People with dementia have a high prevalence of psychological distress but are underserved with evidence-based psychological interventions. To promote choice and improve clinical outcomes, there is a necessity to test different psychological intervention options for this population.
Purpose
This study aimed to investigate the effectiveness and acceptability of acceptance and commitment therapy (ACT) for people with dementia, considering carer-supported, remote delivery and necessary therapy adaptations.
Methods
A hermeneutic single-case efficacy design series was used to analyse the therapy process and change for three clients with dementia and psychological distress. A matrix of quantitative and qualitative data was collated (“rich case records”) and subject to critical analyses by three independent psychotherapy experts (“judges”) who identified change processes and determined the outcome for each client.
Results
Adjudication concluded that one client made positive changes, specifically reliable reductions in psychological distress, which were largely attributable to ACT. Two clients remained unchanged.
Discussion/Conclusion
Where change was achieved, the ACT-specific processes of values, committed action and acceptance, in combination with non-specific therapy factors, including a strong client–carer relationship, existing client interests and individualised therapy adaptations, were facilitative. Hence, ACT may be a feasible and effective vehicle for therapeutic change by helping carers to better meet the needs of their loved ones. Future research to optimise ACT delivery in this population may be beneficial. Furthermore, the assessment of carer factors (e.g., their psychological flexibility and the client–carer relationship) may strengthen the evidence base for systemic ACT use. 相似文献
Prior work and theory suggest many vulnerabilities, stressors, and adaptive processes shape relationship satisfaction. In the current research, we used machine learning to understand which constructs have greater predictive importance for perceived changes in satisfaction since the pandemic began and satisfaction over the prior week. In a large sample collected at the beginning of the pandemic (N = 1873; Study 1), relationship processes were most predictive, explaining up to 70% of variance in satisfaction. Feeling appreciative of one's partner and being satisfied with quality time spent with one's partner were consistently top predictors of satisfaction. We also examined whether these important predictors were associated with changes in relationship satisfaction across the first year of the pandemic in a longitudinal subsample (N = 618; Study 2). Appreciation and satisfaction with quality time were associated with high and relatively stable relationship satisfaction over time. 相似文献
ABSTRACT— We ( Fox et al., 2005 ) recently described a gene-by-environment interaction involving child temperament and maternal social support, finding heightened behavioral inhibition in children homozygous or heterozygous for the serotonin transporter (5HTTLPR) gene short allele whose mothers reported low social support. Here, we propose a model, Plasticity for Affective Neurocircuitry, that describes the manner in which genetic disposition and environmental circumstances may interact. Children with a persistently fearful temperament (and the 5HTTLPR short allele) are more likely to experience caregiving environments in which threat is highlighted. This in turn will exacerbate an attention bias that alters critical affective neurocircuitry to threat and enhances and maintains anxious behavior in the child. 相似文献
Those who aim to give an account of modal knowledge face two challenges: the integration challenge of reconciling an account of what is involved in knowing modal truths with a plausible story about how we can come to know them, and the reliability challenge of giving a plausible account of how we could have evolved a reliable capacity to acquire modal knowledge. I argue that recent counterfactual and dispositional accounts of modal knowledge cannot solve these problems regarding specifically metaphysical modal truths—leaving us with the threat of skepticism about large portions of metaphysics, and certain other areas of philosophy. I argue, however, that both of these problems look insuperable only if we assume that metaphysical modal discourse serves a describing or tracking function. If we adopt instead a normativist approach to metaphysical modal discourse, which sees the basic function of modal discourse as giving us perspicuous ways of conveying, reasoning with, and renegotiating semantic rules, the problems show up very differently. The modal normativist can give a plausible response to both of the classic problems of how we can come to know metaphysical modal truths.
We examined which of two instruments (Text Reading and Comprehension inventory [TRC] or a traditional informal reading inventory [IRI]) provides the more valid assessment of a primary-grade student's reading instructional level. The TRC is currently the required, benchmark reading assessment for students in grades K–3 in the state of North Carolina. The TRC and IRI were administered to 196 students in grades 1–3. Results showed that the TRC, when compared to the IRI, (a) underestimated students’ reading instructional level, and (b) took three times as long to administer. What happens when people suppress their emotions when they sacrifice for a romantic partner? This multimethod study investigates how suppressing emotions during sacrifice shapes affective and relationship outcomes. In Part 1, dating couples came into the laboratory to discuss important romantic relationship sacrifices. Suppressing emotions was associated with emotional costs for the partner discussing his or her sacrifice. In Part 2, couples participated in a 14-day daily experience study. Within-person increases in emotional suppression during daily sacrifice were associated with decreases in emotional well-being and relationship quality as reported by both members of romantic dyads. In Part 3, suppression predicted decreases in relationship satisfaction and increases in thoughts about breaking up with a romantic partner 3 months later. In the first two parts of the study, authenticity mediated the costly effects of suppression. Implications for research on close relationships and emotion regulation are discussed. 相似文献
Modality presents notorious philosophical problems, including the epistemic problem of how we could come to know modal facts and metaphysical problems about how to place modal facts in the natural world. These problems arise from thinking of modal claims as attempts to describe modal features of this world that explain what makes them true. Here I propose a different view of modal discourse in which talk about what is “metaphysically necessary” does not aim to describe modal features of the world, but, rather, provides a particularly useful way of expressing constitutive semantic and conceptual rules in the object language. The result is a “modal normativist” view that enables us to avoid the epistemic problems of modality and mitigate the metaphysical worries, while also leaving open the possibility of a unified account of the function of modal language. Finally, I address a serious challenge: we have the norms we do in order to track the modal facts of the world, so that the order of explanation must go in the opposite direction. I close by showing how the normativist may answer that challenge. 相似文献
