The present study focused on the link between parental bonding and personality pathology. We developed a clinician-report version of the Parental Bonding Instrument (PBI; Parker, Tupling, and Brown, 1979), which 203 clinicians applied to a randomly selected patient in their care. The goal of the study was to examine the reliability and validity of a clinician-report PBI, and to provide a preliminary examination of the relation between personality disorder (PD) symptoms and parental bonding in a national sample of patients in treatment in the community. Factor analysis yielded two factors (parental rejection/care and parental overprotection) for each parent, which had high internal consistency (coefficient alphas all >.80). The factor structure of the clinician-report measure strongly resembled the factor structure of the widely used self-report instrument. Parental bonding as assessed by clinician report showed coherent patterns of association with PD diagnoses, which resembled those found in other samples. Maternal rejection/care showed a particular link to borderline PD (BPD), and remained a significant predictor of BPD along with sexual abuse and physical abuse in multiple regression analysis. These data, along with others from recent studies using clinicians as informants, suggest that clinicians can provide reliable and valid data when their observations are quantified using psychometrically sound instruments. Clinician-report methods may provide a useful complement to studies of personality disorders that rely primarily on self-reports or structured interviews.  相似文献

Event-related potentials (ERPs) were obtained from 15 electrode sites in six average and six impaired reading children, 12 years of age, during visual letter discrimination tasks. Subjects responded to target letters with an enclosed area in the form task and to letters that rhymed with "e" in the rhyme task. Response accuracy was similar between the groups. Reaction time was relatively longer for the impaired group during the rhyme task. At lateral sites, condition differences were evident as greater negative shifts in the rhyme task than the form at 170 and 470 ms as well as a delayed late positivity for the rhyme. In terms of reading ability, the average readers' ERPs were more negative than those of the impaired group at 270 and 450 ms. Inter-hemispheric variations were also seen between the groups, with the average readers more negative than the impaired readers at right hemisphere sites. Contrary to expectations, group differences in the ERP did not vary substantially as a function of condition, and task demands were evaluated in view of these findings.  相似文献

In the state of Victoria, Australia, a government funded genetic counseling service exists to meet the needs of families. An audit showed that many families do not use this service after the birth of a child with a genetic problem. To investigate this we surveyed families of children born with Down syndrome over 2 years in Victoria. Questionnaires were completed by 74 mothers, of whom only 18 had received genetic counseling between the birth and the time of the study (mean 3.5 years). Of those not receiving genetic counseling, 71% said they were not offered or had not heard of it. Mothers who had genetic counseling were younger than those who had not, and were more likely to have attended University. Those who had genetic counseling indicated less satisfaction with care at the diagnosis and were more likely to perceive their child as unwell at birth than those who were aware of genetic counseling but did not have it. Of those who did not have genetic counseling, over half were unclear about what it is, although 74% agreed with the statement genetic counseling is most useful when planning to have another child. Of those who had heard of genetic counseling, 73% said they were not sure how it could help. Many families with children with Down syndrome are not aware of the existence or functions of genetic counseling. With greater awareness, some may still choose not to have genetic counseling, but others enunciated needs that could be met by this service if it were offered to them.  相似文献

The responsibility of informing relatives that predictive genetic testing is available often falls to the proband. Support is required during this process, however the perceived utility of genetic counseling and other strategies to facilitate communication have not been explored. We investigated the experiences of 12 individuals with hereditary nonpolyposis colorectal cancer (HNPCC) in a semistructured telephone interview. Respondents informed their immediate family about the availability of genetic testing, however many more-distant relatives were not directly informed. Respondents were mostly satisfied with the way they told family members about testing and had mixed views about the usefulness of genetic counseling. Gender differences were observed, with most men expressing a need for guidance or support in communicating to relatives. Letters and booklets were thought to enhance the quality of information but the provision of further aids is unlikely to increase the number of relatives made aware of predictive testing by the proband.  相似文献

