On an influential model of limb praxis, ideomotor apraxia results from damage to stored gesture representations or disconnection of representations from sensory input or motor output (Heilman & Gonzalez Rothi, 1993; Gonzalez Rothi et al., 1991). We report data from a patient with progressive ideomotor limb apraxia which cannot be readily accommodated by this model. The patient, BG, is profoundly impaired in gesturing to command, to sight of object, and to imitation, but gestures nearly normally with tool in hand and recognizes gestures relatively well. In addition, performance is profoundly impaired on imitation of meaningless gestures and on tasks requiring spatiomotor transformations of body-position information. We provide evidence that BG's apraxia is largely attributable to impairments external to the stored gesture system in procedures coding the dynamic positions of the body parts of self and others; that is, the body schema. We propose a model of a dynamic, interactive praxis system subserved by posterior parietal cortex in which stored representational elements, when present, provide "top-down" support to spatiomotor procedures computed on-line. In addition to accounting for BG's performance, this model accommodates a common pattern of ideomotor apraxia more readily than competing accounts.  相似文献

Two models have been suggested to depict the relationship between disorders of limb and orofacial praxis. The first views apraxia as a unitary disorder in which the underlying mechanisms for each type are similar, while the second model suggests that there are two separate praxis systems: one for planning and controlling limb gestures and a second one for planning and controlling orofacial movements. The purpose of this study was to investigate whether a common mechanism may underlie deficits in limb and orofacial praxis in children. This was done by analyzing the types of praxis errors demonstrated by children with developmental motor deficits and normal controls when performing limb and orofacial gestures. Results indicated that there was consistency across modalities (i.e., limb, orofacial) in the types of praxis errors made by children with motor deficits, providing support for the idea that a common mechanism may underlie disruptions to limb and orofacial praxis in children. This study also examined developmental trends in gestural representation and in types of praxis errors. The findings revealed a striking developmental maturation in gestural ability between the ages of 6 and 11 years for all children. However, over this age range, children with developmental motor deficits were impaired relative to normal controls.  相似文献

For normal writing it is essential that both motoric and linguistic competence be present; disruption of one or the other of these faculties may result in qualitatively different types of agraphia. Two right-handed patients became agraphic after left hemisphere lesions; pure apraxic agraphia in the absence of limb apraxia developed in one patient and pure linguistic agraphia in association with severe ideomotor limb apraxia in the other. The performance of these patients not only serves to illustrate the dissociation between the motoric and linguistic faculties that underlie writing but also confirms that ideomotor limb apraxia and apraxic agraphia are distinct and dissociable entities.  相似文献

Assessment of limb apraxia is still suffering from Liepmann's legacy and performance in gesture-processing tests is generally rendered by classifying patients' profile according to the classic clinical labels of ideomotor and ideational apraxia. At odds with other cognitive functions, interpretation of apraxia has suffered from a lack of a reliable model which does justice to its complexity. Recently such a model has been proposed (Rothi et al., 1991, 1997). In this article a modified version of this model is presented and predictions are made according to its functional architecture. Five different patterns of impairment of gesture processing are postulated. To validate the predicted performance profiles, 19 left-hemisphere-damaged patients were assessed by means of an ad hoc battery of four praxis tests. Four of the five predicted apraxia patterns were observed, the fifth being more equivocal. These results support the need to overcome the simplistic dichotomous view of apraxia and confirm the fruitfulness of a model of normal gesture processing in order to understand dissociations in apraxia.  相似文献

The relations between finger localization and various types of finger praxis were assessed by estimates of the inter-correlations among the performance of 100 six-year-old children. Finger localization was related significantly to those types of finger praxis which involve movements of the individual fingers with respect to each other. Right-left discrimination was related to finger localization, but not to the measures of finger praxis.

