The majority of studies which have evaluated health related quality of life are limited in the duration of follow up. The objective of this study was to prospectively conduct an evaluation using a repeated cross sectional analysis of separate patient cohorts who were up to four years after gastric bypass surgery. Adult Roux-en-Y gastric bypass patients were recruited to the study. All patients were requested to complete a general health status questionnaire, the Short Form-36 (SF-36), before surgery or at their post operative out patient follow up visits. Patient weight was documented at each follow up visit. A cross sectional analysis was performed to evaluate SF-36 scores in each annual cohort. Data are reported as mean +/− S.D. Three-hundred-eight patients completed at least one SF-36 assessment [Initial assessment at the time of surgery, time 1, n = 245, 1y n = 149, 2y n = 70, 3y n = 59, 4y n = 61]. The SF-36 scores were greater (p < 0.05) in each of the separate post surgery cohorts for physical functioning, role limitations due to physical health, social functioning, pain, vitality, general health and the physical component summary (PCS) scores. While not comparing changes in scores within individuals over time, these data suggest early improvement especially in the physical dimension of health related quality of life. In this analysis, this finding was also observed in each of the separate cohorts up to 4 years after gastric bypass surgery.  相似文献

We examined the relations among generalized positive expectations (optimism), prostate-cancer specific expectations, and prostate cancer-related quality of life in a prospective sample of 83 men who underwent robotic assisted laparoscopic prostatectomy (RALP) for prostate cancer. Optimism was significantly associated with higher prostate cancer-specific expectations, β = .36, p < .001. In addition, optimism and prostate cancer-specific expectations were independent prospective predictors of better scores on the following prostate cancer-related quality of life scales: Sexual Intimacy and Sexual Confidence; Masculine Self-Esteem (specific expectations only), Health Worry, Cancer Control, and Informed Decision Making (βs > .21, ps < .05). When considered simultaneously, both optimism and specific expectations contributed uniquely to better Health Worry and Cancer Control scores, optimism was a unique predictor of better Sexual Intimacy and Sexual Confidence scores, and specific expectations uniquely predicted higher scores on Informed Decision Making. Although optimism and prostate-cancer specific expectations are related, they contribute uniquely to several prostate cancer-related quality of life outcomes following RALP and may be important targets for quality of life research with this population.  相似文献

Insomnia is a major public health challenge. Due to its high prevalence and impact on health in recent years it has attracted attention of health care providers. The concept of quality of life (QOL) has gained importance as an outcome measure in sleep disturbed people. This study aims to determine the prevalence of insomnia among software engineers as the job related stress is considered very high. Ninety-one software engineers aged between 21 and 45 from a software developing company in Mysore formed the study population. Insomnia Screening Questionnaire and SF 36 Health Survey Questionnaire were used to elicit information about sleep quality and quality of life respectively. Sleep status among the subjects was as follows: 20.9% severe insomnia, 35.2% mild insomnia and 43.9% normal sleeper. Mean scores for SF 36 and those for physical and mental health were considerably lower among severe insomniacs. Software engineers run at the risk of developing insomnia, those with severe insomnia had poor QOL in comparison with the others. Quality of sleep among software engineers needs special attention since they are prone to develop sleep disturbances.  相似文献

The aim of this study was to examine the relationship between health-related quality of life and brain-tumor related posttraumatic stress symptoms (PTSS) in adults who had been treated for a primary benign meningioma (MGM). Utilizing a cross-sectional survey design, 70 adults who had previously been treated with radiotherapy [M = 4.4 years] for a benign MGM were administered questionnaires assessing MGM-related PTSS, health-related quality-of-life (QOL), and current mood, as well as a test of executive functioning. Sixteen percent of the sample reported heightened MGM-related PTSS. Elevated PTSS was significantly associated with a substantial reduction in executive functioning, QOL, as well as heightened symptoms of depression, fatigue and confusion. The majority of participants with elevated PTSS reported the need to have professional support due to feeling distressed and having a fear of their tumor progressing or recurring. The findings suggest that one in six patients treated for a benign MGM are at risk of experiencing MGM-related PTSS both in the short- and longer-term, and PTSS may seriously hamper patients QOL irrespective of prognosis. The findings suggest that psychological effects need to be taken into account when treating MGM patients.  相似文献

