Some theoretical issues in relation to the nature of obsessional rituals and the most commonly adopted method of behaviour therapy for this disorder are critically considered. International guidelines for emergency response note the importance of cultural adaptions and facilitation of local spiritual and religious healing practices when appropriate. Deciding whether traditional medicine is safe, effective and appropriate is a complex matter. This study explores the therapeutic factors of a local cleansing ritual in Northern Uganda aimed at healing former child soldiers. The components of the ritual are analysed and compared with research-based therapy for post-traumatic stress disorder. Participant observation was used during the ritual, followed by qualitative in-depth interviews and follow-up interviews over three years. The key informant is a former child soldier participating in the ritual. He was abducted and forced to commit several killings during a three-year period. The collective mobilisation of social support, the collective forgiveness and the strength of the psycho-education appear central for the effect of the ritual. In addition, the ritual draws on common therapeutic factors. The findings indicate that such a ritual involves elements that safely and effectively deal with symptoms in accordance with modern research on trauma therapy, perhaps more powerfully than Western-style therapy. Implications for collaboration with local healers in emergency settings are indicated.  相似文献

The view presented in this paper is that frameworks are needed to help understand the nature of the organization and problems of severely underorganized families. Some basic propositions are suggested as a ‘rule of thumb’ diagnostic tool. Requirements for healthy development are outlined, against which the family context of many underorganized families is illustrated. It is proposed that the life-cycle framework of such families is essentially ‘two generational’. This has implications for those offering help in these situations, and suggestions are made about appropriate treatment approaches.  相似文献

Current family therapy has as its basis the concept of the family as a system. Systems theory has developed a set of explanations that are frequently functional in nature; they are often applied as though they assume that within a system a common goal is worked towards as in biological systems. For example, the concept of homeostasis is used in much the same way as Cannon (1929) used it to describe the mechanisms of stability within biological systems such as body temperature regulation. Here all the components of the system co-operate to maintain body temperature within certain limits. This functional assumption of a common aim amongst components of a system to achieve balance should be compared with a pluralist systems analysis which does not assume such commonality of goals. For example, a pluralist analysis could highlight the conflict inherent amongst different subsystems when one subsystem has more power than the other. At a theoretical level it can be seen that family systems theory has restricted itself to a limited set of assumptions about the explanatory concepts it uses. An analysis of women's psychology which has emerged through the developments of feminist therapy can enrich the theory and practice of family therapists.  相似文献

Asylum seekers and refugees fleeing to Britain or other host countries face a range of dilemmas, which are qualitatively different from those facing any other client group. Refugees find themselves in an entirely different country, having often fled at short notice without making preparations. They may have suffered multiple practical and psychological losses. Significant numbers of refugees will have been subject to organised violence in their country of origin. Responding clinically to the needs of some of those becoming refugees has many implications and challenges for existing models of therapeutic intervention. This paper draws upon the author's clinical work in Britain and abroad, it details some observations on therapeutic practice and highlights the challenges and rewards of working with this client group.  相似文献

This paper explores the effects of exile on the subjectivity of pregnant migrant women through the lens of the processes of deterritorialization and reterritorialization. Having escaped the gaze of the parental superego, the subject’s encounter with sexuality becomes possible. However, in addition to the emancipatory aspects of migration, we observe particular somatic-psychical effects on reproductive ability. These “exile” pregnancies are generally experienced as difficult and painful, laying bare a symptomatology that is as much psychical as somatic, and which highlights the cost of a desire for independence. In this context, where perinatal risks must be evaluated and treated through an interdisciplinary approach, a clinical accompaniment proves to be indispensable for the maternity to progress smoothly on foreign soil.  相似文献

Moral injury is a complex wound of the soul affecting many veterans returning from combat. This article will propose that a blended theological anthropology, which incorporates Irenaeus’s understanding of spiritual growth and Augustine’s focus on individual accountability and sin, will better foster healing and growth from morally traumatic experiences. In order to do so, I will introduce elements of the psychological paradigm of posttraumatic growth with a theological anthropology I develop in order to propose a mindset which I believe will increase resiliency to the morally challenging environment of combat.  相似文献

‘I’m like the man who took along a brick/to show the world what his house was like.’ Reflecting on Bertolt Brecht’s brick, which is symbolically taken along from the homeland, allows us the opportunity to explore the changing modalities of traumatic experience in displaced persons. The brick does not merely represent lost objects, home, and homeland but also stands for the displaced person and his psychic integrity. The challenge for every displaced person is to transition away from the basic survival mode, dominated by the drives of self-preservation, to regain the ability to symbolize and to start dreaming again. The way a poet processes his own traumatic experiences of displacement and escape may serve as a roadmap for others. Works by Mario Benedetti and Bertolt Brecht, both émigré poets, alongside clinical examples, form the psychological material. Recurring thoughts of an uncertain return to the homeland may become debilitating, causing a displaced person to descend into mourning or melancholy and can even lead to open outbursts of previously strongly defended aggression. If it is not possible to sufficiently work through the trauma, then the bricks will fall everywhere. In the regressive emotionally charged condition, it holds true: in the beginning was the act.  相似文献