The findings are interpreted in terms of the construct of the finger schema, which can be viewed as a determinant of performance in both the localization and praxis tasks. This construct can also serve as an explanation of the observed concurrence of finger agnosia and finger apraxia as a consequence of cerebral disease.  相似文献

A neurological truism is that each side of the brain controls movements on the opposite side of the body. Yet some left hemisphere brain lesions cause bilateral impairment of complex motor function and/or ideomotor apraxia. We report that the left dorsal premotor cortex of normal right-handed people plays a fundamental role in sequential movement of both right and left hands. Subjects performed sequential finger movements during functional magnetic resonance imaging of the motor cortices. In right-handed subjects, the volume of activated dorsal premotor cortex showed a left hemispheric predominance during hand movements. We suggest that the observed left premotor dominance contributes to the lateralization found in lesion studies.  相似文献

An extensive set of neuropsychological measures was administered to 60 Parkinson's disease patients and age-, sex-, and education-matched controls in order to investigate the nature and prevalence of the cognitive deficit in the disease. Parkinsonian patients performed significantly poorer on all measures with the exception of tests for apraxia and object recognition, and on a test of vocabulary knowledge. Discriminant analysis of the test data revealed that over 93% of patients are impaired relative to matched controls, but that assigning a prevalence rate for dementia in the disease may be difficult due to the continuous distribution of cognitive deficits.  相似文献

A basic neuropsychological examination of language and praxic abilities was administered to extreme educational groups (100 illiterates and 100 professionals). Subjects were matched according to sex and age (16-25, 26-35, 36-45, 46-55, and 56-65). The following tasks were included: language comprehension, phonological discrimination, naming (objects, figures, and body parts), repetition of words, verbal fluency, calculation, buccofacial and ideomotor praxis, finger alternating movements, meaningless movements, cancellation task, coordinated movements with both hands, and motor impersistence tasks. All the differences between the two educational groups were statistically significant. Two of the eight language tests (phonological discrimination and naming figures) and three of the seven praxic tests (buccofacial praxis, coordinated movements, and cancellation task) showed differences between age groups with a better performance in the younger groups. Calculation tasks and ideomotor praxis showed differences between sexes with a better performance in males. Influence of educational factors in performing routine neuropsychological tests is analyzed.  相似文献

Limb apraxia is a neurological disorder of higher cognitive function characterized by an inability to perform purposeful skilled movements and not attributable to an elementary sensorimotor dysfunction or comprehension difficulty. Corticobasal Syndrome (CBS) is an akinetic rigid syndrome with asymmetric onset and progression with at least one basal ganglia feature (rigidity, limb dystonia or myoclonus) and one cortical feature (limb apraxia, alien hand syndrome or cortical sensory loss). Even though limb apraxia is highly prevalent in CBS (70–80%), very few studies have examined the performance of CBS patients on praxis measures in detail. This review aims to (1) briefly summarize the clinical, neuroanatomical and pathological findings in CBS, (2) briefly outline what limb apraxia is and how it is assessed, (3) to comprehensively review the literature on limb apraxia in CBS to date and (4) to briefly summarize the literature on other forms of apraxia, such as limb-kinetic apraxia and buccofacial apraxia. Overall, the goal of the review is to bring a model-based perspective to the findings available in the literature to date on limb apraxia in CBS.  相似文献

The relevance of gesture knowledge for the semantic representation of manipulable objects was investigated in a series of 15 patients with a focal left-hemisphere lesion. The patients were classified according to the presence/absence of ideomotor and ideational apraxia. We investigated picture naming and word-picture matching (pointing to a picture on verbal command) with stimuli including items from three categories: manipulable objects, non-manipulable objects, and animals. The analysis was performed at group level and at single-patient level. Nine patients were affected by ideational apraxia and nine by ideomotor apraxia: two cases presented ideomotor but not ideational apraxia, and two cases presented the opposite dissociation. Patients affected by ideational apraxia were more severely impaired in all tasks, but did not show a disproportionate impairment with the category of manipulable objects in contrast to the other categories. The presence of ideomotor apraxia did not cause a greater impairment in any task or category. Finally, we observed a patient with ideational apraxia who performed flawlessly in naming and word-picture matching for all the stimuli, including manipulable objects. In conclusion, we did not find a relationship between ideational apraxia and a disproportionate impairment of the semantic knowledge of manipulable objects.  相似文献

