Controversy remains regarding the reliability of methods used to determine adherence to antiretroviral medication in HIV. In this study the authors compared adherence rates of 119 HIV-positive participants during a 6-month study, as estimated via electronic monitoring (EM) and self-report (SR). Adherence for both short (4-day) and long (4-week, or intervisit) periods was examined, as well as factors that underlie discrepancies between EM and SR. Results showed that intervisit EM estimates were consistently lower than those of SR. SR estimates based on shorter periods (4 days) were closer to those of EM. Higher discrepancies between EM and SR estimates were associated with lower cognitive functioning and externalized locus of control. These findings lend support for using both EM and SR methods; however, study design (e.g., length) and other factors (e.g., cognitive status, cost) should be considered. 相似文献

In recent years, much attention has been given to the epistemic credentials of belief based on moral testimony. Some people think pure moral deference is wrong, others disagree. It comes as a surprise, however, that while the epistemic responsibilities of the receiver of moral testimony have been closely scrutinized, little to no discussion has focused on the epistemic duties of the speaker. While there is extensive research on feedback, little research is aimed at the use of feedback to optimise conflicting goals. A task modelled after In Home Displays for providing feedback about electricity cost was designed to investigate the effects of feedback frequency, detail, and stability, when participants try to balance cost and utility. Frequent feedback proved to be advantageous in a deterministic system, but feedback aggregated over time was advantageous in a system with noisy feedback. Surprisingly, performance was better with noisy feedback, where the probabilism, in effect, acted as a filter, highlighting the applications that are most important for the cost and the utility. Computational modelling suggested that the best-fitting model assumes that the participants are sequential, considering one goal at a time, first satisfying the cost budget, only thereafter trying to maximise the utility, and reflexive, myopically responding primarily to the feedback explicitly available on a given trial. 相似文献

In the context of HIV, women’s sexual rights and sexual autonomy are important but frequently overlooked and violated. Guided by community voices, feminist theories, and qualitative empirical research, we reviewed two decades of global quantitative research on sexuality among women living with HIV. In the 32 studies we found, conducted in 25 countries and composed mostly of cis-gender heterosexual women, sexuality was narrowly constructed as sexual behaviours involving risk (namely, penetration) and physiological dysfunctions relating to HIV illness, with far less attention given to the fullness of sexual lives in context, including more positive and rewarding experiences such as satisfaction and pleasure. Findings suggest that women experience declines in sexual activity, function, satisfaction, and pleasure following HIV diagnosis, at least for some period. The extent of such declines, however, is varied, with numerous contextual forces shaping women’s sexual well-being. Clinical markers of HIV (e.g., viral load, CD4 cell count) poorly predicted sexual outcomes, interrupting widely held assumptions about sexuality for women with HIV. Instead, the effects of HIV-related stigma intersecting with inequities related to trauma, violence, intimate relations, substance use, poverty, aging, and other social and cultural conditions primarily influenced the ways in which women experienced and enacted their sexuality. However, studies framed through a medical lens tended to pathologize outcomes as individual “problems,” whereas others driven by a public health agenda remained primarily preoccupied with protecting the public from HIV. In light of these findings, we present a new feminist approach for research, policy, and practice toward understanding and enhancing women’s sexual lives—one that affirms sexual diversity; engages deeply with society, politics, and history; and is grounded in women’s sexual rights. 相似文献

Population ageing in most western countries involves an increase in public expenditures and the risk of labour shortage. One
way to meet these challenges is to retain older workers in the labour market by improving their work life. This article assesses
whether quality of work life measures differ in importance for male and female workers in their retirement planning. This
study applies samples of workers and retirees born in 1940 and 1945 drawn from Danish panel surveys in 1997 and 2002 and merged
with longitudinal register data. Results suggest that male and female workers’ retirement plans are affected differently by
various aspects of the job. Indeed, job demands lower planned retirement age, while increases in earnings, work hour satisfaction,
and the opportunity to use skills on the job increase this age for men and women. Nevertheless, the impact of earnings is
largest for men, and only male workers attach importance to job control and job security. These gender differences suggest,
first, that men are more influenced than women by the quality of job dimensions in their retirement planning and, second,
that an employer-initiated effort directed towards retaining older workers at the workplace will not necessarily be as effective
for female as for male workers.
