This study examined differences between healthy children (n = 35) and those with spina bifida myelomeningocele (SBM; n = 42) on the Behavior Rating Inventory of Executive Function (BRIEF), a measure of executive function behaviors. It also examined whether aspects of biological risk associated with SBM and reserve factors within the family could account for variability in BRIEF scores for children and adolescents with SBM. Patients in the SBM group exhibited more problems than both published norms and a local comparison group of healthy children in metacognition but not behavior regulation. Behavior regulation problems in children with SBM were predicted by parent psychological distress. More shunt-related surgeries and history of seizures predicted poorer metacognitive abilities.  相似文献

T. E. Joiner's (2004, in press) theory of suicidal behavior suggests that past suicidal behavior plays an important role in future suicidality. However, the mechanism by which this risk is transferred and the causal implications have not been well studied. The current study provides evaluation of the nature and limits of this relationship across 4 populations, with varying degrees of suicidal behavior. Across settings, age groups, and impairment levels, the association between past suicidal behavior and current suicidal symptoms held, even when controlling for strong covariates like hopelessness and symptoms of various Axis I and II syndromes. Results provide additional support for the importance of past suicidality as a substantive risk factor for later suicidal behavior.  相似文献

The prevalence and correlates of depressive symptoms following childhood traumatic brain injuries (TBI) were examined using data drawn from a prospective longitudinal study. Participants included 38 children with severe TBI, 51 with moderate TBI, and 55 with orthopedic injuries (OI). Assessments occurred shortly after injury (baseline) and at 6- and 12-month follow-ups. Children completed the Child Depression Inventory (CDI). Parents rated depressive symptoms using the Child Behavior Checklist (CBC), with baseline ratings reflecting premorbid status. Assessments also included measures of children's neurocognitive functioning and the family environment. The three groups did not differ overall in self-reported symptoms on the CDI, but did display different trends over time. The three groups did not differ on parent ratings of premorbid depressive symptoms on the CBC, but parents reported more depressive symptoms in the TBI groups than in the OI group at 6- and 12-month follow-ups. Child and parent reports were correlated for children in the TBI groups, but not for those in the OI group. Depressive symptoms were related to socioeconomic status in all groups. Socioeconomic status also was a significant moderator of group differences, such that the effects of TBI were exacerbated in children from more disadvantaged homes. Although self-reports of depressive symptoms were related inconsistently to children's verbal memory, parent reports of depressive symptoms were unrelated to IQ or verbal memory. The findings suggest that TBI increases the risk of depressive symptoms, especially among more socially disadvantaged children, and that depressive symptoms are not strongly related to post-injury neurocognitive deficits.  相似文献

Implications of both computer science and the computer are critical for a deeper penetration into the history of science. Building directly on “Formalized Historiography,” which appeared in this Journal last year [14 (1978): 247–263], the present essay contrasts the analysis of history of science discourse by a human reader with that at present or in prospect possible with computer-based ‘intelligent systems.’ Further, related issues are posed as to how scientific the history of science may itself become, and as to the types of transformed understanding that may emerge both of the processes of science and of the history of science. For example, would a connotation-free understanding of the structure of scientific discourse through successive state transitions (paradigm shifts) disclose science itself to be an abstract machine?  相似文献

Objective: This study examined the effects of age at injury on the persistence of behavior problems and social skill deficits in young children with complicated mild to severe traumatic brain injury (TBI). Method: A concurrent cohort/prospective research design was used with repeated assessments of children with TBI (n = 82) or Orthopedic Injury (OI) (n = 114). Parents completed the Child Behavior Checklist, the Behavior Rating Inventory of Executive Functions, and the Preschool and Kindergarten Behavior Scales or the Home and Community Social and Behavior Scales shortly after injury to assess preinjury functioning, and at an extended follow-up an average of 38 months postinjury. Generalized linear modeling was used to examine the relationship of age at injury to the maintenance of behavior problems, and logistic regression was used to examine the persistence of clinically significant behavior problems. Results: At the extended follow-up, severe TBI was associated with significantly greater anxiety problems relative to the Group OI. With increasing time since injury, children who sustained a severe TBI at an earlier age had significantly higher levels of parent-reported symptoms of ADHD and anxiety than children who were older at injury. Conclusions: Findings suggest that longer-term treatment for behavior problems may be needed after severe TBI, particularly for those injured at an earlier age. Long-term deficits in executive functions following childhood traumatic brain injuries (TBI) were examined using the Behavior Rating Inventory of Executive Function (BRIEF). Parents completed the BRIEF approximately 5 years postinjury as part of a prospective study of children injured between the ages of 6 and 12. The children were between 10 and 19 years of age at the time of the assessment, and included 33 with severe TBI, 31 with moderate TBI, and 34 with orthopedic injuries. Parents also rated children's adaptive functioning and completed several other measures of parent and family functioning. Children were administered a neuropsychological test battery that included several measures of executive functions. The groups displayed a significant linear trend in BRIEF scores, with the largest deficits in executive functions reported in children with severe TBI. BRIEF scores were related consistently across groups to a test of working memory, but not to other neuropsychological measures. BRIEF scores also predicted children's adaptive functioning and behavioral adjustment, as well as parent psychological distress, perceived family burden, and general family functioning. The findings indicate that TBI results in long-term deficits in executive functions that are related to children's psychosocial outcomes, as well as to parent and family functioning.  相似文献

