AbstractBackground: Psychosocial stress and anger trigger cardiovascular events, but their relationship to heart failure (HF) exacerbations is unclear. We investigated perceived stress and anger associations with HF functional status and symptoms.Methods and Results: In a prospective cohort study (BETRHEART), 144 patients with HF (77% male; 57.5?±?11.5 years) were evaluated for perceived stress (Perceived Stress Scale; PSS) and state anger (STAXI) at baseline and every 2 weeks for 3 months. Objective functional status (6-min walk test; 6MWT) and health status (Kansas City Cardiomyopathy Questionnaire; KCCQ) were also measured biweekly. Linear mixed model analyses indicated that average PSS and greater than usual increases in PSS were associated with worsened KCCQ scores. Greater than usual increases in PSS were associated with worsened 6MWT. Average anger levels were associated with worsened KCCQ, and increases in anger were associated with worsened 6MWT. Adjusting for PSS, anger associations were no longer statistically significant. Adjusting for anger, PSS associations with KCCQ and 6MWT remained significant.Conclusion: In patients with HF, both perceived stress and anger are associated with poorer functional and health status, but perceived stress is a stronger predictor. Negative effects of anger on HF functional status and health status may partly operate through psychological stress. 相似文献

Fear arousal—vividly showing people the negative health consequences of life‐endangering behaviors—is popular as a method to raise awareness of risk behaviors and to change them into health‐promoting behaviors. However, most data suggest that, under conditions of low efficacy, the resulting reaction will be defensive. Instead of applying fear appeals, health promoters should identify effective alternatives to fear arousal by carefully developing theory‐ and evidence‐based programs. The Intervention Mapping (IM) protocol helps program planners to optimize chances for effectiveness. IM describes the intervention development process in six steps: (1) assessing the problem and community capacities, (2) specifying program objectives, (3) selecting theory‐based intervention methods and practical applications, (4) designing and organizing the program, (5) planning, adoption, and implementation, and (6) developing an evaluation plan. Acquisition of conditioned responding is thought to be determined by the number of pairings of a conditioned stimulus (CS) and an unconditioned stimulus (US). However, it is possible that acquisition is primarily determined not by the number of trials but rather by quantities that often correlate with the number of trials, such as cumulative intertrial interval (ITI) and the number of sessions. Four experiments examined whether the number of trials has an effect on acquisition of conditioned responding, once cumulative ITI and number of sessions are equated. Results of the experiments with rats and mice favor the hypothesis that over an eightfold range, variation in number of CS-US pairings has little effect. It is suggested that learning curves might more accurately be plotted across cumulative ITI or number of sessions, and not across number of trials. Results pose a challenge to trial-centered accounts of conditioning, as demonstrated by simulations of the Rescorla-Wagner model, a simplified version of Wagner's standard operating procedure model (SOP), and Stout & Miller's sometimes competing retrieval model (SOCR). A time-centered account, rate estimation theory (RET), predicts the main finding but has trouble with other aspects of the learning process more easily accommodated by trial-centered models. 相似文献

Participants (N = 10) made flexions or extensions about the elbow. Movements either were pointing (i.e., self-terminated) or terminated by impact on a barrier. The author examined how the trajectory and the electromyographic (EMG) patterns varied according to the distance moved, the instruction provided concerning speed, or the type of termination. Variations in kinematics induced by changes in the target distance or the instruction regarding speed were the same for impact and pointing movements. In comparison with a pointing movement of similar distance and speed instruction, an impact movement (a) accelerated longer and reached a higher velocity, (b) had a longer agonist EMG burst, and (c) had a low level of contraction that started slightly after the agonist burst and continued throughout the movement but had little or no antagonist burst. Because the different types of movements required different forces from the muscles, there were systematic, task-specific differences in EMG patterns that reflected task-specific differences in central control. The results of this experiment demonstrate that impact movements share some of the rules used in the control of other tasks, such as pointing and reversing movements. Wit explores modes of reading representations of death and dying, both through the play’s sustained engagement with Donne’s Holy Sonnets and through Vivian’s self-reflexive approach to her illness and death. I argue that the play dramatizes reparative readings, a term coined by Eve Kosofsky Sedgwick to describe an alternative to the paranoid reading practices that have come to dominate literary criticism. By analyzing the play’s reparative readings of death and dying (as well as its representation of the shortcomings of paranoid readings), I show how Wit provides lessons about knowledge-making and reading practices in the field of health humanities. 相似文献

Experimentally naive mice matched the proportions of their temporal investments (visit durations) in two feeding hoppers to the proportions of the food income (pellets per unit session time) derived from them in three experiments that varied the coupling between the behavioral investment and food income, from no coupling to strict coupling. Matching was observed from the outset; it did not improve with training. When the numbers of pellets received were proportional to time invested, investment was unstable, swinging abruptly from sustained, almost complete investment in one hopper, to sustained, almost complete investment in the other-in the absence of appropriate local fluctuations in returns (pellets obtained per time invested). The abruptness of the swings strongly constrains possible models. We suggest that matching reflects an innate (unconditioned) program that matches the ratio of expected visit durations to the ratio between the current estimates of expected incomes. A model that processes the income stream looking for changes in the income and generates discontinuous income estimates when a change is detected is shown to account for salient features of the data. 相似文献
