Although researchers have identified a multitude of factors that contribute to family participation in mental health services, few studies have examined them specifically for Latino youth and their families in the U.S., a population that continues to experience significant disparities related to the availability, accessibility, and quality of mental health services. Latino youth and their families are at greater risk of dropping out of treatment prematurely and demonstrating poor treatment engagement, both of which have subsequent negative effects on treatment response outcomes. In order to help to guide efforts to improve the accessibility and quality of mental health services for Latino youth and their families, the current paper integrates modern conceptualization of family participation in youth mental health services and provides a summary of contextual factors within an ecological framework (Bronfenbrenner in The ecology of human development: experiments by nature and design, Harvard University Press, Cambridge, 1979). The current review aims to integrate empirical research on the impact of various contextual factors across multiple levels (i.e., culture, community, mental health system, family, parent/caregiver, and child/adolescent) on Latino family participation in youth mental health services, including treatment retention, engagement, and response. Clinical implications will be discussed, and an integrated, conceptual model will be presented. Not only does this model help to demonstrate the way in which existing literature is conceptually linked, but it also helps to highlight factors and underlying processes that health care providers, administrators, and policy makers must consider in working to improve mental health services for Latino youth and their families living in the U.S.  相似文献

Research investigating the link between mental health, crime and violence often rely on populations that are at a high-risk of violent and criminal behaviour, such as prison inmates and psychiatric patients. As a result of this selection bias, the relationship between mental health, criminal and violent behaviour is significantly over-estimated, with mental health being incorrectly linked with violent and criminal behaviours. This study examines the relationship between mental health, violence and crime in a more representative community-based sample. One hundred and twenty-one individuals with and without a mental health disorder reported their involvement in crime and completed an aggression questionnaire. The results revealed that there is no statistically significant difference in terms of violence and crime involvement between individuals with a mental health diagnosis and those without. Moreover, the study did not find any statistically significant associations between specific mental health disorders and specific crime offences. The findings suggest that certain mental health disorders do not strongly contribute to crime violence and involvement. Limitations and implications are discussed in detail.  相似文献

Family processes of communication, mutual support, and sustenance of cultural values can play vital roles in recovery from psychological and material damage in societies afflicted by terror. This is particularly the case when a campaign of terror has specifically targeted family life and its traditions, when the culture is one whose identity has been centered in its families, and when public mental health resources have been scarce. At the end of the 1999 war in Kosova, the Kosovar Family Professional Educational Collaborative (KFPEC) was initiated to counter mental health sequelae of war in Kosova. This initiative focused upon the recovery and strengthening of Kosovar families, rather than the psychiatric treatment of individuals for post-traumatic symptoms. Findings and outcomes from this project may usefully inform the design of other international public mental health initiatives.  相似文献

Sex offenders have been singled out for differential treatment by the legal and mental health systems. This article attempts to inform law reform efforts and criminal justice mental health policy by examining the assumptions underlying differential legal and mental health treatment of sex offenders. These assumptions include the theories that sex offenders are mentally disordered and in need of treatment, specialists in sex crimes, and more dangerous than other criminal offenders. Empirical findings demonstrate that sex offenders are not specialists in sex crimes and are not mentally disordered. Examination of past research suggests that sex offenders are not at more risk than other criminal offenders to commit future sex crimes. Implications of research findings for selective prosecution of sex crime cases, mental health policy, sex offender legislation, and predictions of future dangerousness are discussed. Proposals for future research needs and law reform are presented.  相似文献

The purpose of this article is to present literature regarding perinatal posttraumatic stress disorder and to describe the implications of research findings for occupational therapy screening and treatment in early intervention practice. Changes in legislation regarding early intervention practice and family centered care are presented as a rationale for including maternal mental health screening as a standard practice in the field of early intervention. A case study illustrates symptoms of perinatal posttraumatic stress disorder and the effect on maternal-infant co-occupational engagement. The Model of Human Occupation is used to guide clinical reasoning about potential occupational therapy interventions.  相似文献

