Objective: Patients with eating disorders (ED) often suffer from co-morbid depression, which may complicate the ED treatment. Previous studies have found that ED interventions seem to have limited capacity to reduce depressive symptoms. Several studies of interpersonal psychotherapy (IPT), have found that when patients have been treated for depression, co-morbid symptoms have diminished. As depression and EDs are commonly co-occurring conditions, this pilot study aimed to examine the effect of an IPT treatment for these conditions, with the focus on the depressive symptoms.
Method: In this multi-centre study, 16 patients with EDs and co-occurring major depression received 16 weeks of depression-focused IPT.
Results: Significant improvements with substantial effect sizes were found for both depression (d = 1.48) and ED (d = .93). Symptom reduction in the two syndromes were strongly correlated (r = .625, p = .004). Patients with a restrictive ED did not improve on either depression or ED symptoms.
Conclusion: These findings point to the usefulness of IPT for concurrent depression and ED with a bingeing/purging symptomatology. Working with negative affect and problem-solving related to current interpersonal problems may alleviate general psychological distress among these patients. 相似文献
Four studies developed and validated two dictionaries to capture agentic and communal expressions in natural language. Their development followed the Linguistic Inquiry and Word Count (LIWC) approach (Study 1) and we tested their validity with frequency-based analyses and semantic similarity measures. The newly developed Agency and Communion dictionaries were aligned with LIWC categories related to agency and communion (Study 2), and corresponded with subjective ratings (Study 3), confirming their convergent validity. Very low or absent correspondence between proposed dictionaries and unrelated LIWC categories demonstrated their discriminant validity (Study 2). Finally, we applied both dictionaries to language used in advertisements. In correspondence to gender stereotypes, male-dominated jobs were advertised with more agentic than communal words, and female-dominated jobs with more communal than agentic words (Study 4). Both dictionaries represent reliable tools for quantifying agentic and communal content in natural language, and will improve and facilitate future research on agency and communion. 相似文献
In this paper we articulate confirmation and disconfirmation as components in human motivation. We develop a theory of motivation on the basis of a model of human action and we explore aspects of confirmation and disconfirmation in the context of the meeting of dysphagic patients with their physicians. We distinguish four central elements in confirmation and disconfirmation and use these and the relations between them for the purpose of constructing a typology. Finally, on the basis of the results obtained we interpret a small volume of remarks reflecting the meaning field of some dysphagic patients in relation to their physicians. The underlying motive is to develop tools for understanding health care processes. The “SAUC-Confirmation-Model” and the theoretical framework in which it is embedded should be seen from that point of view. 相似文献
The achievement of optimal therapeutic results presupposes the use of appropriate treatment combined with maximal utilization of placebo effects. These aims may sometimes be difficult to satisfy in randomized clinical trials (RCTs). The question thus arises whether there is a conflict between the goals of therapy and those of experimental research; and if so, to what extent, and how is it handled in practice by clinicians and researchers. Various ethical problems have been discussed in several reports connected with RCTs. But we have found no discussion concerning the conflict between obtaining informed consent and promoting optimal placebo effects. Information about RCTs can be given in various ways. Sometimes appropriate information about RCTs to patients involves non-optimal utilization of placebo effects. This gives rise to ethical and methodological problems, which are discussed in this article. 相似文献
In this paper, I argue against defining either of ‘good’ and ‘better’ in terms of the other. According to definitions of ‘good’ in terms of ‘better’, something is good if and only if it is better than some indifference point. Against this approach, I argue that the indifference point cannot be defined in terms of ‘better’ without ruling out some reasonable axiologies. Against defining ‘better’ in terms of ‘good’, I argue that this approach either cannot allow for the incorruptibility of intrinsic goodness or it breaks down in cases where both of the relata of ‘better’ are bad. 相似文献
Abstract In their discussions and criticisms of the idea that language use is essentially a matter of following rules, Davidson and Cavell both invoke as counterexamples instances of intelligible linguistic innovation. Davidson’s favorite examples are malapropisms. Cavell focuses instead on what he calls projections. This paper clarifies some important differences between malapropisms and projections, conceived as paradigmatic forms of linguistic innovation. If malapropisms are treated as exemplary it will be natural to conclude, with Davidson, that a shared practice, be it rule-governed or not, matters only instrumentally – as something that may enhance but is neither necessary nor sufficient for successful communication. By contrast, if Cavellian projections are seen as exemplary, a shared practice will be conceived not only as essential to the possibility of meaningful linguistic innovation, but as already permeated by the sort of creativity of which projections are only particularly striking examples. It is also argued that malapropisms are not particularly convincing as counterexamples to the sort of view Davidson wants to reject. Cavellian projections, on the other hand, are powerful as counterexamples, and reflecting on the nature of their inventiveness is crucial to understanding and seeing the plausibility of Cavell’s own conception of language. 相似文献
Abstract Burnout has been highlighted as an important issue, not only in occupational settings but also among athletes. Optimists appear to be more resistant to burnout, which might be partly explained by lower levels of stress. The purpose of this study was to investigate the relationship between optimism and burnout symptoms in 217 athletes (139 males and 78 females, aged 16 to 19 years), while also examining stress as a mediator in this relationship. The results showed that optimism had a significant negative relationship with both stress and burnout. Mediation analyses indicated that perceived stress fully mediated the links between optimism and two symptoms of burnout, emotional/physical exhaustion and sport devaluation, and partly mediated the link between optimism and a third symptom, reduced sense of accomplishment. The findings indicate that individual factors, such as optimism, may play an important role in the development of burnout by virtue of their association with stress. Future research should, therefore, investigate the longitudinal effects of optimism on stress and burnout. 相似文献
