A virtual reality (VR)-based locomotor training system has been developed for gait rehabilitation post-stroke. The system consists of a self-paced treadmill mounted onto a 6-degrees-of-freedom motion platform. Virtual environments (VEs) that are synchronized with the speed of the treadmill and the motions of the platform are rear-projected onto a screen in front of the walking subject. A feasibility study was conducted to test the capability of two stroke patients and one healthy control to be trained with the system. Three VE scenarios (corridor walking, street crossing, and park stroll) were woven into a gait-training program that provided three levels of complexity (walking speed, slopes, collision avoidances), progression criteria (number of successful trials) and knowledge of results. Results show that, with practice, patients can effectively increase their gait speed as demanded by the task and adapt their gait with respect to the change in physical terrain. However, successful completion of tasks requiring adaptation to increasing demands related to speed and physical terrains does not necessarily predict the patient's ability to anticipate and avoid collision with obstacles during walking. Post-9/11 service members may return from military service with a complicated set of symptoms and conditions, such as posttraumatic stress disorder (PTSD), depression, substance misuse, and traumatic brain injury (TBI), that interfere with reintegration and impair functioning. Although evidence-based treatments that facilitate recovery exist, their successful delivery at a sufficient dose is limited. Barriers to accessing treatment combined with challenges compiling a comprehensive treatment team further delay delivery of effective evidence-based care for PTSD, TBI, and co-occurring mental health conditions. This paper describes the development of a comprehensive, multidisciplinary, 2-week intensive day program for post-9/11 veterans with complex mental health concerns. The treatment program combines skill building groups, family education, and integrative health approaches with evidence-based individual PTSD or TBI care. Initial results from the first 132 participants were notable for a 97% completion rate, as well as statistically significant and clinically meaningful reductions in PTSD, neurobehavioral, and depression symptom severity for the 107 veterans who completed the PTSD track and the 21 who completed the TBI track. These data suggest the intensive program approach is an effective, well-tolerated model of treatment for post-9/11 veterans with PTSD and/or TBI. Future controlled studies should examine the effectiveness of this intensive model compared to standard evidence-based therapy delivery, as well as longitudinal outcomes. 相似文献
Given the potentially harmful effects of parenting stress on parents, children, and their relationship, it is critical to have a reliable and valid measure of parenting stress in clinical and community samples. The Family Strain Index (FSI) is a brief questionnaire designed to measure stress and demand on parents of children with ADHD. The present study is the first to evaluate the psychometric properties of scores on the FSI in a general community sample. Parents (89% mothers) of 550 preschool children (aged 2–5 years; 50% boys) sampled through 17 kindergartens located in Danish cities and villages completed the FSI, the ADHD Rating Scale (RS)‐IV Preschool Version, and a background questionnaire. FSI scores were characterized by restricted range and floor effects. The scale's construct validity was not supported and the measurement repeatability after 1 month was low. The scale did have convergent validity as levels of parenting stress were associated with perceived ADHD behavior in off‐spring, but overall, results did not encourage the use of the FSI as a measure of parenting stress in the general population. Measures that include more normative events may be more appropriate when attempting to capture parenting stress in general community samples. 相似文献
According to the widely endorsed Knowledge Account of Assertion, the epistemic requirements on assertion are captured by the Knowledge Norm of Assertion, which requires speakers only to assert what they know. This paper proposes that in addition to the Knowledge Norm there is also an Epistemic Propositional Certainty Norm of Assertion, which enjoins speakers only to assert p if they believe that p on the basis of evidence which makes p an epistemic propositional certainty. The paper explains how this propositional certainty norm accounts for a range of data related to the practice of assertion and defends the norm against general objections to certainty norms of assertion put forward by Duncan Pritchard, John Turri, and Timothy Williamson, by drawing on linguistic theories about epistemic modals and gradable predicate semantics. Together these considerations show that the prospects of a certainty account of assertion are much more promising than is usually assumed.
Social networking sites (e.g., MySpace and Facebook) are popular online communication forms among adolescents and emerging adults. Yet little is known about young people's activities on these sites and how their networks of “friends” relate to their other online (e.g., instant messaging) and offline networks. In this study, college students responded, in person and online, to questions about their online activities and closest friends in three contexts: social networking sites, instant messaging, and face-to-face. Results showed that participants often used the Internet, especially social networking sites, to connect and reconnect with friends and family members. Hence, there was overlap between participants' online and offline networks. However, the overlap was imperfect; the pattern suggested that emerging adults may use different online contexts to strengthen different aspects of their offline connections. Complex systems ensure resilience through multiple controllers acting at rapid and slower timescales. The need for efficient information flow through complex systems encourages small-world network structures. On the basis of these principles, a group of regions associated with top-down control was examined. Functional magnetic resonance imaging showed that each region had a specific combination of control signals; resting-state functional connectivity grouped the regions into distinct 'fronto-parietal' and 'cingulo-opercular' components. The fronto-parietal component seems to initiate and adjust control; the cingulo-opercular component provides stable 'set-maintenance' over entire task epochs. Graph analysis showed dense local connections within components and weaker 'long-range' connections between components, suggesting a small-world architecture. The control systems of the brain seem to embody the principles of complex systems, encouraging resilient performance. 相似文献
In this study we explored the ways in which men's and women's personal incomes are treated and experienced differently in the family. Two hundred and fourteen participants (102 men and 112 women) in dual-earner couples were recruited in a random sample telephone survey. Participants reported a double standard in which women receive more praise than men for their incomes. Men reported stronger negative and positive feelings about their incomes than women. Regardless of gender, the higher the participants' income category the more gratitude they received from spouses and the more appreciated they felt. Wives' absolute incomes elicited husbands' gratitude, whereas husbands' relative incomes influenced wives' gratitude. Women who outearned their husbands reported no negative consequences. Although money still carries gendered meaning, women's incomes may earn increasing clout in the family. The ways in which praise, criticism, and gratitude for earnings provide a window on how gender is done and undone in the family are discussed. 相似文献
In this paper, we examined the impact of the numerical representation and the intrusiveness of immigrants on feelings of group threat voiced by the majority. The present evaluation of group threat differs from previous studies in its inclusion of temporal comparisons. The relationship between feelings of threat and attitudes toward immigration was also evaluated. In all, 221 college students completed a questionnaire. As predicted, results obtained show that numerical representation was positively associated with feelings of group threat resulting from invidious social comparisons. Intrusiveness was positively related to feelings of group threat resulting from temporal comparisons. Contrary to hypotheses, the final model confirms that only feelings of temporal group threat were associated with negative attitudes toward immigration. Practical implications and the important role of temporal comparisons are discussed. 相似文献