People who live in places with high levels of crime and disorder are more likely to experience mental illness compared with those who do not live in these types of place (Weisburd et al., 2018; Weisburd & White, 2019). The increased police presence on high crime streets may also increase the likelihood that these individuals will encounter law enforcement. There is a strong body of literature focused on the relationship between neighborhoods and the physical and mental health of residents (e.g. Arcaya et al., 2016; Duncan & Kawachi, 2018; Leventhal & Brooks‐Gunn, 2003), but there are very few studies that look at the perceptions of people with mental illness directly, particularly as they relate to the environment of the street on which they live and attitudes toward the police. In turn, existing studies generally look at the most serious mental health problems (e.g. schizophrenia), ignoring more common mental health concerns such as post‐traumatic stress disorder (PTSD) and depression. This paper uses self‐report data from a large in‐person survey of people who live on crime hot spot and non‐hot spot streets in order to assess attitudes among a broader group of persons with mental health problems. Furthermore, we examine the interaction between living in crime hot spots and non‐hot spots and perceptions of these residents. Our findings in this broader sample confirm earlier studies that identify greater fear and less trust of the police among persons with mental illnesses. At the same time, our findings suggest that fear of crime and perceptions of police are moderated by living in a crime hot spot.  相似文献

Family psychology is a new and relatively undeveloped discipline in much of the English-speaking Caribbean, particularly in the U.S. Virgin Islands. Family structure and youth behavior in the region have changed dramatically over the past few decades. Given the family-oriented nature of the culture, it is posited that the use of family psychology as an approach to research and intervention may prove to be a rich method to address this cultural metamorphosis. This article examines the potential effectiveness of interventions at the family level when mental health providers are working with youth with conduct disorder in the Virgin Islands. This article is offered as a microcosm of the global changes in family structure and the youth culture that are occurring particularly in the developing world, in part due to the rapid development of telecommunications. Possible roles of the family psychologist in this global transformation are presented.  相似文献

Adolescent mental health is a significant societal concern in the United States. Diagnosable mental health disorders have been reported at rates of 10–20 % among children and adolescents and this does not include adolescents experiencing personal and interpersonal distress not meeting diagnostic criteria. Adolescents who do not respond to traditional mental health services are often placed in residential treatment centers or other out-of-home treatment programs. Outdoor Behavioral Healthcare (OBH) is growing as a viable treatment option for adolescents who struggle with emotional, behavioral or substance related problems; however, questions have been raised about how to integrate the family into an OBH treatment setting. This article describes a case study illustrating how techniques from Narrative Family Therapy can be used to accomplish this integration, and offers a view of using Narrative Family Therapy to further involve families in the treatment and post-treatment process in an OBH program.  相似文献

Schizophrenia spectrum disorders are rare in childhood and little is known about their psychosocial treatment. Relevant findings from the adult and child literature are reviewed. The case of 10-year-old “Michael” is presented, who participated in a randomized clinical trial of a psychoeducational family treatment for mood disorders. Following treatment, quantitative and qualitative data show that Michael's mood and social functioning improved, utilization of mental health services improved, the overall family emotional climate became more positive, and his parents felt supported. He no longer met criteria for a mood episode but continued to display psychosis. This family's response suggests that group psychoeducation may be an effective treatment for families of children with emerging schizophrenia spectrum disorders.  相似文献

In this study, the author investigated differences in Japanese and American college students' tendencies to advise a hypothetical rape victim (their sister) to seek help from police, family members, or mental health professionals. Japanese students tended to encourage the victim to seek help from her family members, whereas American students tended to encourage her to seek help from police and mental health counselors. Cross-cultural discrepancies were marked by the following factors: (a) feelings of shame moderated advice to seek help from police; (b) minimization of rape mediated the likelihood to advise the involvement of police and mental health counselors; (c) attitudes toward mental health counselors mediated advice to seek help from them; and (d) the type of rape (stranger vs. date rape) moderated advice to report the crime to police.  相似文献