This paper aims to examine whether life satisfaction and health-related quality of life (HRQoL) are simultaneously related, as well as to quantify the bias that occurs if simultaneity is not accounted for. The study sample consisted of 870 respondents, representative of the Spanish adult general population. Using a simultaneous equations system —with the satisfaction with life scale (SWLS) and the SF-6D index as outcome variables—, we found a simultaneous association between life satisfaction and HRQoL, although this relationship is heterogeneous in individual characteristics such as age and sex. More important, the fact of estimating the relationship between life satisfaction and HRQoL under a unidirectional approach severely underestimates the effect of life satisfaction on HRQoL and, to a lesser degree, the reverse direction effect. In consequence, policy decisions intended to improve satisfaction with life or HRQoL can be wrong if they rely on unidirectional estimates. Another relevant implication of this research is that, as a result of the simultaneous relationship between life satisfaction and HRQoL, not only health interventions may increase satisfaction with life, but also policies that improve life satisfaction can lead to positive side effects on HRQoL.  相似文献

Obesity not only increases risk for morbidity/mortality, but also impacts the quality of life of obese individuals. In the United States, black women have the highest prevalence of obesity of any other group with approximately 80% of black women over age 20 having a body mass index (BMI)????25?kg/m2. We aimed to examine the association between BMI and quality of life in this high risk population compared to this association in white women, using the Impact of Weight on Quality of Life (IWQOL)-Lite questionnaire. Data from 172 black women (mean BMI?=?35.7; age?=?40.5) and 171 white women (mean BMI?=?35.5; age?=?40.4) were collected between 2000 and 2010 analyzed in 2010. The mean IWQOL-Lite total score was 81.6 for black women compared to 66.9 for white women, a statistically significant difference. Hierarchical linear regression models revealed a significant BMI-by-race interaction indicating that the relationship between BMI and IWQOL-Lite score was moderated by race. Our findings suggest notable differences in weight-related quality of life in black and white women. At similar BMIs, black women consistently reported better quality of life than white women on all IWQOL-Lite subscales. The greatest difference in IWQOL-Lite scores between black and white women was seen in the self-esteem subscale. Additional research is needed to understand how to incorporate the weight perspectives of black women into weight management messages and interventions.  相似文献

Relationships between perceived life satisfaction and sexual risk-taking behaviors were examined in a statewide sample of public high school students (n = 4,758) using the self-report CDC Youth Risk Behavior Survey (YRBS). Adjusted polychotomous logistic regression analyses and multivariate models (via SUDAAN) constructed separately, revealed a significant race by gender interaction for each race-gender group. Age of first intercourse (13), two or more lifetime sexual intercourse partners, alcohol/drug use before last intercourse, no use of contraception at last intercourse, being forced to have sex, forcing someone to have sex, and having beaten up a date in the last 12 months and having been beaten up by a date (in last 12 months) were associated (p = .05) with reduced life satisfaction. Measures of life satisfaction as a component of comprehensive assessments of adolescent sexual risk-taking behaviors in fieldwork, research, and program-evaluation efforts should be considered.  相似文献

Following recent contradictory findings, the present aim was to further examine the association between religiosity and life satisfaction. The Francis Scale of Attitude Towards Christianity (Francis & Stubbs, 1987) and the Satisfaction With Life Scale (Diener, Emmons, Larsen & Griffin, 1985) were completed by 55 (31 male and 24 female) Northern Irish adults and 141 (50 male and 91 female) Northern Irish students. Among the adult sample, a significant association was found between scores on the Francis Scale of Attitude Towards Christianity and the Satisfaction With Life Scale for both males and females. However, among the student sample, no significant association was found for either males or females. These data are consistent with previous findings with evidence that, among Northern Irish adults, but not among Northern Irish students, those with a more positive attitude towards Christianity are more satisfied with life.  相似文献

