The conceptual structure of aphasic and schizophrenic patients in a nonverbal sorting task

Hierarchical and overlapping cluster methods were applied to the sortings of aphasic, nonaphasic brain-damaged, schizophrenic, and normal subjects presented with 30 pictures of animals. The hierarchical structure solutions were most diffuse for the groups of the schizophrenics and the fluent aphasics. The structure for the nonfluent aphasics showed more clarity, but was also deviant from the structures of the normals and the brain-damaged without aphasia. Fluent aphasics but not nonfluent aphasics tended to sort pictures which they could not name into smaller groups. For the nonfluent aphasics, there was a significant correlation between the commonality of the sortings and the severity of aphasic disturbances as measured by the Token Test. The relationship between conceptual disorganization and language impairment seems to be functionally different for fluent and nonfluent aphasics.This research was supported by a research grant from the Deutsche Forschungsgemeinschaft.     

Commentary on Becoming a Daughter: Trauma is a Powerful Teacher

Personal life crises profoundly impact genetic counselor practice. In this commentary, themes from Matloff’s (in press) article, Becoming a Daughter are highlighted and expanded upon. These themes include: personal impact of a life crisis, and professional impact vis a vis empathy countertransference, self-disclosure, nondirectiveness, and self-confidence. Strategies that help genetic counselors manage personal life crises within their clinical practice and also promote their professional development are emphasized, including normalization of life crises, self-reflection, boundary-setting, and use of peer supervision and consultation.     

Discrepancies between premorbid and current IQ as a function of progressive mental deterioration

The declining cognitive functioning typically found in patients with Alzheimer's disease presents an opportunity to study that decline. The changing magnitude of ever widening discrepancies between premorbid estimators of IQ and observed IQ increases as severity of the disease increases. Premorbid IQs estimated by these scores (the National Adult Reading Test-Revised, the reading tests of the Revised and Third Editions of the Wide Range Achievement Test, and a demographically based regression index for the Wechsler Adult Intelligence Scale-Revised) had relatively similar discrepancies from obtained WAIS-R Full Scale IQs in samples of normal elderly (n = 30), and elderly patients diagnosed with mild (n = 30) and moderate Alzheimer's disease (n = 30) dementia. The discrepancies became larger, regardless of premorbid estimator, as disease severity progressed from none to mild to moderate across the samples.     

A methodology for assessing the functions of emerging speech in children with developmental disabilities

An approach based on Skinner's (1957) theory of verbal behavior has been developed to understand and teach elementary communication skills to children with autism and developmental disabilities (Sundberg & Partington, 1998). However, few studies have directly examined the characteristics of emerging language in children with developmental disabilities. The purpose of this study was to develop and evaluate an assessment for identifying the elementary functions of vocal speech in children. Participants were 4 children with developmental disabilities, aged 6 years to 12 years, who exhibited at least one distinguishable vocal response (word or phrase) frequently in the natural environment. The assessment focused on three verbal operants delineated by Skinner (mand, tact, and intraverbal). One or more functions were identified for at least one vocal response of each child. Results suggested that this assessment would be useful for (a) evaluating Skinner's theory, (b) guiding decisions about language training for individual children, and (c) studying the nature of expressive language development in children with developmental disabilities.     

Disillusionment and Devaluation in Winnicott's Analysis of Masud Khan

Biographical references to Winnicott's analysis of Masud Khan provide the background for this exploration of Winnicott's and Khan's conflicts about disillusionment. On the basis of historical data, speculative hypotheses are presented about possible sources of Winnicott's difficulty engaging Khan's hate and destructiveness. Winnicott's feelings about Khan may have been influenced not only by his denied disillusionment with his own father and his analysts, James Strachey and Joan Riviere, but also by the impact of Strachey's covert anti-Semitism.     

Pyramidal training for families of children with problem behavior

The pyramidal training model was extended to multiple family members of children with behavior disorders. Three primary caregivers were taught to implement individualized treatments for problem behavior. They were then taught how to use various instructional strategies (e.g., prompting, feedback) to teach 2 other family members to implement the treatment. Results showed that pyramidal training was effective in increasing caregiver implementation of treatments across three families.
