Rethinking therapeutic action   总被引:1,自引:0,他引:1  
Like other core psychoanalytic constructs, the theory of therapeutic action is currently in flux, as theorists of differing persuasions propose different mechanisms. In this article, the authors attempt to integrate developments within and without psychoanalysis to provide a working model of the multifaceted processes involved in producing change in psychoanalysis and psychoanalytic psychotherapy. A theory of therapeutic action must describe both what changes (the aims of treatment) and what strategies are likely to be useful in facilitating those changes (technique). The authors believe that single-mechanism theories of therapeutic action, no matter how complex, are unlikely to prove useful at this point because of the variety of targets of change and the variety of methods useful in effecting change in those targets (such as techniques aimed at altering different kinds of conscious and unconscious processes). Interventions that facilitate change may be classified into one of three categories: those that foster insight, those that make use of various mutative aspects of the treatment relationship and a variety of secondary strategies that can be of tremendous importance. They propose that, in all forms of psychoanalytic treatment, we would be more accurate to speak of the therapeutic actions , rather than action .  相似文献

This study developed an HIV risk reduction intervention for people living with HIV (PLWH) obtaining care at primary healthcare clinics in KwaZulu-Natal, South Africa by (1) conducting elicitation research to understand the dynamics of risk behaviour among PLWH, (2) modifying an existing HIV risk reduction intervention based on research findings and (3) conducting a pilot study to evaluate feasibility, acceptability and fidelity of the modified intervention implemented by trained lay counsellors at a rural clinic in KwaZulu-Natal. A total of 61 healthcare providers and 77 HIV+ patients from four primary healthcare clinics participated in 14 focus groups and 20 individual interviews to identify informational, motivational and behavioural skills (IMB) factors contributing to PLWH's sexual risk behaviour. Elicitation research findings were incorporated into a revised version of Options for Health, an evidence-based risk reduction intervention for PLWH in clinical care. In a 5-day training, lay counsellors learned strategies to address IMB barriers to safer sex identified in elicitation research. The revised intervention, which was implemented by six counsellors with 39 patients, was feasible to implement, acceptable to patients and counsellors, and implemented with good fidelity. This study makes an important contribution towards development of a theory-based HIV risk reduction intervention for PLWH linking prevention with treatment in South Africa.  相似文献

Little is known about the mechanisms underlying how interviewers establish and monitor baselines of respondent behaviour. This study addresses this knowledge gap by characterising the practice of establishing baselines and measuring the accuracy of interviewer judgements based on assessments of baseline and departure behaviours. Eighteen professional interviewers viewed two videos of naturalistic interviews, reported the cues they perceived as informative for establishing and assessing a baseline, and diagnosed respondents' internal states. Participants reported multiple cue types as informative, but predominantly relied on non‐verbal cues. Overall, participants were sensitive to 27% of respondents' identified internal states but showed improvement over time. They achieved a hit rate of 20% during early interview stages and a hit rate of 33% during late stages. Although non‐verbal cues dominated reports, attending to verbal cues afforded participants greater efficiency in identifying respondent internal states. Implications and recommendations for practice are discussed.  相似文献

Research has considered how exposure to prosocial television narratives influences children’s social inclusion behaviors (e.g., Mares & Acosta, 2010). In these experiments, children typically view a stimulus episode alone; however, we know that children often watch with others at home (Chandler, 1997). Thus, in this study we examined how children’s proximal social context during viewing influenced effects. Using data collected from a 3-condition experiment (control, view-alone, coview with close friend) with Dutch children ages 5 and 6 (N = 80), we found that exposure largely did not influence children’s inclusion judgments or stigmatization beliefs. There was, however, an age × condition interaction, such that 6-year-old children in the coview condition demonstrated greater stigmatization beliefs toward other children, compared to 5-year-olds in the same condition, or all children in the other conditions. We discuss the implications of these findings while considering previous work on learning inclusion from prosocial television shows, reality judgments, and the bystander effect.  相似文献

This study assessed a number of potential predictors of host-country and ethnic-group identification of members of immigrant groups to Australia. The study included 281 adult participants from Vietnam, Hong Kong, and New Zealand, who responded to questions relating to their acculturation attitudes, host-culture and ethnic-group identification, and experiences in their new country. Results showed that the predictors of host country (Australian) identification were the immigrants' acculturation attitude of wanting to live according to host standards and values, lower ethnic versus Australian social involvement, lower ethnic prejudice, higher job status, and acceptance by Australians. The major predictors of ethnic-group identification were an acculturation attitude of wanting to live according to ethnic-group standards, higher ethnic versus Australian social involvement, lower self-esteem, fewer Australian friends, lower ability to speak English, higher education, lower self-efficacy, and number of ethnic friends. The implications for the acculturation of immigrants are discussed.  相似文献

The strengths and weaknesses of personality psychology are examined. Major strengths of the field include its breadth and integrative quality, and sophistication in methodology and analytic strategies. However, intellectual isolation stands as a potential threat to the vitality of personality, and a heavy reliance on single-time self-report measures places limitations on findings. The authors give an overview of the major topics published in JPSP in recent years, and highlight the international character of personality research. Our recommendations include continuing the tradition of employing rigorous methods, using multiple methods and multiple studies to replicate findings, and bridging the gap between childhood temperament and adult personality.  相似文献

A four-list version of a release from proactive interference paradigm was used to assess the degree to which older and younger adults tested at optimal and nonoptimal times of day are vulnerable to interference effects in memory, effects that may increase at nonoptimal times. Morning type older adults and Evening type younger adults were tested either early in the morning or late in the afternoon. Standard buildup and release effects were shown for all age groups except for older adults tested in the afternoon; they failed to show release. Recall and intrusion data suggested that older adults are more vulnerable to proactive interference than younger adults and that for older adults at least, interference effects are heightened at nonoptimal times of day. The data are discussed in terms of an inhibitory model of control over the contents of working memory (Hasher, Zacks, & May, 1999).  相似文献

One hundred forty-nine inpatients within a maximum security psychiatric facility were assessed with the Psychopathy Checklist: Screening Version (PCL:SV; S. D. Hart, D. N. Cox, & R. D. Hare, 1995). Within the total sample, 68% had a psychotic disorder and 30% met criteria for psychopathy. Using confirmatory factor analysis, the authors tested the 2-factor PCL:SV model of psychopathy and recent 3- and 4-factor models. Results indicated good fit for each model, with the 4-factor model showing best overall fit. Structural equation modeling was used to determine which psychopathy factors predicted 6-month follow-up of inpatient aggression. The 2-, 3-, and 4-factor models, respectively, accounted for 16%.27%. and 3l% of the variance in aggression.  相似文献

A functional analysis isolated peer attention as the primary maintaining variable for disruptive behavior displayed by a student with attention deficit hyperactivity disorder. Using a brief reversal design, noncontingent reinforcement was then shown to reduce disruptive behavior relative to the peer attention condition. Implications for assessing behavior disorders in mainstream school settings are discussed.  相似文献