This study aims to investigate the utility of the Contextual Model of Health-Related Quality of Life (HRQOL) to explain the relationship among the domains of HRQOL with a diverse, population-based sample of breast cancer survivors (BCS). We employed a cross-sectional design to investigate HRQOL among 703 multiethnic, population-based BCS. The study methodology was guided by the Contextual Model of HRQOL. Structural Equation Modeling (SEM) was conducted to assess the hypothesized model. SEM identified significant relationships among the bio-psychological domain (general health status, cancer-related factors, and psychological factors), the cultural-socio-ecological domain (health care satisfaction, socio-ecological factor, and socio-economic status), and HRQOL. The best fitting model indicates direct pathways from ‘general health status’, ‘years since diagnosis’, ‘health care satisfaction’ and ‘socio-ecological factor’ to ‘HRQOL’ variables. Additionally, ‘socio-ecological factor’ and ‘socio-economic status’ variables were indirectly associated with HRQOL through ‘general health status’. Findings suggest that the Contextual Model of HRQOL adds valid factors to explain overall HRQOL and increases our understanding of the socio-ecological dimensions predicting HRQOL outcomes. The revelation of inter-relations among the dimensions of HRQOL may inform the translational and clinical utility of the HRQOL construct.

Quality of life is a subjective and multidimensional concept that includes all aspects of an individual’s life. Many investigations indicate that marital satisfaction is an important determinant of quality of life and social support affects both marital satisfaction and quality of life. Since medical staff face job stressors in their everyday life, in the present cross-sectional study, the relationships between quality of life, marital satisfaction, and social support in medical staff in Tehran were analyzed. Data collection was performed in 653 medical staff using socio-demographic questions, the SF-36 questionnaire, the ENRICH Marital Satisfaction Inventory, and the Social Support Questionnaire. The results revealed that men reported significantly higher quality of life and marital satisfaction than women, but there was no gender difference in social support. The quality of life and marital satisfaction domains were found to be differentially correlated among male and female participants. All domains of marital satisfaction were found to be significantly associated with the “vitality” and “mental health” domains among females and “physical functioning” domain in the male. Also, social support was associated with almost all quality of life domains in both sexes. The multiple regression analyses indicated that all the study variables combined with socio-demographic factors can explain between 12 % and 28 % of the variance in quality of life domains. Therefore, focusing on these factors could be an effective approach to improve quality of life in medical staff as a group with a stressful job.  相似文献