This study was desinged to determine if motor deficits in limb apraxia are task specific. Non-brain-damaged patients and apraxic and nonapraxic patients with left hemisphere damage performed language and limb apraxia tests and six motor tasks with the left hand. Contrary to previous data, no significant group differences occurred on a finger tapping task. Although task complexity or sequencing requirements affected group differences, greatest apraxic impairment was noted on a task of precise steadiness, but only when its response inhibition requirements were increased. This pattern of deficits appears to be associated with greater involvement of the premotor area in the apraxic group.  相似文献

Limb apraxia is a neurological deficit characterized by an inability to pantomime and/or imitate gestures, which can result from neurodegenerative disorders such as Alzheimer's disease (AD). The major goal of the study was to describe comprehensively the apraxia deficits observed in AD patients and to relate those deficits to general cognitive status, measures of daily activity, and other neuropsychological measures. Limb apraxia was assessed on a variety of conceptual and gesture production tasks in 30 AD patients. As a group, AD patients were impaired across gesture production tasks: of note was the greater impairment in imitation, as opposed to pantomime, which was especially pronounced when patients were imitating with a delay. Imitation performance was best predicted by measures of visuospatial processing, while imitation with delay was best predicted by measures of working memory. In addition, pantomime in response to pictures of tools was less accurate than Pantomime to Verbal Command and holding the tool during performance did not decrease the participants' impairment, while introducing a verbal cue during imitation increased the severity of deficits. Furthermore, investigation into patterns of deficits clearly demonstrated that the nature of limb apraxia deficits observed in AD can be quite heterogeneous and that dissociations between the conceptual and the production system exist. Finally, we also report on significant correlations between general cognitive status and limb apraxia.  相似文献

The present study was designed to investigate the relationship between performance of limb gestures and the severity of Alzheimer's disease (A.D.). Apraxia tends to occur at later stages of A.D., and the severity of apraxia has been shown to vary with the severity of A.D. dementia. Participants were 19 mild (including 9 with no cognitive impairment and 10 with mild impairment) and 18 moderate A.D. patients as well as 25 controls and they were asked to pantomime (P) or imitate (both concurrent (CI) and delayed (DI)), eight transitive gestures to assess praxis performance. Results indicated that the moderate patients performed less accurately than mild and non-impaired patients, and that across all groups, the imitation conditions were performed less accurately than pantomime, relative to controls. Correlational analyses revealed that MMSE scores were correlated with all three performance conditions suggesting that impaired praxis performance may relate to more global impairment. Finally, a frequency analysis was conducted to examine whether A.D. patients showed patterns of apraxia as described in Roy's model (1996). Results indicated that A.D. patients showed greatest impairment on pattern 7 (deficits in P, DI, and CI), reflecting late-stage gesture production, with a greater frequency of moderate patients exhibiting each apraxic pattern.  相似文献

Corticobasal degeneration (CBD) is a degenerative disease that often presents with an asymmetric progressive ideomotor limb apraxia. Some apraxic subjects may fail to perform skilled purposive movements on command because they have lost the memories or representations that specify how these movements should be performed (representational deficit). In contrast, other apraxic subjects may have the movement representations but are unable to utilize the information contained in them to execute skilled purposive movements (production-execution deficit). To learn if the apraxic deficit in CBD is induced by a representational or a production-execution deficit, we tested three nondemented subjects with CBD on tasks requiring production of meaningful or meaningless gestures to command, gesture imitation, gesture discrimination, and novel gesture learning. A fourth subject with incomplete data also is presented. The results suggest that the apraxia associated with CBD is initially induced by a production-execution defect with relative sparing of the movement representations.  相似文献

The problem of how word meaning is processed in the brain has been a topic of intense investigation in cognitive neuroscience. While considerable correlational evidence exists for the involvement of sensory-motor systems in conceptual processing, it is still unclear whether they play a causal role. We investigated this issue by comparing the performance of patients with Parkinson’s disease (PD) with that of age-matched controls when processing action and abstract verbs. To examine the effects of task demands, we used tasks in which semantic demands were either implicit (lexical decision and priming) or explicit (semantic similarity judgment). In both tasks, PD patients’ performance was selectively impaired for action verbs (relative to controls), indicating that the motor system plays a more central role in the processing of action verbs than in the processing of abstract verbs. These results argue for a causal role of sensory-motor systems in semantic processing.  相似文献

