Logistic latent trait models with linear constraints

Two linearly constrained logistic models which are based on the well-known dichotomous Rasch model, the ‘linear logistic test model’ (LLTM) and the ‘linear logistic model with relaxed assumptions’ (LLRA), are discussed. Necessary and sufficient conditions for the existence of unique conditional maximum likelihood estimates of the structural model parameters are derived. Methods for testing composite hypotheses within the framework of these models and a number of typical applications to real data are mentioned.     

Changes in depressive-symptom experiences among older women.

Changes in depressive-symptom experiences over a 5-year period were investigated in a community sample of 251 women between the ages of 51 and 92 years. Findings from a confirmatory factor analysis of the SCL-90-R Depression and Additional Symptoms scales (Derogatis, 1983) indicate that two phenomenally different syndromes underlie symptom-reporting patterns. A depressive syndrome, more classic in form, shows decreasing levels with increasing age, although a depletion syndrome, marked by feelings of enervation and a loss of interest in things, shows increasing levels with advancing age. The relative independence of the two syndromes, as well as their differing relations to four more delimited forms of distress that were identified in the analyses, have important implications for future research on the age-depression relation.     

Systems use to objectify program evaluation,clinical, and management decisions

The utility of the observational assessment systems at different levels — from local clinical to systemwide management — is outlined. An overview is provided of TSBC information applied to individualized problem identification and programming/monitoring, discharge and competency determinations, and both absolute and comparative program evaluation. The utility of SRIC information for prgoramming/monitoring and for staff training and evaluation is outlined in addition to the applied uses of the information of the assessment systems in combination. Achieving “Narrative Flow”: Pre-Medical Education as an Essential Chapter of a Physician’s Story

This article explores the disconnection between what pre-professional students expect from college and what their undergraduate education might foster, between the focus on "getting into medical school" and the development of humanistic physicians. It reviews the longstanding challenge inherent in helping pre-meds acquire not only sufficient scientific background but also well-developed interpersonal skills to help them understand patients' experience of illness and their own interactions with other members of the health care team. Clinical experiences from the NEH Institute are interpreted from the perspective of a pre-med advisor who also teaches literature; they are also incorporated into an undergraduate course on literature about illness.     

In Defense of Indexicalism:Comments on Davis

Wayne Davis (2004) argues against the thesis that knowledge claims are indexical, and he presents an alternative account of the contextual variability of our use of S knows p. In this commentary I focus on the following three points. First, I want to supplement Daviss considerations about the inability of indexicalism to deal with skeptical paradoxes by considering what the consequence would be if the indexicalists explanation of these paradoxes were satisfactory. Second, I am going to take a brief look at Daviss alternative theory. Third, in the main part of my commentary I try to show that indexicalism may be true in spite of the linguistic evidence Davis presents against it.     

An experiential approach to the inpatient anger management group

Groups that target aggression on the inpatient milieu usually use psycho-educational and cognitive-behavioral techniques. In contrast, this article presents an experiential approach that targets the moment-by-moment experience within the inpatient group. It makes unique use of group-closing relaxation exercises that facilitate cohesion, consolidate the affectively charged material generated during the session, and reorganize patients' emotional/cognitive understanding of themselves and the world around them. These groups also give patients the opportunity to negotiate the affective residue associated with past aggressive acts, and the opportunity to examine the personal meaning and underlying assumptions behind ongoing group interactions. We describe several techniques or modes of intervention. The authors also use case examples to illustrate the theory, technique, and impact the approach can have on its members.
