Illness perception (IP) concerns how patients evaluate living with a disease. To get a broader understanding of IP in patients with chronic obstructive pulmonary disease (COPD), we investigated whether breathlessness is an important precursor of IP and whether IP in its turn is related to mental health, physical health and global quality of life (QOL). One hundred and fifty‐four patients with COPD participated in a cross‐sectional survey. Participants underwent pulmonary function testing, provided socio‐demographic and clinical information, and completed the following standardized instruments: Brief Illness Perception Questionnaire, Respiratory Quality of Life Questionnaire, Short‐Form 12 Health Survey and the Quality of Life Scale. Multiple regression analyses were performed. A high IP score indicates that a patient believes that his/her illness represents a threat. Participants with a high score on the IP dimensions consequences, identity, concern and emotional representation, experienced more breathlessness. High scores on the IP dimensions consequences, identity and concern were associated with impaired physical health and high scores on the IP dimensions consequences, identity and emotional representation were associated with impaired mental health. Impaired global QOL was associated with high scores on the IP dimensions consequences, identity, concern, coherence and emotional representation. The strength of the associations between breathlessness and physical/mental health and global QOL decreased when certain dimensions of IP were included as predictors, indicating that IP to some extent acts as a mediating factor. These findings may have practical implications of patient counselling by helping COPD patients to cope with their disease by restructuring their personal models of illness.  相似文献

Recent studies have emphasized the importance of the prenatal period for children's cognitive development. Prenatal exposure to psychological distress has been identified as one potential agent affecting neurodevelopment, although research in this area has been marked by some contradictory findings and methodological limitations. This study aimed to investigate the effect of maternal distress during pregnancy on language development of preschool children. This study was based on the Norwegian Mother and Child Cohort Study conducted at the Norwegian Institute of Public Health. The sample was composed of 34,089 women and their children. We assessed the role of maternal prenatal distress on child language over and above that of premature low birth weight (PLBW) status and postnatal maternal distress. Fear of giving birth had a negative impact on language outcome at age three over and above the effect of PLBW status and postnatal maternal distress. However, this effect was of minimal magnitude. Moreover, when examining whether differences in language skills between siblings were associated with differences in experienced levels of prenatal stress across pregnancies (sibling fixed-effects models), no significant effects were found. Our study suggests that prenatal distress within moderate limits does not seem to have an independent impact on child language performance in the preschool years and emphasizes the importance of considering the contextual role played by more stable maternal and family characteristics.  相似文献

Metacognitive evaluations refer to the processes by which people assess their own cognitive operations with respect to their current goal. Little is known about whether this process is susceptible to social influence. Here we investigate whether nonverbal social signals spontaneously influence metacognitive evaluations. Participants performed a two-alternative forced-choice task, which was followed by a face randomly gazing towards or away from the response chosen by the participant. Participants then provided a metacognitive evaluation of their response by rating their confidence in their answer. In Experiment 1, the participants were told that the gaze direction was irrelevant to the task purpose and were advised to ignore it. The results revealed an effect of implicit social information on confidence ratings even though the gaze direction was random and therefore unreliable for task purposes. In addition, nonsocial cues (car) did not elicit this effect. In Experiment 2, the participants were led to believe that cue direction (face or car) reflected a previous participant's response to the same question—that is, the social information provided by the cue was made explicit, yet still objectively unreliable for the task. The results showed a similar social influence on confidence ratings, observed with both cues (car and face) but with an increased magnitude relative to Experiment 1. We additionally showed in Experiment 2 that social information impaired metacognitive accuracy. Together our results strongly suggest an involuntary susceptibility of metacognitive evaluations to nonverbal social information, even when it is implicit (Experiment 1) and unreliable (Experiments 1 and 2).  相似文献

This study investigated twenty four 18‐month‐olds’ memory for dynamic visual stimuli. During the first visit participants saw one of two brief movies (30 seconds) with a simple storyline displayed in four iterations. After 2 weeks, memory was tested in the visual paired comparison paradigm in which the familiar and the novel movie were contrasted simultaneously and displayed in two iterations for a total of 60 seconds. Eye‐tracking revealed that participants fixated the familiar movie significantly more than the novel movie, thus indicating memory for the familiar movie. Furthermore, time‐dependent analysis of the data revealed that individual differences in the looking‐patterns for the first and second iteration of the movies were related to individual differences in productive vocabulary. We suggest that infants’ vocabulary may be indicative of their ability to understand and remember the storyline of the movies, thereby affecting their subsequent memory.  相似文献

