Psychiatric hospitalization after deliberate self-poisoning

The decision for psychiatric hospitalization after deliberate self-poisoning (DSP) is not well understood. This study, a longitudinal cohort study of 3,148 consecutive DSP patients found 920 (29.2%) subjects were referred for psychiatric hospitalization, 576 (18.3%) on involuntary basis. A logistic regression analysis showed increased risk for: age 25 or older, homelessness, unemployment, previous self-harm, psychiatric inpatient treatment within 12 months, earlier psychiatric inpatient treatment, suicidal ideation or plan, mood or psychotic disorders, and lower clinician experience; and lower risk for being married/defacto, and after hours presentation. Recommendation for psychiatric hospitalization was based on complex decision making. These findings have implications for clinical practice guidelines, service costs, and service organization.     

The role of comorbid disruptive behavior in the clinical expression of pediatric obsessive-compulsive disorder

This study explored the impact of disruptive behavior disorder (DBD) comorbidity on a number of theoretically relevant outcomes among 192 children and adolescents with obsessive-compulsive disorder (OCD). Youth were divided into three groups: OCD Only, OCD+DBD comorbidity and OCD+Other comorbidity. Relative to those without a comorbid condition and those with any non-DBD comorbidity, comorbid DBD was associated with greater family accommodation and less symptom resistance. Youth with comorbid DBD exhibited greater OCD symptom severity, OCD-related impairment, anxiety, and internalizing problems than those with no comorbidity. Those with comorbid DBD were over 3.6 times as likely to be taking an atypical antipsychotic medication relative to children without comorbidity. The present results suggest that comorbid DBD contributes to more severe illness presentation above and beyond an OCD diagnosis alone, and may be associated with differing pharmacotherapy prescribing practices.

The Conflict Between Aristotelian and Galileian Modes of Thought in Contemporary Psychology

Kraepelin combined the constant method and the method of limits to avoid the inconvenience of the former and the response bias of the latter. The use of this approach together with Guttman scaling provides a method that requires few observations, is easy to administer and score, and can be applied to a variety of measurement situations. Examples are given of its use for point of subjective equality, aesthetic preference, and thresholds.     

Pragmatic Psychodynamic Psychotherapy—Bridging Contemporary Psychoanalytic Clinical Practice and Evidence-Based Psychodynamic Practice

Pragmatic psychodynamic psychotherapy (PPP; Summers & Barber, 2009 Barber, J. P. 2009. Toward a working through of some core conflicts in psychotherapy research. Psychother. Res, 19(1): 1–12. [Taylor & Francis Online], [Web of Science ®] , [Google Scholar]) describes the therapeutic principles and approaches found in many forms of manualized psychodynamic psychotherapies, which are being used in research studies with the aim of building an evidence-based psychodynamic practice. This article focuses on one such treatment, Supportive-expressive psychotherapy (SEP; Luborsky 1984), which employs the Core Conflictual Relationship Theme (CCRT) method to formulate core interpersonal problems and treatment goals and identify therapeutic interventions. To illustrate SEP, we describe the 40-session treatment of a patient who met diagnostic criteria for several personality disorders. The case illustrates how complex ego-syntonic character pathology is expressed in CCRT terms and worked through in the transference, gradually becoming ego-dystonic and amenable to modification. In this case, the patient became more aware of his interpersonal wishes and vulnerability. Although his anxiety increased somewhat, he had an improvement in psychosocial functioning.

In this article, the author attempts to clarify the motivation and goals of early Christian ascetics by treating asceticism as a political concept. By reading the ascetic tracts of Isaac of Nineveh (7^th century), and drawing on some views on the role of the body in the practice of resistance in Michael Hardt and Antonio Negri, the author argues that ascetics like Isaac used their bodies to resist the control of ancient society and to create a tangible image – or icon – of a different way of living and of a different kind of society – the kingdom of God. Using a threefold typology of asceticism as transformative, performative and bodily, and focusing especially on the role of the body, a new way of understanding Christian asceticism emerges that, on the one hand, clearly distances itself from earlier models that viewed asceticism as an alien element in the Christian tradition; and, on the other hand, makes it possible to use the concept of asceticism to understand present day forms of resistance.
