Many children with chronic genetic diseases are followed by specialty clinics that provide genetic information as part of the care. Health services restrictions in the Republic of Ireland (ROI) can make the wait for an appointment with a genetic counsellor long. We examined whether genetic information was being adequately understood when presented by medical, but non-genetics staff to long term patients, using our national metabolic service as an example. The aim was to inform health professionals about the need or role of a genetic counsellor in a specialist setting. A questionnaire was used to assess knowledge among parents and patients affected by galactosaemia and Maple Syrup Urine Disease (MSUD). Twenty seven families with galactosemia and 10 with MSUD were interviewed in clinic. Comparative analysis showed significant differences in knowledge between parents of children with galactosemia and adult patients (p = 0.001) and between ethnicities (p > 0.05). While parents are well informed, the majority expressed a wish for more information about the condition and its transmission. Adult patients with galactosemia and parents from certain ethnic backgrounds could especially benefit from genetic counselling. This study highlights the need for a genetic counsellor in specialist clinics.  相似文献

The reciprocal exchange of employees' work for pay that is central to employment relationships is viewed here through the lens of the psychological contract. A psychological contract involves promised inducements, promised contributions, delivered inducements, and delivered contributions: How an employee cognitively integrates these 4 elements is a central question in psychological contract theory. Three alternative approaches for integrating the 4 elements were drawn from discrepancy theory, from equity theory, and from need theories of satisfaction, respectively. Experimental findings disconfirmed the discrepancy and equity approaches. Findings were consistent with the premise of the needs model, which is that appraisal is driven by how psychological contract elements facilitate or hinder an employee's effort to fulfill personal needs. Results showed that promised and delivered pay and work contribute uniquely to appraisal but that they vary in their influence on appraisal. These findings were consistent with the needs model principle that elements proximal to need satisfaction matter more than distal elements. That is, what is delivered (for pay and for work) matters more than what is promised, and pay matters more than work.  相似文献

The authors evaluated the psychometric properties of the Firefighter Coping Self-Efficacy (FFCSE) Scale, a new measure developed to assess firefighters' perceived competence in managing stressful and traumatic experiences encountered on the job. Two samples of firefighters completed the FFCSE Scale at two different time points. Exploratory factor analysis yielded a unidimensional structure, which was further supported with confirmatory factor analysis using a second sample. Internal consistency of the measure was excellent. Analysis of cross-sectional data indicated FFCSE was positively associated with measures of psychological well-being and social support, and negatively associated with work-related stress and psychological distress. FFCSE also uniquely contributed to the variance in psychological distress, over and above variables previously shown to be associated with distress among this population. Implications and suggestions for future research are discussed.  相似文献

The interfering effect of an unattended stimulus on processing of an attended item was studied in a single split-brain participant (LB) and in normal controls. Pairs of letters were presented to the left visual field (LVF), right visual field (RVF), or bilaterally. Participants attended to the rightmost letter while attempting to ignore the leftmost letter. Responses associated with the attended and to-be-ignored letters could be compatible or incompatible. Manual response latencies were generally slower on Response Incompatible compared to Response Compatible trials. Notably, LB displayed this effect on Bilateral trials, where target and distractor were presented to opposite visual fields. LB was unable to perform a same-different matching task with bilateral letter stimuli, but was able to name bilateral letters accurately. Hence, in the bilateral condition, the ability to cross-compare letters was dissociated from attentional interference and from letter naming. Implications of these findings are discussed.  相似文献

Previous epidemiological studies of correlates of child and adolescent mental disorders in the general population have focused more on child/adolescent and socioeconomic/sociodemographic characteristics than on family characteristics. Moreover, there are no generally accepted methods to analyze and interpret correlates. The purpose of the Quebec Child Mental Health Survey in this regard was twofold: (1) to identify correlates of DSM-III-R internalizing and externalizing disorders according to informant (youth, parent, teacher), for three age groups (6–8, 9–11, and 12–14 years), including relevant family characteristics not considered in previous studies; and (2) to interpret the relative importance of risk indicators by ranking correlates according to strength and consistency of association across age groups. Logistic regression models suggest the inconsistency of correlates across informants. The ranking of correlates reveals that individual and family characteristics make a more important contribution than do socioeconomic characteristics, thereby supporting the relevance of proximal variables in the development of psychopathology.  相似文献

Although past research has indicated that stress and the accompanying increase in glucocorticoids compromises hippocampal neurons, little is known about the effect of stress on hippocampal glial cells. In the current study, male rats were exposed to activity-stress (A-S) for six days; this comprised housing with an activity wheel and restricted access (1h/day) to food. Physiological data (e.g., relative adrenal and thymus weights, gastric ulceration) suggested that the A-S rats experienced more stress than pair-fed (no wheel) and control (fed ad libitum, no wheel) rats. Whereas stress did not influence the quantitative morphology of glial fibrillary acidic protein (GFAP)-immunoreactive cells, a semi-quantitative analysis revealed that the A-S rats had significantly more (30%) GFAP-immunoreactive cells in the hippocampal CA3 region than the control rats. Based on the present findings, it appears that the hippocampal astrocytic response to chronic stress may be similar to the response found in endangered, or challenged hippocampal environments, such as in ischemia.  相似文献

The cognitive processing strategies of two groups of French-English bilinguals were studied by means of an auditory Stroop test designed to evaluate cerebral hemispheric involvement. An “early bilingual” group were bilingual before the age of 5, and a “late bilingual” group were bilingual after the age of 10. Stimuli were words uttered in pitches that were related to word meanings either congruently (as in the word “high” uttered in a high pitch) or incongruently (the word “haute” uttered in a low pitch). In one condition, subjects were to differentiate low from high pitches, disregarding meaning, while in a second condition, they were to disregard pitch and respond to word meanings. Measures of field independence were also taken. Results of data analyses suggest that male early bilinguals—the most field independent subgroup—process meaning efficiently in both cerebral hemispheres, but process pitch better in the right hemisphere. However, male late bilinguals and female bilinguals, both early and late, process meaning more rapidly in the right cerebral hemisphere and pitch equally rapidly in both hemispheres. The findings are interpreted as reflecting hemisphere-based strategy and sex differences in information-processing by the two bilingual groups.  相似文献
