This essay uses Solzhenitsyn's Cancer Ward to explore the exilic effects of illness and pain. The novel is uniquely suited for such an analysis given the theme of exile that predominates both in the narrative and in the composition of multiple characters within that narrative. I argue that illness, and in particular pain, is a liminal state, an existential hinterlands. The ethical approach to literature and medicine may suggest, as a response to these exilic effects, the need to cultivate connection and empathy by sharpening the moral imagination. If pain and illness exile the sufferer, the imperative to reach out takes on ethical content. 相似文献

The objective of this short‐term longitudinal study was to examine the concurrent and prospective associations of infants' sleep arrangements and night waking with cortisol responses to an inoculation at 6 and 12 months, controlling for several key covariates. To our knowledge, this was the first study to concurrently and prospectively link proximity in sleep arrangements and night waking to physiological stress reactivity. A sample of 92 mother–infant dyads participated in the study when the infants were 6 and 12 months of age, although sample sizes were reduced for some analyses. Both proximal cosleeping arrangements and more frequent night wakings' were associated concurrently with an increased cortisol response to inoculations at both ages. Night waking at 6 months also was associated with a slightly increased cortisol response to inoculation at 12 months. Results aimed at exploring the direction of influence suggested that cosleeping and night waking may influence infant stress physiology rather than the reverse. Adaptive and maladaptive implications of infants' nocturnal experiences and greater stress‐induced cortisol responses are discussed. Copyright © 2009 John Wiley & Sons, Ltd. 相似文献

The concept of confidentiality is examined from its absolute position espoused by some psychoanalysts to the many exceptions to this position allowed by others. The suggestion made in this paper is offered as a psychoanalytic one which urges us to see confidentiality as posing risks in both positions, that is, the absolute and the necessary exceptions. Confidentiality is representative of many, if not all, of the rules and methods that are implicit in the conduct of an analysis and thus may well be taken for granted. This paper draws attention to a particular use of the body, one in which basic psychical security is achieved through a radical detachment from body vitality, necessitating the creation of a coercive regime of psychosomatic control and autostimulation for purposes of artificial enlivenment of the self. When the personality is organized predominantly along the lines of a systematic dissociation (and then pseudo-integration) of the mental and the somatic realms of psychical life, the psyche-soma undergoes a far-reaching transmutation: the desiring body is eclipsed and replaced with a fabricated body. Clinical observation of this set-up is obscured by the fact that, in these cases, the body is deployed in a 'realistic' manner, rather than in a recognizably symptomatic way. It is, indeed, the hallmark of a successful somatic false-self organization that the colonization of the body is disguised as natural, ego-syntonic and compatible with social norms. The treatment approach requires a broadening of analytic attention to apprehend shifts in states of consciousness and psychophysical cues. 相似文献

In a sample of 431 white-collar professionals, we examined the impact of gender and self-esteem on individuals' intentions to seek legal counsel, confront the harasser, and make formal reports within the organization in response to approach-based same-sex sexual harassment. We hypothesized that gender, self-esteem, and their interaction would be related to assertive responses. All three hypotheses received support: men were more likely than women to respond assertively; self-esteem was positively related to response intentions; and self-esteem had a greater impact on men's responses than on women's responses. These results suggest that frameworks used to describe responses to cross-sex sexual harassment may not be adequate for same-sex sexual harassment. Post hoc analyses revealed that perceiving the behavior as harassment mediated the relationship between self-esteem and the responses to harassment, but did not mediate the relationships between gender and the responses to harassment. These findings contribute to the research on acknowledging sexual harassment. 相似文献
