The long-term mental health effects of war-zone deployment in the Iraq and Afghanistan wars on military personnel are a significant public health concern. Using data collected prospectively at three distinct assessments during 2003–2014 as part of the Neurocognition Deployment Health Study and VA Cooperative Studies Program Study #566, we explored how stress exposures prior, during, and after return from deployment influence the long-term mental health outcomes of posttraumatic stress disorder (PTSD), depression, anxiety disorders, and problem drinking. Longer-term mental health outcomes were assessed in 375 service members and military veterans an average of 7.5 years (standard deviation = 1.0 year) after the initial (i.e., “index”) Iraq deployment following their predeployment assessment. Anxiety disorder was the most commonly observed long-term mental health outcome (36.0%), followed by depression (24.5%), PTSD (24.3%), and problem drinking (21.0%). Multivariable regression models showed that greater postdeployment stressors, as measured by the Post-Deployment Life Events scale, were associated with greater risk of depression, anxiety disorders, and problem drinking. Anxiety disorder was the only outcome affected by predeployment stress concerns. In addition, greater postdeployment social support was associated with lower risk of all outcomes except problem drinking. These findings highlight the importance of assessing postdeployment stress exposures, such as stressful or traumatic life events, given the potential impact of these stressors on long-term mental health outcomes. This study also highlights the importance of postdeployment social support as a modifiable protective factor that can be used to help mitigate risk of long-term adverse mental health outcomes following war-zone exposure.  相似文献

Evolutionary psychologists propose that human cognition evolved through natural selection to solve adaptive problems related to survival and reproduction, with its ultimate function being the enhancement of reproductive fitness. Following this proposal and the evolutionary-developmental view that ancestral selection pressures operated not only on reproductive adults, but also on pre-reproductive children, the present study examined whether young children show superior memory for information that is processed in terms of its survival value. In two experiments, we found such survival processing to enhance retention in 4- to 10-year-old children, relative to various control conditions that also required deep, meaningful processing but were not related to survival. These results suggest that, already in very young children, survival processing is a special and extraordinarily effective form of memory encoding. The results support the functional-evolutionary proposal that young children's memory is "tuned" to process and retain fitness-related information.  相似文献

Selectively retrieving a subset of previously studied information enhances memory for the retrieved information but causes forgetting of related, nonretrieved information. Such retrieval-induced forgetting (RIF) has often been attributed to inhibitory executive-control processes that supposedly suppress the nonretrieved items' memory representation. Here, we examined the role of working memory capacity (WMC) in young adults' RIF. WMC was assessed by means of the operation span task. Results revealed a positive relationship between WMC and RIF, with high-WMC individuals showing more RIF than low-WMC individuals. In contrast, individuals showed enhanced memory for retrieved information regardless of WMC. The results are consistent with previous individual-differences work that suggests a close link between WMC and inhibitory efficiency. In particular, the finding supports the inhibitory executive-control account of RIF.  相似文献

This study investigated parental decision-making to terminate or continue a pregnancy after prenatal diagnosis of a chromosomal abnormality among a sample of patients in Southeast Turkey. Between 2004 and 2007, 1068 amniocentesis tests were performed in the Medical Biology and Genetic Department Laboratory at Dicle University. Aneuploidy was found in 38 cases (3.56%). Genetic counseling was provided for the couples that received abnormal results, and they were later interviewed and asked if they had continued or interrupted the pregnancy after the diagnosis. When confronted with autosomal aneuploidy in which a severe prognosis was expected, 85% of cases decided to terminate the pregnancy. When confronted with sex chromosome aneuploidy with a low risk of an abnormal clinical phenotype 60% of cases decided to continue the pregnancy. Among the diagnoses with aneuploidy, pregnancy was continued in 21.1% of cases due to religious beliefs regardless of whether there was a low or severe risk of an abnormal clinical phenotype. These findings indicate that both severity of abnormality and religiosity play an important role in genetic counseling patients’ decision-making processes and outcomes in Turkey. In addition, the findings suggest the need for legislation that reduces the differences in approaches between the physicians and institutions regarding parental decision-making to terminate or continue a pregnancy in our country.  相似文献

A prominent theory of cognitive development attributes the poor performance that children show in many cognitive tasks to a general lack of inhibitory control. We tested this theory by examining children’s inhibitory capabilities in retrieval-induced forgetting (RIF), a memory task in which selective retrieval of previously studied material causes forgetting of related, nonretrieved material. Such forgetting is often attributed to inhibitory control processes, which supposedly suppress the nonretrieved items’ memory representation. We examined RIF in kindergartners, second graders, and adults, using both recall and recognition testing. Although all three age groups showed significant RIF in recall, only adults and second graders, but not kindergartners, showed RIF in recognition. Because inhibition-based RIF should be present in recall and recognition, these findings indicate that in adults and second graders, but not in kindergartners, RIF is mediated by inhibition. The results support the view of inefficient inhibitory processes in young children’s cognition.  相似文献

What should clinicians know about sleep disorders? This article briefly introduces the reader to sleep medicine and succinctly reviews common sleep disorders. First, the authors describe the diagnostic tools unique to sleep medicine: the over-night polysomnogram and the multiple sleep latency test. Next, the authors review essential features of a subset of the sleep, described in the International Classification of Sleep Disorders-Revised, that sleep disorder specialists commonly evaluate, diagnose, and treat. The disorders reviewed include the intrinsic and circadian rhythm subsets of the dyssomnias group and the parasomnia group of sleep disorders. The authors identify the core signs and symptoms, polysomnogram findings, multiple sleep latency test findings, and treatment of these disorders.  相似文献

The effect of threatened morality on negative emotions and on altruistic behaviour has been shown to diminish following physical cleansing (hand-washing). We hypothesised that threatened morality will broadly impair the executive control system, and that physical cleansing will moderate this detrimental effect. Thirty-seven participants were asked to write about an immoral deed they had committed, whereupon half of them were allowed to wipe their hands. Three executive control tasks—Stroop, stop-signal, and object interference—were then administered to all participants. Participants who had not wiped their hands, but not those who did, demonstrated impaired performance, compared to hand-washing controls, in all three tasks. We conclude that threatened morality has a detrimental effect on executive control, specifically on conflict monitoring and response inhibition, and that physical cleansing “frees” this system, counteracting the detrimental effects of morality threats. We discuss possible implications for obsessive–compulsive disorder, which is characterised by deficient executive control and in which both threatened morality and physical cleansing are central concerns.  相似文献