A patient's termination from group therapy is a powerful experience for the departing patient, the therapist, and all group members. Unless the feelings evoked are channeled into constructive expression, they may undermine this potentially valuable phase of both the departing patient's group treatment and the life of the group as a whole. A termination ritual, styled by a particular patient according to his or her own need, therapy goals, and personality may help the patient achieve a more clearly defined sense of self. The authors suggest that the group therapist's careful attunement to and thorough exploration of the significance of any termination ritual or gift will help to extract maximum therapeutic benefit for the departing member and the group as a whole.  相似文献

This article studies the experiences of same-sex couples in connection with a prayer ritual conducted over their registered partnerships and focuses on the pre-legal context of same-sex marriage in Finland. The aim is to analyze conformity and resistance in the participants’ understanding of personalized ritual through Grimes’s categories of language, space, time, and actors. The findings reveal that most of the rituals had both elements of resistance that were understood as following a same-sex culture and of conformity with heterosexual nuptial traditions. Double affiliation with Christian and gay culture produces complex forms of conformity and resistance. Personalization of the religious rituals was more important to the participants of the study than following heterosexual traditions.  相似文献

As a discipline of psychology coalesced during the latter half of the 19th century in American institutions of higher learning, the role of God as an explanatory force declined dramatically. Part of this story must include the role played by James McCosh, president and primary psychology professor at Princeton, 1868-1888, in assuring orthodox educated believers that the new psychology was essentially a safe endeavor and that the truths discovered therein posed no threat and ultimately would harmonize with the truths of the historic Christian faith. Several of McCosh's students later credited him with leading the way. Where McCosh led and how he did it is the focus of this article.  相似文献

This first paper focuses on the key themes that emerge in work with young asylum seekers and refugees and on our assessment of the needs of individual young asylum seekers, alongside the ways in which we meet the needs of these young people given the holistic, integrated and developmental approach of the Baobab Centre. The fact that psychotherapeutic methods are useful in this context and the need to have flexible approach to psychotherapy in our context are two ideas explored here. This paper will explore how we can slowly build sturdy and protective connections during and after the developmental years following experiences of sequential violence. This means building bridges and links within the internal worlds of these young people between: (1) different aspects of themselves that emerge after violence and loss; (2) themselves and their peers; (3) listening adults; (4) various social influences; (5) cultural influences; (6) their past, present and future lives; (7) links with local, national and international communities all of which are addressed via a ‘Transitional Non-Residential Therapeutic Community’. This paper is derived from a talk given in November 2016 as the Birkbeck Counselling Association Autumn Talk in collaboration with Psychodynamic Practice. The original talk has been divided into two related papers of which this is the second, concentrating on the effects on young asylum seekers of the political and social situation in which they find themselves when they reach the UK. Both papers explore how, working with young asylum seekers, we can slowly build sturdy and protective connections during and after the developmental years following experiences of sequential violence. This means building links within the internal worlds of these young people between themselves and: (i) different aspects of themselves that emerge after violence and loss; (ii) their peers; (iii) listening adults; (iv) various social influences; (v) cultural influences; (vi) their past, present and future lives; (vii) links with local, national and international communities. Describing further the work that takes place at the Baobab Centre, a ‘transitional therapeutic community’, and using case material to illustrate this, the author raises the challenge of how we might best address the needs of this group of children, adolescents and young adults who objectively have to live for several years of their lives in a situation of ongoing uncertainty that is in fact much more than a young person can bear. As Jo Goldstein a legal specialist, Anna Freud a child psychoanalyst and Al Solnit a paediatrician write, in their trilogy of works, States and State Agencies cannot parent children, and children need involved parenting, (i.e. involved relationships with individual caring adults). Often for children separated from their original families, the parenting role is split between large bureaucracies and foster carers who work with agencies outsourced from the Social Services Departments. Yet this country makes Social Services Departments and the Home Office responsible for decisions about whether or not child asylum seekers might remain in the UK, in fact often without a very deep assessment and hence little knowledge and understanding of their experiences and their developmental needs. Asylum seeking minors usually have to wait for decisions about their safety and protection, and about permanence and continuity, for a much longer period of time than a child can bear.  相似文献

This study examined the association between self‐reports of family rituals and relationship attachment, relationship quality, and closeness in 150 married couples in Portugal. Using structural equation modeling to examine both within‐individual and across‐partner effects, the results were generally as predicted. First, lower levels of avoidant attachment were related to greater family investment in rituals. Second, greater family investment in rituals was associated with more positive relationship quality and closeness. Third, family investment in rituals partially mediated the link between avoidance and relationship quality and closeness. Fourth, the mediational models were significant for women but not for men. These results highlight the importance of links among family interactions, couple interactions, and individual differences.  相似文献
