Youth with serious mental illness come into contact with juvenile justice more than 3 times as often as other youth, obliging communities to expend substantial resources on adjudicating and incarcerating many who, with proper treatment, could remain in the community for a fraction of the cost. Incarceration is relatively ineffective at remediating behaviors associated with untreated serious mental illness and may worsen some youths' symptoms and long-term prognoses. Systems of care represent a useful model for creating systems change to reduce incarceration of these youth. This paper identifies the systemic factors that contribute to the inappropriate incarceration of youth with serious mental illness, including those who have committed non-violent offenses or were detained due to lack of available treatment. It describes the progress of on-going efforts to address this problem including wraparound and diversion programs and others utilizing elements of systems of care. The utility of systems of care principles for increasing access to community-based mental health care for youth with serious mental illness is illustrated and a number of recommendations for developing collaborations with juvenile justice to further reduce the inappropriate incarceration of these youth are offered. 相似文献

Three studies tested the hypothesis that the relation between perceived racial discrimination and substance use reported in previous research is moderated by use of substances as a coping mechanism. Studies 1 and 2 were experimental studies of African American adolescents' and young adults' reactions to a discrimination experience. Results revealed that those who endorsed substance use-as-coping reported more willingness to use substances after experiencing discrimination. Study 3 was a prospective study of the relation between perceived discrimination and substance use over an 8-year period in African American adolescents. Results demonstrated that discrimination is associated with increases in substance use, but only among adolescents who endorse substance use-as-coping. Together, these three studies provide evidence that experiencing discrimination has both short- and long-term detrimental effects on African Americans' substance use, but significantly more so for those who adopt a pattern of using substances as a coping mechanism. Less supervision by the executive system after disruption of the right prefrontal cortex leads to increased risk taking in gambling because superficially attractive-but risky-choices are not suppressed. Similarly, people might gamble more in multitask situations than in single-task situations because concurrent executive processes usually interfere with each other. In the study reported here, we used a novel monetary decision-making paradigm to investigate whether multitasking could reduce rather than increase risk taking in gambling. We found that performing a task that induced cautious motor responding reduced gambling in a multitask situation (Experiment 1). We then found that a short period of inhibitory training lessened risk taking in gambling at least 2 hr later (Experiments 2 and 3). Our findings indicate that proactive motor control strongly affects monetary risk taking in gambling. Traditional explanations of reading disability (e.g., perceptual, memory, and phonetic receding deficits) have encountered logical difficulties and substantial empirical disconfirmation. An alternative conceptualization of the nature of reading disability is proposed. Disabled readers are hypothesized to suffer a fundamental problem in acquiring word knowledge and word processing skills. Poor decoding skills prevent them from developing sophisticated reading comprehension skills. From this perspective, the cognitive deficits of disabled readers result from failure to acquire and generalize complex processing skills originally mastered through reading experience.
