It is almost universally acknowledged that risks have to be weighed against benefits, but there are different ways to perform the weighing. In conventional risk analysis, collectivist risk-weighing is the standard. This means that an option is accepted if the sum of all individual benefits outweighs the sum of all individual risks. In practices originating in clinical medicine, such as ethical appraisals of clinical trials, individualist risk-weighing is the standard. This implies a much stricter criterion for risk acceptance, namely that the risk to which each individual is exposed should be outweighed by benefits for that same individual. The different choices of risk-weighing methods in different policy areas seem to have emerged from traditional thought patterns and social relations, rather than from explicit deliberations on possible justifications for the alternative ways to weigh risks against benefits. It is not obvious how the prevalent differences in risk-weighing practices can be reconstructed in terms of consistent underlying principles of preventive health or social priority-setting.  相似文献

Medical decision‐making capacity (MDC) is known to decline in individuals with Alzheimer's disease (AD). The vignette method uses hypothetical information as a prerequisite for measuring the capacity to make well‐informed decisions to clinical trials. Our aim was to investigate if adapted vignettes can help individuals with mild AD to assimilate information, make decisions and express them in an understandable way, compared to corresponding decisions based on linguistically more demanding vignettes, as measured by the Swedish Linguistic Instrument for Medical Decision‐making (LIMD). Two vignettes from LIMD were altered linguistically with the aim to facilitate understanding for individuals with AD. An experimental within‐subject design was used to study the influence on MDC of readability (original/adapted vignettes) and content (two different clinical trials). We included 24 patients with mild AD in this prospective study, which read all four vignettes along with a few other tests. This allowed us to investigate the association between MDC and cognitive function. Adapted vignettes did not yield significant differences regarding MDC as compared with original vignettes using a two‐way repeated measures analysis of variance. A difference was found between the two clinical trials where LIMD score was significantly higher for Kidney disease than hypertension vignettes. Our results indicate that adapted vignettes may not improve MDC for individuals with mild AD. MDC was affected by which clinical trial the vignettes regarded, which implies that other factors affecting MDC need to be investigated, like length of text and vocabulary used.  相似文献

In order to explore public views on nanobiotechnology (NBT), convergence seminars were held in four places in Europe; namely in Visby (Sweden), Sheffield (UK), Lublin (Poland), and Porto (Portugal). A convergence seminar is a new form of public participatory activity that can be used to deal systematically with the uncertainty associated for instance with the development of an emerging technology like nanobiotechnology. In its first phase, the participants are divided into three “scenario groups” that discuss different future scenarios. In the second phase, the participants are regrouped into three “convergence groups”, each of which contains representatives from each of the three groups from the first phase. In the final third phase, all participants meet for a summary discussion. This pilot project had two aims: (1) to develop and assess the new methodology and (2) to gather advice and recommendations from the public that may be useful for future decisions on nanobiotechnology (NBT). Participants emphasized that they wanted the technology to focus on solutions to environmental and medical problems and to meet the needs of developing countries. The need for further public participation and deliberation on NBT issues seemed to be acknowledged by all participants. Many of them also raised equality concerns. Views on the means by which NBT should be steered into socially useful directions were more divided. In particular, different views were expressed on how much regulation of company activities is needed to curb unwanted developments. The participants’ responses in a questionnaire indicate that the methodology of the convergence seminars was successful for decision-making under uncertainty. In particular, the participants stated that their advice was influenced both by access to different possible future developments and by the points of view of their co-participants, which is what the method is specifically intended to achieve.

Two types of measures of probabilistic uncertainty are introduced and investigated. Dispersion measures report how diffused the agent’s second-order probability distribution is over the range of first-order probabilities. Robustness measures reflect the extent to which the agent’s assessment of the prior (objective) probability of an event is perturbed by information about whether or not the event actually took place. The properties of both types of measures are investigated. The most obvious type of robustness measure is shown to coincide with one of the major candidates for a dispersion measure, the mean square deviation measure.  相似文献

Although choice and preference are distinct categories, it may in some contexts be a useful idealization to treat choices as fully determined by preferences. In order to construct a general model of such preference-based choice, a method to derive choices from preferences is needed that yields reasonable outcomes for all preference relations, even those that are incomplete and contain cycles. A generalized choice function is introduced for this purpose. It is axiomatically characterized and is shown to compare favourably with alternative constructions.  相似文献

Traffic research shares a fundamental dilemma with other areas of empirical research in which humans are potentially put at risk. Research is justified because it can improve safety in the long run. Nevertheless, people can be harmed in the research situation. Hence, we need to balance short-term risks against long-term safety improvements, much as in other areas of research with human subjects. In this paper we focus on ethical issues that arise when human beings are directly affected in the performance of research by examining how the ethical requirements in biomedical research can inform traffic research. After introducing the basic ethical requirements on biomedical research, each of the major requirements is discussed in relation to traffic research. We identify the main areas where biomedical research and traffic research differ, and where the ethical requirements from the former cannot easily be transferred to the latter. Finally, we argue that there is a need for systematic studies of the ethics of traffic research and point to some of the issues that need to be addressed.  相似文献

In clinical neuropsychology, the present status of a patient is evaluated in relation to the assumed premorbid status. However, in Sweden, existing methods to assess premorbid status are far from optimal. In the present study, the design and evaluation of a Swedish Lexical Decision Test (SLDT) for premorbid global cognitive function (i.e., premorbid intelligence) is described. The design was based on the empirical finding that, in general adult population, word knowledge is strongly associated with measures of global cognitive functioning. Linear stepwise regression analysis demonstrated that SLDT findings accounted for 48% of the variance of global cognitive function as assessed by the Full Scale Intelligence Quotient (FSIQ) from the Wechsler Adult Intelligence Scale Revised (WAIS-R). Demographic variables alone accounted for 31% and a combination of SLDT results and demographics accounted for 60%. Psychometric properties are presented using data from 109 healthy individuals stratified according to age, gender, and level of education. In addition, a case of Alzheimer's disease is presented to illustrate the relationship between SLDT performance and cognitive function. Finally, the theoretical foundation for the relationship between word knowledge and global cognitive function is discussed.  相似文献

Blockage contraction is an operation of belief contraction that acts directly on the outcome set, i.e. the set of logically closed subsets of the original belief set K that are potential contraction outcomes. Blocking is represented by a binary relation on the outcome set. If a potential outcome X blocks another potential outcome Y, and X does not imply the sentence p to be contracted, then Y?≠?K ÷ p. The contraction outcome K ÷ p is equal to the (unique) inclusion-maximal unblocked element of the outcome set that does not imply p. Conditions on the blocking relation are specified that ensure the existence of such a unique inclusion-maximal set for all sentences p. Blockage contraction is axiomatically characterized and its relations to AGM-style operations are investigated. In a finite-based framework, every transitively relational partial meet contraction is also a blockage contraction.  相似文献

AGM 25 Years     
The 1985 paper by Carlos Alchourrón (1931–1996), Peter Gärdenfors, and David Makinson (AGM), “On the Logic of Theory Change: Partial Meet Contraction and Revision Functions” was the starting-point of a large and rapidly growing literature that employs formal models in the investigation of changes in belief states and databases. In this review, the first twenty-five years of this development are summarized. The topics covered include equivalent characterizations of AGM operations, extended representations of the belief states, change operators not included in the original framework, iterated change, applications of the model, its connections with other formal frameworks, computatibility of AGM operations, and criticism of the model.  相似文献
