 Dutch-language websites thought to be visited by potential sperm donors. Nine internet donors completed the survey, men who typically knew the women they were donating to. Their responses were compared with those of a control group of 35 general sperm bank donors who were recruited using flyers in Dutch sperm banks, and who were typically unaware of the identity of the eventual recipients. The findings shed light on the motives and attitudes of internet donors. Both groups of donors indicated that their primary motivation for donating was altruism (>80% of all respondents). However, internet donors had a more pronounced desire to procreate than sperm bank donors (6 out of 9, i.e. 66 vs. 22%, respectively) and more often felt that they had good genes they wished to pass on (5 out of 9, i.e. 55 vs. 31%, respectively). The main reason internet donors gave for donating via the internet was that they wanted to know the prospective parents and be kept up to date on the progress of the offspring conceived from their donations. This distinguishes them significantly from sperm bank donors. A further finding was that they were not prompted to avoid the formal donation circuit for which, by law in the Netherlands, pregnancies resulting from donations have to be registered in a central database. This study is subject to several, in some cases inevitable, limitations, but it provides an interesting starting point that future studies can seek to confirm and extend.  相似文献

Time along with space is one of the two fundamental dimensions of life. Whereas spatial aspects have been considered in experiments with marine mammals, research has so far not focused on timing per se although it is most likely involved in many behaviours such as foraging or navigation. This study investigated whether harbour seals possess a sense of time and how precisely they are able to discriminate time intervals. Experiments took place in a chamber that allowed keeping ambient illumination constant at 40 lx. The animal was presented with a white circle on a black background on a monitor displayed for a preset time interval. In a two-alternative forced-choice experiment, the animal had to indicate the presence of the standard or a longer comparison time interval by moving its head to one out of two response targets. Time difference thresholds were assessed for various standard intervals between 3 to 30 s adopting a staircase procedure. The experimental animal found access to the task easily and discriminated time intervals with difference thresholds partly in the millisecond range. Thus our study revealed a well-developed sense of time in a pinniped species. Time, besides information provided by the classical senses, is thus most likely an important parameter seals can rely on for various tasks including navigation and foraging.  相似文献

The knowability paradox is an instance of a remarkable reasoning pattern (actually, a pair of such patterns), in the course of which an occurrence of the possibility operator, the diamond, disappears. In the present paper, it is pointed out how the unwanted disappearance of the diamond may be escaped. The emphasis is not laid on a discussion of the contentious premise of the knowability paradox, namely that all truths are possibly known, but on how from this assumption the conclusion is derived that all truths are, in fact, known. Nevertheless, the solution offered is in the spirit of the constructivist attitude usually maintained by defenders of the anti-realist premise. In order to avoid the paradoxical reasoning, a paraconsistent constructive relevant modal epistemic logic with strong negation is defined semantically. The system is axiomatized and shown to be complete.  相似文献