The practice of pediatric neuropsychologists was examined by reviewing all referrals made in 1 year to neuropsychology programs located in three children's hospitals. The review identified 472 cases, 75% of whom were between the ages of 7 and 11. The sample included 86 inpatients and 386 outpatients. The most common diagnoses, accounting for 87% of the cases, were learning disability, traumatic brain injury, attention deficit-hyperactivity disorder, seizure disorder, primary psychiatric disorder, phenylketonuria, idiopathic mental retardation, brain tumor, leukemia, stroke, and encephalitis. The most common referral sources, accounting for 92% of the cases, were neurologists, pediatricians, parents, physiatrists, neurosurgeons, oncologists, specialists in metabolic disease, psychologists, rheumatologists, and psychiatrists. The distribution of diagnoses and referral sources differed significantly for inpatients and outpatients and across the three hospitals. The results may distinguish child neuropsychology from adult neuropsychology and highlight the diversity that characterizes its practitioners.  相似文献

The current study examined processing speed in children two years post-treatment for brain tumors (BT) with radiation therapy (RT) compared to those treated with without RT. Participants included 59 children (4–17 years) with BT assessed as part of the Brain Radiation Investigative Study Consortium (BRISC). Processing speed was assessed at two time points: Time1 (3–9 months post-surgery) for 26 children who received whole brain or focal RT (RT group) and 33 treated without RT (no-RT group), and again two years later (Time2) for 42 participants (17 RT, 25 no-RT). Linear mixed effects (LME) regression analyses examined differences in cognitive and motor speed between groups and across visits, with age at Time1 (age1) treated as a moderating variable, and sex and primary tumor size as covariates. No effects for treatment group or visit were found for motor speed (Pegboard) or mean reaction time (Attention Network Task). On the Wechsler Processing Speed Index (PSI), the no-RT group performed better than the RT group, with a group-by-age interaction such that across visits, the difference between the no-RT and RT groups was larger among children who were older at initial treatment (≥10 years) than among those who were younger (<10 years). Cumulative brain injury earlier in life (tumor, surgery, plus RT) may result in greater impact on more complex tasks of cognitive efficiency. Children receiving RT showed reduced processing speed over time, with a larger group difference among those who were over 10 years at treatment.  相似文献

Reading achievement depends in part on children's ability to employ strategies to monitor their comprehension, that is on children's use of metacognitive skills. An experimental intervention program created to foster such skills was evaluated. The story grammar training was designed to increase both strategic reading behavior and explicit, observable comprehension monitoring. Fourth-grade (N = 20) and fifth-grade (N = 16) children were randomly assigned to experimental and control groups. Maintenance was examined using free and probed oral recall, notetaking, and summarization of brief narrative and expository texts. Generalization was assessed through free and probed written recall of a lengthy text, which required both strategic reading and summarization skills for adequate comprehension. Skill acquisition was clearly demonstrated; trained readers were able to recall and summarize passages better than untrained readers. The evidence for generalization was more equivocal: although trained readers' free recall was significantly better than that of untrained readers, their probed recall was not. Provision of metacognitive skills that promote active comprehension monitoring, then, appears to provide children with the tools needed for independent reading. Programs designed to provide such skills may aid efforts to prevent reading failure.  相似文献