The possibilities of adverse drug reactions and disease-related psychosocial implications have led to the current emphasis on health-related quality of life (HRQoL) assessment in epilepsy. The main objective of this study was to determine the impact of various demographic characteristics i.e. gender, age, marital status, ethnic origin, religion, and employment status on HRQoL profiles in Malaysian epilepsy patients. Patients with epilepsy were recruited from ten public hospitals in Sabah, Malaysia. Upon written consents, they completed the Malay Quality of Life In Epilepsy-30 (QOLIE-30) instrument which assesses Seizure Worry, Overall QoL, Emotional Well-Being, Energy/Fatigue, Cognitive Functioning, Medication Effects and Social Functioning. An Overall Score was generated to represent a summarised HRQoL score. Univariate and multiple linear regression analyses were both carried out using SPSS 14 for (1) all patients and (2) patients possessing Poor QoL (Overall Score < median) and Good QoL (Overall Score ≥ median). One-hundred-and-thirteen patients participated in the study. The respondents ranged from 18–76 years with a mean age of 31 years. Majority were Kadazandusuns (n = 46; 40.7%) and many were unmarried (n = 68; 60.2%), Muslims (n = 50; 44.2%) and unemployed (n = 54; 47.8%). Age was the most significant factor across all patients (B = −0.372, t = −2.238, p = 0.027), regardless of Poor or Good QoL level. Older patients (>56 years) fared the worst in many HRQoL areas. Those with Poor QoL were mostly affected by marital status (widowed/divorced) and ethnic origin (Kadazandusun/Chinese). Religion (Buddhism/Hinduism) emerged as the most prominent HRQoL predictor for patients experiencing Good QoL and being Chinese too played a role. These outcomes indicate that specific demographic characteristics are influential in determining the HRQoL of epilepsy patients. Consequently, healthcare professionals could utilise such information in planning the best treatment and care by considering both the medical and psychosocial impacts towards patients. This study examined the social, infrastructural and economic conditions prevalent in some rural communities of Enugu State, Nigeria, where recent oil and gas prospecting activities have generated concerns for the environment and quality of rural life. Mixed but complimentary methods of data collection employed include consultations with key stakeholders, focus group discussions, questionnaire survey, field observations, key-informant interviews and secondary sources. Existing socio-economic and infrastructural conditions, citizens’ perceptions of oil and gas activities, and quality of life (QoL) expectations of study communities were documented. Six major units of analysis (or levels of aggregations) with the aid of qualitative and quantitative methodologies were used to describe both empirical and /or secondary data. The study revealed large population sizes and even higher population densities, with the economic base in the study area being agriculture. Findings of the study indicate that despite some negative socio-environmental perceptions, citizens’ receptiveness and attitude towards the projects were patently positive. QoL expectations of the communities were embedded in environmentally responsible, culturally sensitive and socially responsive operation of oil companies. Agents and institutions of social control and well- being were also appraised to highlight their QoL contributions in the area.  相似文献

Using more precise methodology, the current study investigated expectations of psychotherapy duration for specific disorders. Duration expectations were obtained for young adults and compared to those obtained from psychotherapists as reported by Lowry and Ross (Psychotherapy: Theory, Research, Practice, Training 34:272–277, 1997). Results revealed that expected duration for young adults by disorder was quite similar to the psychotherapists, where less severe psychological disorders and problems were expected to require shorter treatment durations than moderate and severe disorders and problems. For disorders and problems with which young adults were familiar, their expectations were essentially the same as psychotherapists. A meaningful difference was found for suicidal ideation, with young adults expecting significantly longer treatment duration.  相似文献

The purpose of this study was to distinguish between the influence of attachment styles and behaviors on marital quality for couples. Data were gathered from 680 couples in a married relationship. Results showed attachment style and behaviors predicted marital quality for both men and women, with higher levels of attachment related to greater quality. Attachment behaviors predicted more of the variance in quality than did styles. Specific implications regarding how therapists may wish to foster behaviors that promote attachment security in marriages are discussed.  相似文献

The purpose of this study was to determine health-related quality of life (HRQoL), fatigue, and posttraumatic growth (PTG) among patients with breast or prostate cancer during and following radiation therapy (RT). A total of 91 patients completed measures at three time points as follows: one or two weeks before the start of RT, in the fifth/sixth week of treatment, and four to six weeks post-treatment. Consistent with the previous literature, the results of this study revealed that RT was associated with declines in global HRQoL, Physical Well-being, Social/Family Well-being, and Functional Well-being, as well as an increase in fatigue. By four to six weeks post-treatment, these factors improved significantly and returned to baseline levels with the exception of Social/Family Well-being. The PTG total score and almost all of its subscale scores did not change significantly during the period of the study. However, the subscale of Spiritual Change for PTG significantly increased during the RT, and sustained this improvement at four to six weeks post-treatment. A significant negative correlation was found between Physical Well-being and PTG total score, whereas a significant positive correlation was revealed between Social/Family Well-being and PTG total score. The results from this analysis suggest that a significant decrease in HRQoL is evident in the fifth/sixth week of RT treatment. However, recovery of HRQoL occurs as quickly as a few weeks after the end of treatment. Moreover, patients with high ratings of Social/Family Well-being experience more positive psychological changes (PTG) resulting from the cancer experience.  相似文献

