Background: Globally, there is a lack of a standardized assessment process prior to the initiation of gender affirming medical interventions and consequently there is a discrepancy in this process among different transgender health services.
Aim: The main objective of this study is to investigate the outcome of the initial assessment process at a national transgender health service.
Method: The outcome of people over the age of 17 years, assessed at a large national transgender health service in the United Kingdom during a 2-year period was categorized into: (1) recommendation for cross-sex hormone treatment, or (2) no recommendation for cross-sex hormone treatment. In addition, 200 case notes were reviewed in order to investigate the level of agreement between the two clinicians involved in the assessment process.
Results: During the study period, a total number of 617 people completed their assessment at the service. Following assessment 380 (61.6%) patients were recommended for cross-sex hormone treatment, leaving 237 (38.4%) patients who required a longer assessment period or were discharged. The factors associated with being recommended for cross-sex hormone treatment were having socially transitioned, not smoking, having initiated cross-sex hormones prior to assessment, being older, and assigned male at birth. Out of the 200 case notes reviewed, agreement between assessor 1 and 2 (3 months apart) was found in 88% (n = 176) of the cases.
Discussion: Although the results of the study may not be generalizable to other international centers, questioning the assessment process and the role of the assessors is important to ensure treatment is offered in a timely and efficient manner. The purpose of the present study was to examine possible gender differences in strategy when completing the mental rotations test. Two experiments examined gender differences and the factor structure on outcomes that can be obtained on this test. Experiment 1 involved large groups testing and Experiment 2 used small groups. Factor analytic results in both experiments generally supported the notion that items with one wrong and one blank response or one correct and one blank reflect reluctance to guess, whereas one correct and one wrong or two wrong answers reflect propensity to guess. Even though the factor structure was the same in males and females, the data provided mitigated support for the hypothesis that males have a higher propensity to guess and females show a greater reluctance to guess. We conducted a comprehensive analysis of assortative mating (i.e., the similarity between wives and husbands on a given characteristic) in a newlywed sample. These newlyweds showed (a) strong similarity in age, religiousness, and political orientation; (b) moderate similarity in education and verbal intelligence; (c) modest similarity in values; and (d) little similarity in matrix reasoning, self- and spouse-rated personality, emotional experience and expression, and attachment. Further analyses established that similarity was not simply due to background variables such as age and education and reflected initial assortment (i.e., similarity at the time of marriage) rather than convergence (i.e., increasing similarity with time). Finally, marital satisfaction primarily was a function of the rater's own traits and showed little relation to spousal similarity. 相似文献
Pathological gambling represents a major public health issue. Risk factors for suicide such as major depression, substance abuse, marital breakdown, unemployment, financial crises, and legal difficulties are commonly found in populations of pathological gamblers. The objective of this study was to systematically investigate the nature of suicidal behavior among treatment-seeking pathological gamblers and its relationship to gambling characteristics and depression. Indices of suicidality were assessed in a sample of 85 treatment-seeking diagnosed pathological gamblers. High rates of suicidal ideation, suicidal plans, and attempts were found; however, no clear relationship was observed between suicidality and indices of gambling behavior. Depression rather than gambling specific characteristics, marital difficulties, or the presence of illegal behaviors appear to be related to the risk of suicidality. 相似文献
Extracellular guanine-based purines, namely the nucleotides GTP, GDP, GMP and the nucleoside guanosine, exert important neuroprotective and neuromodulator roles in the central nervous system, which may be related to inhibition of the glutamatergic neurotransmission activity. In this study, we investigated GMP effects on mice inhibitory avoidance performance and the dependence on its conversion to guanosine for such effect, by using the ecto-5'-nucleotidase specific inhibitor AOPCP. We also investigated if this conversion occurs in the central nervous system or peripherally, and if guanosine and GMP affect nociception by the tail-flick test. I.p. GMP or guanosine (7.5 mg/kg) or i.c.v. GMP (480 nmol) pretraining administration was amnesic for the inhibitory avoidance task. I.c.v. AOPCP (1 nmol) administration completely reversed the amnesic effect of i.c.v. GMP, but not of i.p. GMP, indicating that peripheral conversion of GMP to guanosine is probably relevant to this effect. AOPCP alone did not interfere with the performance. Furthermore, tail-flick measurement was unaffected by i.p. GMP and guanosine, suggesting that the amnesic effect of both purines was not due to some antinociceptive effect against the footshock used in the task. All these data together, in accordance to those previously observed in studies involving glutamate uptake and seizures reinforce the idea that guanosine is the specific extracellular guanine-based purines effector and indicate that its conversion occurs not only in the central nervous system but also peripherally. 相似文献
This study investigated state-level variation in the number of right-wing militia groups. We built on prior quantitative examinations by more carefully operationalizing theoretical concepts and testing alternative hypotheses offered to account for the etiology of social movements generally and of militias specifically. Using negative binomial regression, we found that, consistent with previous research, states with higher levels of farm job loss had more militia groups. However, in contrast to prior studies, we did not find gun culture, hospitable political climate, female empowerment, or general economic deprivation to be associated with the number of militia groups. We also found no relationship between the number of militia groups and levels of fundamentalist religious culture and minority empowerment. Our study was the first to test the effects of social disorganization on the number of militias in a state, and we found the two to be positively and significantly associated. We situate our results in the theoretical and empirical literature and outline two challenges for future research. First, scholars must operationalize variables carefully since different measures of the same theoretical concept may result in diverging conclusions. Second, since one major factor (i.e., social disorganization) found in this study to be associated with the number of militia groups in a state was examined here for the first time, it is important for researchers to continue to investigate new hypotheses. 相似文献
