Stroke is a common cause of death and disability throughout the world. Acute neurologic deficits due to ischemic injury deserve rapid recognition and diagnosis in order to provide effective therapy. Intravenous tissue plasminogen activator (t-PA) provided to carefully selected patients that can be treated within 3 hours of stroke onset results in improved outcome in these patients. Intra-arterial administration of t-PA within a 6-hour window is performed at several academic centers in patients with middle cerebral and other intracranial artery occlusions based on results of one randomized clinical trial and numerous case reports. Although acute therapy of ischemic stroke has received much attention since the approval of intravenous t-PA, only a small percentage of individuals suffering a stroke actually receive t-PA. This article will review the optimal management of the acute stroke patient and discuss thrombolytic clinical trials that have been completed as well as those that are in progress. 相似文献

In this article, the authors explore semantic context effects in speaking. In particular, the authors investigate a marked discrepancy between categorically and associatively induced effects; only categorical relationships have been reported to cause interference in object naming. In Experiments 1 and 2, a variant of the semantic blocking paradigm was used to induce two different types of semantic context effects. Pictures were either named in the context of categorically related objects (e.g., animals: bee, cow, fish) or in the context of associatively related objects from different semantic categories (e.g., apiary: bee, honey, bee keeper). Semantic interference effects were observed in both conditions, relative to an unrelated context. Experiment 3 replicated the classic effects of categorical interference and associative facilitation in a picture-word interference paradigm with the material used in Experiment 2. These findings suggest that associates are active lexical competitors and that the microstructure of lexicalization is highly flexible and adjustable to the semantic context in which the utterance takes place. 相似文献

The aim of the present study was to investigate the impact of intimate partner violence (IPV) on infant regulatory difficulties at 3 months of age and infant socioemotional problems at 12 months of age. Maternal trauma symptoms were explored as potential moderators of these associations. Participants included 120 primarily low‐income, ethnically diverse women and their infants. Results revealed that infants whose mothers experienced IPV during pregnancy did not have significantly more regulatory difficulties at 3 months than did infants whose mothers did not experience prenatal IPV. However, infants whose mothers experienced IPV during the first year after birth displayed significantly more socioemotional problems at 12 months, as evidenced by both maternal report and observational data. Furthermore, maternal posttraumatic stress avoidance symptoms served as a moderator of the association between prenatal IPV and infant regulatory difficulties at 3 months whereas maternal posttraumatic stress hyperarousal and reexperiencing symptoms served as moderators of the association between IPV during the first year after birth and infant socioemotional problems at 12 months. The findings highlight the detrimental impact that IPV can have on very young children and the importance of maternal trauma symptoms as a context for understanding the effect of IPV on young children's functioning. 相似文献

Educational level is a factor of cognitive reserve and older adults with a higher level of formal education have a better memory performance than those having a lower educational level (Angel et al., 2010; Van Der Elst, Van Boxtel, Van Breukelen, & Jolles, 2005). Memory functioning can also be modulated by the beliefs and knowledge of a person about his/her own memory, that is, by his/her metamemory (Hultsch, Hertzog, & Dixon, 1987). The objective of this study was to examine the role of metamemory as a potential mediator of the effect of educational level on memory performance. Eighty-three older adults (60–80 years) participated in the experiment, they have been divided into two subgroups according to their educational level (high: 14.36 years and low level: 9.85 years). Episodic memory was evaluated with a cued recall task and metamemory by the Metamemory in Adulthood questionnaire (MIA). As shown by previous studies, results indicated that educational level had a significant effect on memory and metamemory, higher educational level was associated to better memory and metamemory capacities. At the MIA questionnaire, older adults with a high educational level affirmed using more internal and external strategies for learning, having higher motivation and perceiving less memory change with aging than older adults with a lower educational level. They also showed that the metamemory dimensions associated to the memory performance differed according to the educational level. For participants with a lower educational level, memory performance was correlated to the participants’ perception about their memory capacity and their knowledge about memory tasks, while for participants with a higher educational level, memory performance was correlated to the dimensions linked to memory control (strategies and motivation). Finally, the group effect was mediated by metamemory, specifically by the use of internal strategies. These results suggest that a prolonged educative experience would be associated to a better capacity to implement adapted strategies, which led individuals to maintain an optimal memory performance. 相似文献

Mexican mothers have an important role in adolescent sexuality; however, they report multiple barriers to parent–child sex communication, which may impact adolescent sexual behaviour. This cross-sectional study examines whether adolescent perceptions of maternal barriers to communication are associated with adolescent sexual behaviour frequency indirectly through its association with maternal monitoring, and whether these associations differ by age and gender. Mexican adolescents (N = 1433), ages 12–19 (53% girls), completed a survey on normative sexual behaviours, adolescent perceptions of maternal barriers to sex communication, and maternal monitoring. Structural equation modelling analyses revealed that more barriers to communication (adolescent perceptions) were associated with more sexual behaviour frequency (i.e., oral and vaginal sex) among Mexican adolescents indirectly through its association with maternal monitoring. Findings were stronger for adolescents in 8th grades, but no differences were found by gender. The present study aimed to explore the relation between students’ oral reading efficacy, reading comprehension, and academic performance on a nationwide high school placement exam (TEOG). The students were selected from a public middle school. The students’ oral reading efficacy, comprehension, and TEOG achievement scores were obtained to figure out the relations between them. The students’ TEOG results were obtained from the school administration. The findings revealed that there were statistically significant relations between oral reading efficacy, reading comprehension and TEOG achievements. Particularly, students’ reading comprehension and accuracy skills together explained 57% of variance in overall TEOG achievements. 相似文献

The current study examines the processing of upright and inverted faces in 3‐year‐old children (n = 35). Event‐related potentials (ERPs) were recorded during a passive looking paradigm including adult and newborn face stimuli. We observed three face‐sensitive components, the P1, the N170 and the P400. Inverted faces elicited shorter P1 latency and larger P400 amplitude. P1 and N170 amplitudes were larger for adult faces. To examine the role of experience in the development of face processing, the processing of adult and newborn faces was compared for children with a younger sibling (n = 23) and children without a younger sibling (n = 12). Age of sibling at test correlated negatively with P1 amplitude for adult and newborn faces. This may indicate more efficient processing of different face ages in children with a younger sibling and potentially reflects a more flexible face representation. 相似文献