The praxis test is a less well-documented method to determine functional manifestations of childhood dyspraxia. For this study, children aged 6–8 years were recruited as follows: 17 children with DCD, 18 at risk of DCD and 35 without obvious problems in motor coordination. The Movement Assessment Battery for Children (MABC-2) was used to measure motor performance and identify the motor incoordination. This study developed a battery of tests to assess limb praxis using a praxis imagery questionnaire, gesture representation, and questions about knowledge of object use. In the comparison of subtests within the praxis test, significant differences were observed across groups on the praxis imagery questionnaire and gesture representation tests but not on knowledge of object use. Similar results were observed in the correlation analyses, in which a weak relationship between MABC-2 and praxis tests was observed. The DCD group had lower scores on the praxis imagery questionnaire, whereas the group at risk of DCD had lower scores on most gesture production tests. Our study provides a better understanding of the nature of the childhood dyspraxia and sheds light on its effect on motor coordination to identify praxis tests with specific clinical meanings in children with movement disorders.  相似文献

Patients with Parkinson’s disease (PD) have difficulties in movement adaptation to optimize performance in novel environmental contexts such as altered screen cursor-hand relationships. Prior studies have shown that the time course of the distortion differentially affects visuomotor adaptation to screen cursor rotations, suggesting separate mechanisms for gradual and sudden adaptation. Moreover, studies in human and non-human primates suggest that adaptation to sudden kinematic distortions may engage the basal ganglia, whereas adaptation to gradual kinematic distortions involves cerebellar structures. In the present studies, participants were patients with PD, who performed center-out pointing movements, using either a digitizer tablet and pen or a computer trackball, under normal or rotated screen cursor feedback conditions. The initial study tested patients with PD using a cross-over experimental design for adaptation to gradual as compared with sudden rotated hand-screen cursor relationships and revealed significant after-effects for the gradual adaptation task only. Consistent with these results, findings from a follow-up experiment using a trackball that required only small finger movements showed that patients with PD adapt better to gradual as against sudden perturbations, when compared to age-matched healthy controls. We conclude that Parkinson’s disease affects adaptation to sudden visuomotor distortions but spares adaptation to gradual distortions.  相似文献

Ipsilateral motor and symbolic/linguistic skills were examined in normals and three left hemisphere damaged groups with no, mild, or moderate-severe limb apraxia. When the two apraxic groups were pooled they demonstrated poorer response inhibition and reading relative to the nonapraxic group. There were no significant group differences on motor or language tasks when the three brain damaged groups were separately compared. These results indicate there is a limited relationship between limb apraxia and some motor and linguistic skills, but the relationship was not greater for motor than linguistic skills.  相似文献

There are at least two possible models depicting the relationship between buccofacial and limb apraxia. First, apraxia can be viewed as a unitary motor disorder which transcends the output modalities of both buccofacial and limb output. A high degree of similarity between the two types of apraxia would support this model. Alternatively, the relationship between buccofacial and limb apraxia may not include a unitary mechanism. The presence of quantitative and qualitative differences between buccofacial and limb performance would support this nonunitary model. Motor symptomatology in autism is currently poorly understood, and still not included in the autism spectrum disorder (ASD) diagnostic criteria, although some studies suggest the presence of motor disturbances in this syndrome. We provide here a literature review on early motor symptoms in autism, focusing on studies on psychomotor issues (tone, postural control, manual dexterity, handedness, praxis). The approach adopted in research to study altered motor behaviors is generally global and there is no detailed semiology of the motor or neuromotor disorders observed in people with ASD. This global approach does not enable understanding of the neuro-developmental mechanisms involved in ASD. Identification of clinical neuro-psychomotor profiles in reference to a standard would help to better understand the origin and the nature of the disorders encountered in ASD, and would thus give new directions for treatment.  相似文献
