Genetic literacy is essential for the effective integration of genomic information into healthcare; yet few recent studies have been conducted to assess the current state of this knowledge base. Participants in the Coriell Personalized Medicine Collaborative (CPMC), a prospective study assessing the impact of personalized genetic risk reports for complex diseases and drug response on behavior and health outcomes, completed genetic knowledge questionnaires and other surveys through an online portal. To assess the association between genetic knowledge and genetic education background, multivariate linear regression was performed. 4 062 participants completed a genetic knowledge and genetic education background questionnaire. Most were older (mean age: 50), Caucasian (90 %), female (59 %), highly educated (69 % bachelor’s or higher), with annual household income over $100 000 (49 %). Mean percent correct was 76 %. Controlling for demographics revealed that health care providers, participants previously exposed to genetics, and participants with ‘better than most’ self-rated knowledge were significantly more likely to have a higher knowledge score (p?<?0.001). Overall, genetic knowledge was high with previous genetic education experience predictive of higher genetic knowledge score. Education is likely to improve genetic literacy, an important component to expanded use of genomics in personalized medicine. 相似文献

A standardized method of assessing daytime drowsiness in frail nursing home residents has not been previously available. We present here the development and test characteristics of an instrument to measure daytime drowsiness in nursing home residents with cognitive and functional impairment, the Multiple Sleep Latency Test-Nursing Home (MSLT-NH). In a standardized manner, the resident is tested three times in one day (at 9 a.m., 11 a.m., and 1 p.m.) to measure the amount of time until the subject falls asleep. The average sleep latency (minutes to fall asleep) is the measure of interest. We tested the MSLT-NH in 95 residents from six community nursing homes in the Los Angeles area (82% of subjects were female; 81% were white, mean age was 86.6 years and mean score on the Mini-Mental State Examination [MMSE] was 9.4). Interrater reliability of the MSLT-NH was quite good (correlations between raters ranged from 0.98 to 0.99; all p values were <.0001). Validity was assessed by comparing MSLT-NH results to simultaneous wrist activity estimation of sleep, and by comparing MSLT-NH findings to results from nighttime sleep estimation by wrist actigraphy the night prior to MSLT-NH. The percent agreement between MSLT-NH and wrist actigraph ranged from 77% to 79% MSLT-NH results were significantly associated with peak duration of nighttime sleep episodes the night prior to testing. The MSLT-NH was also quite acceptable for use in the NH setting. In conclusion, we have developed a promising measure of daytime drowsiness in NH residents. Further testing will help establish whether this test is a clinically useful measure of daytime drowsiness from medications or other conditions in the NH setting. 相似文献

In 1986 I began research to address the relationship between early child abuse and neglect and later deliquent and violent criminal behavior using a prospective cohort design. The relationship is not inevitable, suggesting an opportunity for long-range violence prevention through appropriate early intervention. Here, I briefly describe how childhood victimization and violent criminal behavior are related and illustrate a number of promising strategies and opportunities to intervene. Finally, I offer five principles to guide interventions: (1) the earlier the intervention, the better; (2) don't neglect neglected children; (3) one size does not fit all; (4) surveillance—a double-edged sword; and (5) accessibility to resources. Rather than focusing on responses to child abuse or neglect in court proceedings that “treat” offenders, primary prevention efforts should target childhood victims to reduce their risk of becoming offenders in the future. 相似文献

Males with fragile X syndrome (FXS) have difficulties with social interaction and many show autistic features. This study
examined whether the social deficits characteristic of FXS are associated with theory of mind difficulties. Two groups of
boys with FXS participated: a group with few autistic features and a group with many autistic features. An intellectual disability
control group also participated. In addition to using standard theory of mind tasks, new techniques were used that were able
to separate out the various processing demands of the task (e.g., memory, inhibitory control). Overall, the findings indicate
that both groups of boys with FXS have difficulty with theory of mind tasks compared to an intellectual disability control
group. However, both groups with FXS also performed worse on comparison trials that required working memory but not theory
of mind. Theory of mind difficulties are likely to be an important aspect of the FXS clinical profile, but are most likely

With the dramatic increase in recent reports of physical and sexual child abuse and neglect, professionals are well as the public are increasingly concerned about the prevention of child abuse. This article provides an overview of existing child abuse prevention programs (physical abuse and sexual abuse programs are treated separately). Five computerized data bases (psychology, social work, criminal justice, education, and social sciences) were searched for literature published between 1982 and 1991. Five studies examined the cognitive association between thoughts of cancer and thoughts of death and their implication for screening intentions. Study 1 found that explicit contemplation of cancer did not increase death-thought accessibility. In support of the hypothesis that this reflects suppression of death-related thoughts, Study 2 found that individuals who thought about cancer exhibited elevated death-thought accessibility under high cognitive load, and Study 3 demonstrated that subliminal primes of the word cancer led to increased death-thought accessibility. Study 4 revealed lower levels of death-thought accessibility when perceived vulnerability to cancer was high, once again suggesting suppression of death-related thoughts in response to conscious threats associated with cancer. Study 5 extended the analysis by finding that after cancer salience, high cognitive load, which presumably disrupts suppression of the association between cancer and death, decreased cancer-related self-exam intentions. Theoretical and practical implications for understanding terror management, priming and suppression, and responses to cancer are discussed. 相似文献

The “Staying on Track” project tracked infants, preschoolers, and their families in Brockville, Ontario, Canada for a period of 3 years. Public health nurses at preassigned intervals provided information and counseling for families and made referrals to other agencies if problems were judged to be beyond the expertise of the “Staying on Track” staff. To assess the effectiveness of the program in improving child development, a cohort design was selected to allow development to be assessed over 5½ years in 3 years. Three cohorts and a comparison group were included in the study. Each cohort received different procedures for identification and tracking. Cohort 1 received at least four home visits, cohort 2 received three clinic visits, and cohort 3 received one clinic visit. Intervention and follow-up from these visits included home visits, phone calls, and referrals for all three cohorts. Comparing cohort 1 to cohort 2 at 18 months, highly significant effects were found; few significant differences were found at 3½ years between cohort 2 and 3 and very few positive effects were found for 5½-year-old children who had been in the tracking system since they were 3½ years of age.
