The self-reporting of cancer history is becoming increasingly important, as it frequently guides medical decision-making.
We studied the accuracy of personal cancer history using a self-administered questionnaire, comparing the results with the
Tumor Registry at our institution. Among 39,662 records, we identified 3614 women with a single cancer in the Tumor Registry
who reported none or one cancer on their questionnaire. The sensitivity in self-reporting cancers was 85.7%, ranging from
92.1% for breast cancer to 42.9% for leukemia. The accuracy for breast cancer and Hodgkin's Lymphoma was significantly better
than other cancers (p=0.00027, CI: 1.4–3.88). Analysis of patient's characteristics showed that Caucasians reported breast cancer more accurately
than Asian/Pacific Islanders (p=0.008), and those with Jewish ancestry more accurately than non-Jewish (p=0.0435). These results will help us to improve data collection and thus improve medical decision-making. 相似文献

The demands of the modern office are thought to contribute to the development of musculoskeletal disorders. For upper body and lower back disorders, these effects are hypothesized to be mediated by psychological strain. A study of 936 employees from 22 call centers supports this hypothesis. Using logistic regression and structural equation modeling, the authors found that the relationship of workload to upper body and lower back musculoskeletal disorders was largely accounted for by job-related strain. This mediating effect was less evident for arm disorders. Contrary to expectation, job autonomy had neither a direct nor a moderating effect on any musculoskeletal disorder. 相似文献

The authors investigated the role of race, racial identity attitudes and working alliance in counseling supervision using data obtained from supervisors in supervisory dyads. Results revealed the strongest working alliance for supervisor‐supervisee pairs with high racial identity development and the weakest working alliance for pairs with low racial identity development. An anatomical and behavioral review of 29 reported cases of corpus callosum agenesis is presented. This review addresses itself to two questions: (1) Do acallosals manifest split-brain symptoms? (2) Is the corpus callosum necessary for the establishment of lateralization? Unlike the commissurotomy cases, acallosals show few symptoms of hemispheric disconnection. Behavioral strategies, use of noncallosal commissures, elaboration of ipsilateral pathways, and bilateral representation of function are discussed and evaluated as possible mechanisms of compensation. An impairment in spatiomotor functioning often accompanies callosal agenesis. Results of acallosal lateralization testing are examined and problems in interpretation are discussed. Current evidence does not conclusively establish that the corpus callosum is needed for lateralized development of some functions. However, the corpus callosum may play a role in the satisfactory performance of “lateralized” (i.e., spatial, linguistic) functions. 相似文献

We examined the nature of referrals to a health center-based pediatric psychology service from 1990 to 1993 and assessed the satisfaction of health professionals with these services. Archival evaluation of 1467 records showed that over half of the consultation requests came from general pediatrics, pediatric neurology, and surgical services and that 70% of the psychological services were delivered on an outpatient basis. The most frequent referrals were for cognitive/neuropsychological evaluation and externalizing behavior problems. Pediatric psychology trainees were involved in 94% of the consultations. Survey of health professionals (n = 143) indicated very high overall satisfaction with the quality of services delivered. Presenting problems yielding the greatest likelihood for future consultation requests were behavior problems, child abuse, coping with illness, and depression/suicide. Results are discussed in the context of previous evaluations of pediatric psychology services and recommendations for future evaluation research. 相似文献

Factor analyses were used to develop new scales for the Multiple Affect Adjective Check List (MAACL) measuring anxiety, depression, hostility, positive, and sensation-seeking affects. Anxiety, depression, and hostility scales were moderately intercorrelated and a combined score called “dysphoria” was formed from their addition. Positive and sensation-seeking affects were also correlated and combined in a summary score (PASS). The dysphoric and PASS scores were not highly correlated. The number of items checked correlated highly with the positive affect scale, moderately with sensation seeking, and relatively low with the negative affect scales. Internal reliabilities were high and retest reliabilities were low, in conformance with the model for state scales. The anxiety score increased and the positive affect score and number of items checked decreased just prior to a classroom examination. Further plans for scale development are discussed. 相似文献

The generation effect refers to the memory advantage of words that have been generated rather than read. Such a read-generate comparison confounds qualitative task differences and raises methodological problems. A revised methodology is proposed circumventing these problems in that the encoding task is held constant and all stimuli have to be generated, but the degree of generativeness (i.e. the amount of cueing) is varied. In Experiment 1, 1, the (refined version of the) generation effect is demonstrated in a within-subjects design; with increasing generation activity left to the subject, free recall performance increases. No effect is obtained for degree of target masking. The same finding is replicated and shown to be independent of self-paced study time when generative activity is manipulated between subjects (Experiment 2) or within subjects (Experiment 3). As all learning trials involve generation, encoding time is controlled statistically, and free recall is used as a measure of memory, this refined generation effect cannot be explained as an artifact of selective attention or elaboration. Rather, generative activity seems to increase the mobilization of cognitive resources. This motivational account is supported by Experiment 4 showing an enhanced generation effect for positive mood. 相似文献