This paper reports a collaborative project, developed jointly by a group of community mental health workers in association with a specialist team commissioned to initiate family work throughout a central London mental health provider. The result of this project – The 'Mental Health Matters Workshops'– was a series of day workshops for patients who had experienced a major mental illness, and their families, carers and mental health workers. The positive impact of the workshops on attenders is reported, as well as the development of a 'multi-family' and social network culture.  相似文献

Mental Health professionals are challenged with utilizing innovative treatment models to meet the needs of diverse communities. Enhancements in interventions have led to a reconceptualization of the role of mental health professionals, specifically, family therapists. This paper presents a collaborative, home-based model for working with children and adolescents involved in foster care. We begin by examining literature on home-based therapy. This review provides a framework to understand the need for a culturally responsive approach. A collaborative, home-based approach is presented along with unique issues and guidelines for practice. We examine the knowledge base for community-based mental health services for youth and their families. A brief historical perspective on the development of community services is presented, and the fundamental components of a comprehensive child mental health service system are described. Outcome studies for service components as well as the service system as a whole are summarized. Research related to the context of treatment, i.e., service setting, cost, is presented. The authors conclude that a research base for child and family services is emerging, but there is still a paucity of well designed studies that address the complexities of a community-based system as well as the critical outcome questions that need to be addressed. Finally, promising service system developments are described and recommendations for future research are presented.  相似文献

Integrative Family and Systems Treatment (I-FAST) is a systems-based, home-based, and strengths-oriented treatment model that has been developed and implemented within the community mental health system. Using a single case example, this paper discusses the application of I-FAST to a Chinese family in Hong Kong in which the mother was diagnosed with depression. The paper critically examines the deficit-based approach, widely used in the mental health field, which stigmatizes people with psychiatric disabilities. Treatment implications of families with parental mental illness in a Chinese context are discussed.  相似文献

In order to address the mental health disparities that exist for Latino adolescents in the United States, psychologists must understand specific factors that contribute to the high risk of mental health problems in Latino youth. Given the significant percentage of Latino youth who are immigrants or the children of immigrants, acculturation is a key factor in understanding mental health among this population. However, limitations in the conceptualization and measurement of acculturation have led to conflicting findings in the literature. Thus, the goal of the current review is to examine and critique research linking acculturation and mental health outcomes for Latino youth, as well as to integrate individual, environmental, and family influences of this relationship. An integrated theoretical model is presented and implications for clinical practice and future directions are discussed.  相似文献

Might therapeutic jurisprudence, a perspective that attempts to study interaction between the legal and mental health disciplines, be brought to bear effectively with respect to neonaticide, the murder of a newborn infant in the first 24 hours of its life? This is a crime that leads to sentencing that is now rarely therapeutic, rehabilitative, or corrective. An examination of the crime, its motives, and its perpetrators precedes a discussion of ways in which the mental health viewpoint in this matter might be brought to the active attention of the courts in order to promote sentencing that is appropriate to both the crime and the transgressor. Global mental health (GMH) is an emerging field that focuses on the need for culturally sensitive mental health services in low‐ and middle‐income countries (LMICs). While many new initiatives have been established worldwide to understand GMH needs and to provide care in LMICs, family therapists have primarily worked with families in high‐income countries. The few existing family‐based initiatives in GMH focus on psychoeducation and are typically not based on general systems theory. However, emerging trends in family therapy may enable family therapists to impact mental health issues in LMICs. These trends, which are shared interests of both family therapy and GMH, include collaborative care, a growing emphasis on the importance of culture in understanding and treating mental health issues, recognition of the ability of families to support or impede recovery from mental illness, and the use of strength‐based and evidence‐based treatments. This paper describes ways for family therapists to become active in the GMH community.  相似文献