分析晚期胃癌患者的生活质量情况及其影响因素,为晚期胃癌患者的临床护理干预提供科研数据.选择2010年5月至2011年5月在辽宁医学院附属第一医院胃肠外科及肿瘤科的晚期胃癌患者173名进行生活质量的乱查,并通过EORTC QLQ —C30,SSRS,MCMQ,SAS等量表和一般人口学资料及临床病例资料进行评估.晚期胃癌患者总体生活质量评分低,仅29.54±12.21;社会支持、医学应对方式、焦虑及患者临床资料(除放疗外)对晚期胃癌患者总体生活质量影响较明显(P<0.05).晚期胃癌患者的生活质量较差,其影响因素是多方面的,临床上医务人员应采取必要的措施提高晚期胃癌患者的生活质量.  相似文献

Previous research has shown that life satisfaction (LS) and self‐reported health status are strongly related to each other. However, it is not clear whether this association holds across different nations or whether certain country‐level indicators significantly affect this association. The study was based on nationally representative samples of 32 countries from the first six rounds of the European Social Survey (N = 291 686). Results from hierarchical multilevel modelling indicated that there was a positive association between LS and self‐reported health status across countries, but this association was slightly stronger in countries where governments spent less on the health care of their residents. Self‐reported health ratings were also more strongly tied to LS judgements in countries where variability in LS ratings was larger. These results suggest that, especially in less wealthy European countries, policies should target reducing overall social inequality and the negative impact of governments' underinvestment in health care on LS. Purpose  We adopted the conservation of resources model (COR, Hobfoll Am Psychol 44:513–524, 1989; Hobfoll in Stress, culture, and community: the psychology and philosophy of stress, Plenum, New York, 1998) to examine the associations among emotional labor, work family interference, and quality of work life. Design/Methodology/Approach  Cross-sectional, self-reported data were obtained from 442 Hong Kong Chinese service employees. Findings  Correlation and hierarchical regression analyses showed that surface acting was a salient correlate of work-to-family interference, even when organizational display rules and employees’ demographic information were controlled. Furthermore, quality of work life had partially mediated the relationship between surface acting and work-to-family interference. However, deep acting and expression of naturally felt emotion did not relate to work-to-family interference. Finally, we found that family-to-work interference was a salient correlate of the use of surface acting in workplace. Implication  This study provided useful information of how adopting different emotional labor strategies related to work family interference. Based on our results, the use of deep acting should be promoted in workplace because it related positively to quality of work life and it did not amplify the work-to-family interference. Originality/value  While past studies often explored the role of emotional labor as the precursor of work family interference, our study is among the first attempt to examine family-to-work interference as the antecedent of emotional labor. Additionally, we had also confirmed the role of quality of work life as an important mediator between emotional labor-work-to-family interference.  相似文献

Theories suggest that having a meaningful life has beneficial effects on adolescent well-being. Encouraging adolescents to search for meaning in their lives is therefore well advised. However, whether and how the search for meaning in life (SMIL) is related to adolescent well-being is unclear. Thus, this study tested the following two hypotheses, based on a sample of 1539 Chinese adolescents in the tenth grade (Mage = 15.8 years): 1) the SMIL promotes adolescent well-being (“SMIL-as-promotor”), and 2) social connectedness mediates the link between the SMIL and adolescent well-being (“connectedness-as-mediator”). Multiple regression analyses revealed that SMIL was positively associated with life satisfaction, self-esteem and positive affect, and negatively associated with negative affect in the adolescents who exhibited low levels of presence of meaning; thus supporting the SMIL-as-promotor hypothesis. Mediation analyses revealed that SMIL was related to social connectedness (i.e., parent-child communication and peer relationship), which, in turn, was linked to the presence of meaning and hedonic well-being (i.e., life satisfaction, self-esteem, positive affect, and negative affect). These findings support the connectedness-as-mediator hypothesis. The implications of these findings for youth prevention programs and intervention services are discussed.