An integration of psychoanalytic theory with contemporary developments in cognitive neuroscience offers a useful perspective on long-standing controversies about the nature of transference, and a better understanding of the precise mechanisms by which transferential processes occur. Contemporary psychoanalytic views of transference are reviewed, and the many processes that constitute transference are described. Two issues that have emerged in different guises for several decades-the role of the analyst in eliciting transference, and the nature of "real" and "transferential" components of the therapeutic relationship-are reconsidered in the light of concepts such as connectionist networks. Although a useful analytic stance is one that allows the patient's enduring dynamics to dominate the analytic field, it is suggested, anonymity is neither a cognitive possibility nor the driving force behind most transference reactions, and the distinction between "real" and "transferential" perceptions is one of therapeutic interest, not of mechanism. Certain features of the analytic situation make some dynamics more likely than others to enter the treatment relationship, notably those related to authority, intimacy and attachment, and sexuality. Transference reactions are best understood as constructed from a combination of the patient's enduring dispositions to react in particular ways under particular conditions; features of the analytic situation and of the analyst; and interactions between patient and analyst. These reactions do not unfold ineluctably from the patient's mind in the consulting room, nor are they cognitive constructions of the patient-analyst dyad or co-constructions of relatively equal partners exerting their influence on the analytic field.  相似文献

Several studies suggest that a prototype matching approach yields diagnoses of comparable validity to the more complex diagnostic algorithms outlined in DSM-IV. Furthermore, clinicians prefer prototype diagnosis of personality disorders (PDs) to the current categorical diagnostic system or alternative dimensional methods. An important extension of this work is to investigate the degree to which clinicians are able to make prototype diagnoses reliably. The aim of this study is to assess the inter-rater reliability of a prototype matching approach to personality diagnosis in clinical practice. Using prototypes derived empirically in prior research, outpatient clinicians diagnosed patients' personality after an initial evaluation period. External evaluators independently diagnosed the same patients after watching videotapes of the same clinical hours. Inter-rater reliability for prototype diagnosis was high, with a median r = .72. Cross-correlations between disorders were low, with a median r = .01. Clinicians and clinically trained independent observers can assess complex personality constellations with high reliability using a simple prototype matching procedure, even with prototypes that are relatively unfamiliar to them. In light of its demonstrated reliability, efficiency, and versatility, prototype diagnosis appears to be a viable system for DSM-V and ICD-11 with exceptional utility for research and clinical practice.  相似文献

A large body of research has considered children’s novel word learning from video; however, no known work has compared children’s novel word learning from an educational television show in which the character speaks directly to the child (participatory cues) and a show in which the child overhears two characters speaking to one another (third-party joint attention). Using two professionally produced videos that mimic contemporary children’s television shows, we conducted a 2-condition experiment (= 73) to examine under which condition children were better able to learn a novel word. Results indicated a main effect of condition and age: children in the participatory cues condition and older children demonstrated greater word learning than children in the third-party joint attention condition and younger children. There was an interaction between age and condition, but only among children between the ages of 28.20 and 41.46 months. These findings demonstrate that the participatory cues model, typically used in children’s television programming, is a better model to use to promote children’s word learning. These findings run counter to a similar study that used stimulus videos that did not mimic a children’s program, finding that the third-party joint attention condition was more effective.  相似文献

The use of augmentative alternative communication (AAC) in psychotherapy is particularly important for psychotherapists who may have a disability related to speech-language production. The aim of our study was to examine the perceptions of using AAC in psychotherapy. We recruited 186 participants and randomly assigned them to either watch a video of a therapist using AAC or employing traditional talk psychotherapy. Subsequently, we asked participants to complete the Counsellor Rating Form-Short (CRF-S), respond to a question about behavioural intention to seek psychotherapy and complete the Working Alliance Inventory-Observer form (WAI-O). Most participants perceived the use of AAC to be related to the therapist having a disability. The use of AAC had higher ratings on the CRF-S and higher scores on the WAI-O. There were no significant differences in behavioural intention between the therapist using AAC or traditional talk psychotherapy. The use of AAC is a viable option for psychotherapy and is beneficial. Psychotherapists with a disability affecting speech-language production may opt to use AAC as others perceive the therapist positively, the alliance is rated higher and therapists do not need to be concerned with patients not wanting to seek out their services.  相似文献