The question which this paper examines is that of the correct scope of the claim that extra‐linguistic factors (such as gender and social status) can block the proper workings of natural language. The claim that this is possible has been put forward under the apt label of silencing in the context of Austinian speech act theory. The ‘silencing’ label is apt insofar as when one's ability to exploit the inherent dynamic of language is ‘blocked’ by one's gender or social status then one might justly be said to be silenced. The notion that factors independent of any person's linguistic competence might block her ability to exploit the inherent dynamic of language is of considerable social as well as theoretical significance. I shall defend the claim that factors independent of a person's linguistic competence can indeed block her ability to do things with words but I will show that the cases that have been previously considered to be cases of illocutionary failure are instances of rhetic or locutionary act failure instead. I shall refine the silencing claim as previously advanced in the debate in at least one fundamental respect. I also show that considering the metaphysics of speech acts clarifies many of the issues previously appearing as thorny bones of contention between those who hold that the only notion of silencing that is coherent is that of physically preventing someone from speaking or writing and those who hold the opposite sort of claim sketched above.  相似文献

The paper discusses whether the rule of abstinence is still valid in the contemporary theoretical arena of psychoanalysis. Not only is this a debate on principle. It also raises the question of how gratifying the analyst should be in the analytic encounter, still remaining within the psychoanalytic paradigm. Current criticisms of the concept of abstinence are reviewed. The author replies to the objections. He argues that the so-called rule of abstinence is not a “rule” which describes the particular behaviour of the analyst, but a general principle describing the essence of the psychoanalytic cure. In the opinion of the author, abandoning this principle may have detrimental consequences both theoretically and technically. Finally, the author discusses the emotional presence of the analyst in the analytic setting and suggests an updated version of the idea of analytic abstinence.  相似文献

Global measures of trunk sway are traditionally used even though the trunk comprises a multiple number of segments. The authors’ aim was to measure the seated sway of typically developing children using a multisegment approach. Twenty typically developing children divided into 2 groups, older and younger than 10 years old, participated in this study. The children sat unsupported for 30 s while their posture and sway were quantified using stereophotogrammetry. The tendency in both age groups was to sit with a backward tilted pelvis and a kyphotic trunk. The sitting position was most varied in the younger group. Marker sway amplitude and velocity in sitting were age dependent, with reduced sway amplitude and velocity with increased age for all segments. Anteroposterior intersegmental angular sway was not age dependent. The difference in marker sway in the anteroposterior direction for the younger group appeared to result from an equally stable trunk supported on a less stable pelvis. Mediolateral marker sway and intersegmental angular sway showed a clearer age dependency. Trunk postural control does not appear to differ between children older and younger than 10 years old, but sagittal plane pelvic stability can explain the increased sway reported in younger children.  相似文献

The phenomenon of ‘innocent guilt’ regards cases where people feel guilty without being responsible for the wrongdoing or suffering at which the guilt is directed. The aim of this article is to develop a consistent account of innocent guilt and show how it may arise in the aftermath of conflicts. In order to do this, innocent guilt is contrasted with guilt and collective guilt, and the account is substantiated by drawing on the writings of Ludwig Wittgenstein and Emmanuel Levinas, who both consider the phenomenon of innocent guilt a necessary element in the fundamental structure of the ethical subject. These thinkers, furthermore, show a connection between guilt and possible victims of wrongdoing, rather than between guilt and personal acts of wrongdoing. Innocent guilt may thus appear in the aftermath of conflicts as an ethical and emotional response to the fact of finding oneself in a post‐conflict situation still marked by suffering. It thus reveals a fundamental need to contribute to the relieving of such suffering.  相似文献

This article is an attempt to clarify the concept of apatheia as it appears in the Praktikos of Evagrius of Pontus. The condition (katastasis) of apatheia is central to the Evagrian understanding of the goal of monastic life, and the Praktikos is the treatise in the writings of Evagrius which deals most thoroughly with apatheia. The thesis of the article is that apatheia should be understood in terms of the Platonic distinction between stability and movement, and apatheia, thus, appears to be two different kinds of conditions, namely imperfect apatheia, related to the daily ascetic struggle of the monks against the demons and the thoughts, and perfect apatheia, related to the peaceful condition of undisturbed prayer, in which the mind contemplates the holy Trinity.  相似文献
