Angst und Zwang

We report a 33-year-old woman with the diagnosis of agoraphobia with panic disorder, obsessive thoughts and a depressive episode. The patient was treated at University Clinic for Psychiatry Graz twice. After the hospital stays a continuous outpatient treatment was established. During in- and outpatient treatment classic behaviour therapy methods as well as mindfulness-based treatments and elements of the schema therapy were applied.     

Die Welt und die Evidenz. Zu Husserls Erledigung des Cartesianismus

This paper aims to show that Husserl’s thought represents a dismissal of Cartesianism. I argue that at the basis of Husserl’s thought lies an account of perception and evidence that is completely different from Descartes’. Anticipating an insight which will be developed by analytical philosophy, Husserl claims that a perception or evidence can be called into question only on the basis of other perceptions and evidences. Indeed, all questioning of a single perception or evidence presupposes that perception and evidence are reliable and cannot concern perception and evidence as such, but only their single instances. Therefore, phenomenological reduction is not a methodological doubt, and Husserl’s cogito has a different meaning from Descartes’ cogito. This approach is based on an account of reality, at the core of which lies the identification between what is real and what is experienceable, but it does not lead to a reduction of things to consciousness.

The time frame of psychoanalytic long-term treatment depends on the therapy goals and the means by which these are expected to be reached. A core element for this is the concept of working through. In Freud’s view working through deals with resistance towards the acceptance of an interpretation regarding unconscious conflicts. Also, in classical terms there is a working through of the transference neurosis. Conceptual intertwining with the work on mourning (in terms of libidinal decathexis) brings a difficulty to light, namely the potential danger of misusing long-term treatment and the therapeutic working relationship for avoiding termination and separation. The basic concept of working through can be maintained for patients with non-neurotic disorders as well as regarding modified psychodynamic psychotherapies with fewer sessions and lower session frequency, albeit in consideration of the theory of therapeutic change for each form of treatment; however, a three-fold meaning of working through in contemporary psychoanalysis (i.e. working through of resistances, working through of transference neurosis and the structurally changing inner work of the patient) indicates the need to discuss the necessary duration of long-term treatment neither from a viewpoint of economic efficiency nor from a standpoint of mere professional policy but to base this on the logic of the dynamics of a disorder and the treatment planning.
